Mechanic was climbing onto Dozer to make repairs when EE slipped and fell fracturing ribs.

Struck by powered moving object

The mine superintendent was picking up employees to take them to their equipment to start the shift. After stopping to retrieve the shovel operator, the superintendent put the truck in gear and started to pull forward. As EE did, superintendent bumped the IE, knocking IE to the ground. The IE was diagnosed with a fractured vertebrae after the incident.

IE was dismounting a piece of equipment when EE twisted EE's knee. EE continued to work until surgery was performed on 6/26/20 on EE's knee to repair a torn meniscus.

Fall from machine

IE was attempting to dismount from lube truck but a hand slipped and they fell to the ground. IE reported the incident at the next shift when they complained of back pain.

IE WAS ENTERING THE CAB AREA OF DOZER WHERE THE FLOOR BOARD COVER HAD BEEN REMOVED FOR MAINTENANCE AND SLIPPED LANDING ON RIBS.

IE HAD STOPPED TO CHECK THE FUEL LEVEL IN DOZER. WHEN REMOVED THE FUEL CAP AND SHINED THE FLASH LIGHT TO LOOK IN A FLASH FIRE OCCURRED.

IE WAS WALKING UNDER CROSS UNDER GUARD AND KNOCKED THE HARD HAT OFF. THEN WENT TO PICK IT UP AND RAISED UP AND HIT CORNER OF GUARD.

IE WAS REMOVING A TOWING ROPE FROM INOPERABLE EQUIPMENT WHEN THE CABLE FLIPPED AND STRUCK ABOVE THE EYE.

Miner was assisting mechanic to find an air leak when employee stepped up on the catwalk and the exhaust air burned employee's face.

Miner was stepping off of the last step on the end dump and fell to the ground landing on back.

Miner was attempting to flip a 4x8 sheet of metal onto the forklift. Miner's foot slipped causing miner to fall on another sheet of metal.

Miner was pushing rock with a dozer when the blade hit a big rock causing the dozer to stop and tighten the seat belt straining employee's back.

Miner was exiting the truck and when miner ducked under the mirror lost hard hat. Miner reached for hard hat then the door swung open hitting the safety chain. The chain apparently broke and the miner fell off the truck.

Miner was performing maintenance when stepped on pipes and foot slipped. EE attempted to catch himself with hand. EE then felt pain in right wrist.

Operator was traveling down an incline that had been spot watered by another water truck and started sliding. When he corrected the water truck he traveled on dry ground and the truck turned over on its side.

Miner was trying to adjust the seat on Cat Scraper. When miner pushed his weight forward to move the seat the seat did not move. Miner felt pain in his right shoulder. Continued to work the rest of the shift thinking that it would get better. Miner returned to work the following morning and worked the entire shift but still felt pain in right shoulder.

Miner was hooking a hose to a pump and using his hand to hit the dog ears on the connector. The dog ear had a sharp piece on it causing a puncture wound.

Employee was climbing out of (it14) loader unit # 1095. He stepped on a frozen clod of mud twisting his left ankle and foot.

Employee was working on feeder belt under plant hopper. They were replacing belt and pulling it through. He hurt his right shoulder. Pulled muscle or tear.

EMPLOYEE WAS COMING INTO MINE ON HAUL ROAD AND HIT A THE REAR END OF A SEMI TRUCK WAITING IN LINE ON THE HAUL ROAD TO BE LOADED. EMPLOYEE MAY HAVE FALLEN A SLEEP. THE EMPLOYEE HAD INTERNAL INJURIES. INJURIES TO THE LIVER AND INTERNAL BLEEDING.

Employee was opening a drain valve on a water pump and it was hot. When he opened the valve hot steam hit his right hand causing second degree burn on hand.

Employee was using a grinder with a cut of wheel to cut the ends off of a cable that was looped and woven back together. Employee said that he didn't know what happened but he cut his left forearm with the grinder requiring 9 stitches.

Employee was going to operate an end dump unit #1194 he had just started to climb the ladder when his left shoulder popped and he was in a lot of pain, he fell to the ground but said his pain was all in his shoulder. left shoulder sprain with possible derangement.

Employee was working on a D10 Dozer he had a 6 foot ladder leaned against the push arm. He started to climb the ladder and was about five feet off the ground he said the ladder tipped over with him. He fell hitting the steps and the push arm and falling to the ground. He said he was all right. later it was found that he had broken ribs punctured lung and punctured spleen.

Two employees were trying to get to a sheet of metal in a metal rack, when two pieces of metal shifted in the rack and caught one employee hand in between the sheets of metal pinching his left hand also caught his right foot. Contusion and abrasions to left arm.

