Roof Fall, 3 South Section, #5 entry x-c 12, area started working and was dangered off, 2hrs before it fell, size was 60'L x 18' w x 13' thick. Area was bolted with 8' tension rebar bolts.

Found Sun Night on Pre-shift for 3rd shift, roof fall 3 West, #5 entry, x-c 9 in the intake airway, 30'w x 50'l x 12' thick, area in same slip as last fall in #5 entry x-c 6. Area was dangered off.

Roof Fall, 3 west, #5 entry, #5 x-cut, fall was in intake in the intersection in a fault area mined back on 8-6-07. The fall was 50' long x 30' wide x 10' thick.

A roof fall was found on weekly exam in the 2 west bleeder in the #2 entry x-cut 15. The fall was 50' x 50' x 8-10' thick. The bleeder entry is being traveled in the #1 entry.

Continuation of roof fall the other day in 1st left, fell on across into other intake travel way and primary escapeway 20""x20""x8-10' thick, area was bolted with 6' resin bolts around 1998. New escapway made and fall is to be cleaned and bolted

A roof fall in the intake airway that is not used for travel or is not an escapway and is in the 1st Left panel mined around 1998 with 6' full grout resin bolts. The fall was in the #3 entry x-cut 10-11 and was 20'x50' long 3-6' thick. Area will not be cleaned up, just dangered and cribbed.

Roof Fall #3 room in 2 West #2 entry x-c 1, area was bolted with 6' resin bolts and was 30' x 30' x 7'-10' thick in intersection. Submit new Roof Control Plan.

Roof fall discovered on weekly exam in the 2 West 1st Room #5 entry x-c #2, 30'x30'x8'thick, fall had already been dangered and timbered off before falling, mining is now in the 3rd rooms of 2 West.

Sunday Night 2-12-07, 3rd Shift found a roof fall during the preshift exam in 2 West, outby section, #3 entry belt neutral intersection, xc19, was bolted with 6' resin bolts, mine had been idled and fall was cribbed and dangered off. Fall was 30'x30'x8'hi

Accident type, without injuries

Msha issued neg findings 6-1, event 4260825. Then on 6/18 issued #2 on same event, that there was an ignition concerning scsr. Cont.miner punctured while loading gob onto a scar & caught fire & extinguished within 5-7 min. This is not considered an ignition & disagree with Msha's 2nd paper after closing original out on 6-1. Being reported only to abate 7284820 citation 6/18.

Over-exertion (Not Elsewhere Classified)

Employee reported injury 12-27-07 but did not miss any work until 4-17-08, still hurting and went to doctor. Employee stepped through a check curtain and twisted right knee.

Fall onto or against objects

Doing pre-shift in #7 entry and slipped and fell hitting right hand finger on hammer or caplight attached to mining belt.

Struck against stationary object

Employee was operating shuttle car when he came to a 6"" ledge and hit his head on canopy then his neck and back started hurting. Reported to operator on 11-19-07 but never missed any work until 11-26-07 over incident.

Fall down stairs

submitted on 10/19/2007, mistake on filing submission #531688, resubmit. Had been raining earlier in day and employee walked out of office onto aluminum steps and feet slipped out from under him and he fell landing on rear midway down steps, hurting neck & back.

Injured was installing roof bolts using roof bolter and had hand in a pinch point area and when he lowered the boom it caught his hand and cut his pinky finger on left hand.

Struck by rolling or sliding object

Injured was removing blocks for a regulator at a stopping when a section of rib fell and knocked him into the side of trickle duster, hurting leg.

Caught in, under or between a moving and a stationary object

Injured was installing CLA Tips on continuous miner when his 3/4"" ratchet slipped off bolt smashing finger against CLA, breaking finger.

Injured was setting a timber and hit his thumb with a hammer.

Struck by falling object

Employee was operating offside roof bolter, drilling a hole when a loose piece of rock fell down along drill steel and fell on his right hand.

Caught in, under or between a moving and a stationary object

Submitted online 10/16/07 submission # 526437 but had mistake filing so resubmit. Roof bolter operator slipped while moving the drill station, right hand was on canopy post and left hand hit canopy lever, dropping canopy and cutting little finger on right hand.
