IE was treated on 7/13/20 for flash burn of the eyes, but did not notify mine personnel until after being treated. After receiving doctors notes on 8/3/20 it was determined this was a reportable incident as medications were administered to their eyes.
While loosening a radiator cap on a grader, hot antifreeze caused blisters on IE right hand. IE received medicine and was taken off work for 2 days while blisters healed.

Miner was hauling gob from the prep plant when EE suddenly noticed a coal hauler had slowed in front of EE. When miner made contact with coal hauler EE swerved to the ditch and turned over on side.
Miner slipped on ice and felt a pop in left arm.

Miner was preparing to wash pick up truck with the water canon. Miner had placed the transmission in neutral and set the park brake. Miner went to rev up the throttle for the water canon when the truck slipped into gear causing the truck to lunge and the miner fell off.

Employee was walking from his car to his equipment when he was going to work. He stepped on a rock with his right foot and twisted his right knee. Knee was sore and hurts to put pressure on it.
EE was getting into dump truck when his foot slipped off the tread on the step on the fuel tank causing him to fall and grab the hand rail which hyper extended his right arm and injured his right shoulder. (EE worked until 01/29/13 when he went to doctor)
Employee was using a cutting torch when his left glove caught on fire causing a burn to his left hand between his thumb and index finger. (Second degree burn).

EE was walking down the steps in the wash plant and slipped. He tried to catch himself on the hand rail on the steps. He injured his right shoulder.
EE was removing a guard from a tail roller when the guard sprung loose and hit his thumb. Thumb on right hand. (EE did not report accident until 12/15/11).
Employee was setting a water pump unit #1589 with a utility loader. The pump needed jumped. When he hooked up the jumper cables a 3 inch flame started on the cable on loader. When employee went to get fire ext. he stepped in a hole in mud and twisted his right knee.
EE was carrying a coal crusher screen out of the parts trailer, when he stepped on something and twisted his ankle and hurt his back.
EE was working on 1800 shovel #1018. He was pulling a cable through the track frame. He got his first finger on his right hand smashed between the cable and a pin. Finger needed stitches.
Employee was walking across a patch of ice which had snow on top of it. He lost his traction and slipped and sprained his left ankle. He went home and his ankle had swollen up. Then he went to the emergency room.
EE was walking on refuse walkway up to the refuse bin. He was pinging the rollers with a hammer when the hammer got caught in the belt and pulled his hand and arm between the belt and roller. Trauma/compression injury left arm.
EE was working on an engine on unit 1322 lube truck when and was pulling on wrench and his right shoulder popped. EE didn't go to doctor till the next day said that it hurt more the next day. Sprained right shoulder. EE worked on the 1/6/11 and 1/7/11.

EE was working on core drill and was pulling pipe from the drill hole. When he pulled on the pipe he strained his left wrist.

EE was checking tanks on 1323 grease truck. When he was exiting rear door on bed he slipped to the ground, first on right foot and when his foot made contact with the ground his heel was up against the bulk antifreeze meter. When the EE brought his left foot down to the ground his balance was off and he fell and hit the bulk pump with his lower back. EE has lower back pain.
Employee had gotten out of drill and picked up an orange cone to mark his drill hole rows. When he turned he stepped on a rock and twisted his right knee.
Employee had parked the water truck, while walking to pump he tripped on a rock and fell on his left knee, then on his left rib cage. Went to doctor at 11:30 am, contusion left ribs.
Employee was operating end dump #1192 and seat broke. He complained of back pain and was taken to hospital Emergency room. 190 ton truck.
Employee had taken the hitch off of the it14 loader. He was going to put it in his grease truck when he lifted the hitch he felt a pull in his back. Back Strain.
Employee was climbing up the ladder on unit 1199 end dump he had set his lunch bucket on the cat walk his lunch bucket tipped over and dropped a Pepsi onto the ground, he climbed down and got his pepsi when he climbed back up he had mud on his hand and lost his grip on the hand rail falling to the ground. Hurting his knee, he fell about 20 feet.
Employee was cutting out minor bogie pins from dozer with a torch when slag popped back and got under his face shield and safety glasses and got in his left eye. He went to eye doctor and object was removed. Was given antibiotic and returned to work.
EE was using a grinder and grinding on a shovel bucket. He had an appointment with an eye dr later that day. When he went to the eye dr he had something in his left eye, he was sent to an eye specialist, who got a piece of metal out of his eye. EE was wearing face shield and safety glasses. It was very windy that day.
Employee was exiting his dozer. Employee was getting out of his dozer and he stepped from the platform of the cab to the next step. His foot slipped and he hyper extended his left knee.
Employee was cleaning tracks on a dozer. He had cleaned the tracks on one side and was walking around the back of the dozer behind the ripper and slipped when he stepped in the ripper hole. He fell and landed on his hand and wrist.