Employee was cleaning out the batter box on scraper. the wind was blowing very hard that day and dust blew dirt up under his safety glasses. Employee went to Imed 2 days later and had his eye washed out and eye drops.

employee was loading the weed burner in the back of unit 2303 pickup truck. He slipped on the thawing ground and fell on his back and said he must of fell on a rock or piece of coal hurting his middle back. worked until 01/14/2014 then went on light duty 01/15/2014.

Employee was working on setting a pump unit #1607 in the southwest coal pit when he sliced the pinky finger on his left hand. He went to IMED on 10/9/2013. With his arm hurting. Worked part of this day.

EE was in unit 1113 a Volvo gob truck hauling slurry from wash plant to slurry pit. While in transit employee started sliding on wet roadway over correcting and applying retarder the bed slid around the cab and he hit dry ground and turned over the bed, which laid the cab on its right side. Water truck had just watered road. Operator had seat belt on, ankle sore and back sore.

EE was installing a cleaning lip on a loader. EE hit the lip with hammer causing a metal fragment to enter his right arm. EE went to E.R. EE went to another doctor and had the object removed on 8/28/13 and missed one day of work.

Employee said that on 6/5/2013 while replacing a bucket elevator he injured his back but did not report it. Employee said while shoveling coal on 6/17/13 from under a conveyor, his back popped again and was hurting. He went home at this time.

Employee was operating loader #1044 in coal yard pushing up coal on the coal piles. When he backed down off a pile a large piece of coal got under his right front tire and he stopped. The chunk of coal burst jarring the loader and injuring his back. This is an aggravation of a problem he has had before. We were not notified of accident until 6/27/2013.

Employee had gotten out of the IT loader unit #1095. He tripped and fell to the ground hurting his left shoulder.

EE slipped while he was getting off of his dozer (unit # 1240) he hit his right side on the blade arm. He had sharp pain in his lower rib area and went to IMED. Had right rib contusion.

Employee was installing a pin into a hitch on a scraper unit No. 1122. Employee was hitting a pin with a sledge hammer. A piece of the pin broke off and hit him in the left thigh. (Had metal removed and 9 stitches).

EE was getting on unit #1140 end dump. The first step was high and when he pulled himself up onto the ladder he felt something pull in his right arm. Later he could not lift his arm about halfway in the air. EE worked the 17 went to Dr on 18 and was not scheduled to work that day. Not scheduled on 19 and 20 went back to Dr on 21 and released to work with no restrictions on the 22.

EE was climbing into the back of a grease truck when he felt something pop or move between his shoulder blades. He was pulling himself up using his Rt arm/hand to grip a handhold above his head. He experienced immediate pain in his spine and numbness in his right arm and hand. Went to Dr on 5/17/12 and went back to work no restrictions. Went back to Dr on 5/22/12.

EE was rolling up 4"" water hose and he heard something pop in his right shoulder. He was exp. alot of pain and did not have much movement in shoulder.

Victim was leaving the coal yard. He stopped along the road and said he didn't feel well. Other truck drivers stopped and said to call an ambulance. (Victim had a possible stroke)

Employee was working on unit #1144 end dump. He was working in the cab while taking apart the fan box. his tool slipped in the bedding and the sharp point went in to his left hand requiring stitches.

EE was removing roller bolts from 3600 shovel unit #1019 under carriage. He was using an 1"" drive impact gun. When he got through he felt pain in his right shoulder.

EE said he hurt his back while fastening the compactor down on the lowboy for transportation to a different location.

EMPLOYEE WAS SERVICING DOZER #1244 A D11CD. HE SAW NO ANTIFREEZE IN THE SIGHT GLASS AND LOOSENED THE RADIATOR CAP ONE CLICK. ANTIFREEZE STARTED SPRAYING OUT FROM UNDER THE CAP. THEN THE CAP BLEW OFF. HE WAS STANDING ON THE SIDE OF THE RADIATOR AND JUMPED OFF TO THE GROUND. EMPLOYEE BROKE HIS RIGHT ANKLE AND HAD SECOND DEGREE BURNS TO CHEST AND ARMS.

EE was operating unit #1882 backhoe, EE stepped off the machine when he was getting off the backhoe his foot slipped off of the step and he twisted his knee. At that time the EE refused medical treatment and cont. to work. His knee began to hurt him more and he wert to doctor and has continued to work. Will have surgery on 2/6/12 and will be off 4 to 6 weeks.

Exiting from a 740 end dump to meet with supervisor, victim informed supervisor he had fallen from bottom step dump. Victim was okay and then assigned to operate dozer. After appx. 15 min of dozer operation victim informed supv. of numbness of hands. Sup again met with victim and while transporting to office victim started with convulsions. An ambulance was summoned.

While climbing a ladder on the 740 end dump to start, victim slipped and hit his head on a brace and cut his forehead requiring sutures.