Employee was driving a 190 ton end dump. He was on the fill dumping his load. he said that the truck bed slamed back and jerking his neck. He was dizzy, vision was blured, and light headed.
It was dark (before daylight). Driver drove off edge of road and the truck laid over on it's side.
EE was getting out of a utility loader (unit #1026 IT14) when he slipped. When he tried to catch himself, he jammed his thumb on his right hand (sprained right thumb).
Employee started out with a rash on his stomach that moved to his arm and then his face. On Tuesday 6/23/09 he woke with his face swollen and his right eye swollen shut. He does not know what he is allergic to.
EE was driving unit #1199, a 190 ton end dump, across the shovel bench to get a load of rock. He drove through a dip in the road. He hit his knee on the dash. He went to the hospital ER, contusion to knee.
EE was working on stoker plant. EE was on catwalk on second level working on coal chute. EE's foot slipped and he fell, striking his left arm. He had a 5 cm laceration on his left forearm, requiring stitches.
EE was holding up a plate to be welded in a coal chute. He had also been digging in frozen ground, not sure when he hurt shoulder.

Employee was fueling equipment at dinner. He was getting out of the fuel truck and stepped on a piece of coal, twisting his ankle.
EE was climbing down out of the 910 utility loader and stepped on a large piece of coal, causing his ankle to twist over to the ground, causing pain and swelling.
A Mack dump truck driven by EE of Triple C Trucking was traveling north on CR 900 E when a 2000 Mercury passenger car was traveling west on CR 100 S failed to yield right of way to the dump truck. The dump truck impacted with the car in the area of the driver's door & pushed the car off the north west side of the road into a ditch. Driver of car pronounced dead at scene.
Employee was trying to get a pump to pick up prime and he folded the discharge hose to help it pick up prime. As the hose picked up prime it slipped out of his hands and jammed his thumb on his right hand.
Employee was driving a water truck and going down into the pit. When he got to the bottom of the road there was a dip in the road. He hit his head on the air conditioner resulting in a cut to head and receiving 11 staples. He did not hav his seat belt on. (I did not receive this until 5/20/2008).
Employee was checking oil in scraper engine.When he climbed up the ladder, he pulled something in his shoulder. Employee worked until June fourth and he went to the doctor on June fifth and was put on limited duty.
Driver climber on top of the right rear tire on trailer of semitruck to unlatch a tarp strap sliped off the tire, EE's foot landed on a piece of coal and twisted her ankle which blew out her knee.
Contractors truck was parked, he had tarped the trailer and was going back to the truck. Another driver came around the scale house and saw him laying on the ground. He was taken to the hospital by ambulance, possible heart attack.

Employee was installing bushings in shovel bucket. Bushing was crooked & stuck, employee was hitting the bushing with sledge hammer, a piece broke off the bushing & hit him in the stomach entering the muscle tissue. Went to medical doctor on Oct. 25. Released. Went back on Oct. 28. Metal was removed. 2 stitches.
Employee was working on the r.c. screen in the washplant, trying to loosen a bolt with a wrench. The wrench slipped off the bolt causing his left index finger to be cut. He received four sutures.
Employee was pulling a powder truck forward to load the next hole. When he was getting out of the truck he stepped on a rock and twisted his ankle.
EE was climbing down ladder on 190 ton end dump at end of shift. EE said he got muscle cramp in left bicep. Woke up in middle of night & arm was swollen & in pain.
When employee was getting out of the powder truck his foot slipped off the step and he twisted his lower back. His boots were muddy. It had rained earlier that morning.
Employee was setting up water pump & was moving the suction hose and discharge hose. He said he felt a twinge in his left shoulder. Pulled muscle.
EE was helping with changing air filter on shovel. When he went to get off of shovel, he turned his left knee. EE stated his knee was already hurt and that climbing up and down the shovel ladder made it worse.

EE was fueling a fuel tank on the back of an Austin powder bult powder truck. When he finished, he was going to get down, but missed the step, falling to the ground. He fell 3 1/2 to 4 feet. Fractured bones in his left ankle.
Empoyee was coming down the loader on an R120 end dump, was to the bumper, stepped to the next lower strip but missed the step. He lost his balance and fell to the ground, bit tounge requiring stitches. Bruised chest, lost a tooth.
Employee was changing a bulb in a lightplant and got shocked on the hand.
Employee said he was running unit #1176-R190. He was dumping mud. When the bed goes up some time the mud slides out of the bed all at once. Then bed goes up and comes down fast until the cylinders catch up.
Employee was going down the ladder on unit #1196. He said there was a light rain at the time. When he got down to the 3rd step above the bumper there was mud or grease on the handrail, he lost his grip & fell to the ground. He said his shoulder hurt & his lower back. He went on & finished his shift & then went to the Dr. on 7/25/2006.
EE was reaching for ladder the ladder handrail had mud on it when he pulled himself up his fingers slipped causing him to fall off the first step to the ground.

Employee was going down stairs with muddy boots. He slipped, fell and hit his left elbow. He was in washplant going to tool room.
Employee and mechanic were lifting a water pipe off the back of unit #1385 water truck. The chain that was around the pipe slipped, mashing employee's left thumb between pipe and back of the water truck.
Employee was welding on unitl #1244, his left eye started to hurt thought he might have flash burn. He went to emergency room at hospital they removed particles of rust/metal from eye. Employee was not sure if he just got dust in his eye or got something in his eye while welding he was wearing safety equipment.
Employee was climbing up on a scraper when the wind blew dust in his eyes. the dust was from the exhaust on the scraper.
Employee stated that while helping move a dewatering bed from one set of work horses to another, that he had a strange feeling in his back-over the course of the next few dyas. The pain got worse. (was notafied on 9-9-05).

Employee was working on the undercarriage of unit 1230 dozer. He had heated a pin with a torch & he hit the pin with a hammer. A piece of metal got into his eye, he had a corneal abrasion.
EE was attempting to lift a roll of 6"" discharge hose into the 910 loader & bucket. His foot slipped causing him to fall to the ground with one leg in front & one leg behind him.

EMPLOYEE WAS ON THE BOTTOM FLOOR OF THE WASHPLANT. SOMEONE CALLED ON THE RADIO TO LET HIM KNOW THAT THE BELTS ON THE CRUSHER WERE SMOKING. HE WENT TO GO GROUND ONE OF THE CHEMICAL TANKS, SLIPPED & FELL. HE FRACTURED HIS RIGHT WRIST.

THE TRUCK HAD EXTENSIVE DAMAGE TO THE ENGINE COMPARTMENT, CAB, TIRES (FRONT), HYDRAULIC SYSTEM AND ELECTRICAL. DRIVER WAS NOT INJURED. TIRE BURNED ABOUT 1/2 HOUR.
EE WAS CLEANING OUT THE CAB OF HIS END DUMP. HE WAS BLOWING THE DUST OFF THE FLOOR. SOMETHING BLEW OFF THE FLOOR AND HIT HIM IN THE EYE.
EE WAS DIGGING THE TRACKS OUT ON UNIT #1011 SHOVEL. A PIECE OF MUD FELL OFF THE FRAME OF THE TRACK AND HIT THE FINAL DRIVE AND HIT EE BEHIND THE KNEE.

EE was running a Euclid R-120 end dump. His right leg was hurting him, he could not continue working. He had trouble walking, could not remember injuring himself.

EE WAS OPERATING COMPANY PICKUP ON COUNTY ROAD GOING FROM MIDWAY PIT TO CANNELBURG WASH PLANT. HE CAME TO AN INTERSECTION WHEN ANOTHER VEHICLE RAN A STOP SIGN AND EE RAN INTO THE SIDE OF THEIR VEHICLE. EE WORKED SOME DAYS AFTER THE ACCIDENT. THEN WENT TO DOCTOR AND FOUND HE HAD A BROKEN RIB.
EE WAS IN THE PROCESS OF REMOVING TURN OFF OF THE NORTH BREAK. COLLECTING BELT AND WAS STANDING ON THE FLAT COAL CLEANING BUCKET. HE TOOK ONE STEP TO THE LEFT AND STEPPED OFF THE FLAT BUCKET A ND FELL TO THE GROUND.
EE WAS HELPING MOVE HMC IMPELLER ONTO A CART AND PULLED SOMETHING IN HIS LOWER BACK.
ACCORDING TO EE, HE WAS PULLING ON A TOW CABLE AT WORK & SAID HIS BACK HURT SO BAD HE COULD BARELY WALK. HE STATED HE WAS NOT SURE WHEATHER THIS WAS WORK RELATED OR NOT. I WAS INFORMED HE STAR TED CORTIZONE SHOTS IN HIS BACK. ON AUGUST 5 & 6 HAD INJECTIONS.
CLIMBING DOWN FROM THE FUEL TRUCK AND HIS FOOT ESLIPPED OFF OF THE STEP. HE BRUISED HIS RIGHT LEG.
EE STATED THAT HE MIGHT HAVE BROKE HIS FINGER WHILE HE WAS STRINGING WATER HOSE BECAUSE HE FELL DOWN. ALSO HE MIGHT HAVE SHUT IT IN A DOOR.

EE WAS CLIMBING DOWN FROM THE WATER TRUCK, HIS FOOT SLIPPED OFF THE LAST STEP AND HE FELL TO THE GROUND, TWISTING HIS ANKLE AND FOOT.
WHILE RUNNING 988 LOADER EE PINCHED A NERVE IN HIS LEFT SHOULDER.
EE WAS STANDING ON THE TOUNG OF A LIGHTPLANT, TRYING TO ADJUST THE TOWER, HIS FOOT SLIPPED OFF, HE FELL BACKWARDS AND HIT HIS LEFT LOWER BACK ON THE JACK STAND.
EE WAS DRILLING ON THE BENCH FOR SOME UNKNOWN REASON THE DRILL RAISED UP ABOUT 10' AND FELL ONTO ITS RIGHT SIDE. EE JUMPED FROM THE DRILL, HAD THE WIND KNOCKED OUT OF HIM AND BRUISES.
EE WAS UNLOADING A PALLET OF SHOVEL TEETH ADAPTERS. THEY WERE IN A CARDBOARD BOX ON A PALLET. HE WAS PULLING ON THE BOX AND THE SIDEOF THE BOX PULLED OUT. TWO OF THE ADAPTERS ROLLED OUT OF THE BOX. ONE HIS HIS LITTLE FINGER, BREAKIKNG IT BELOW THE FIRST JOINT AND LACERATING THE FINGER.
