Area was bolted with 8ft TR and 12 foot cable bolts and had some supplemental support extra bolts, straps, Area was in a fault zone, ventilation was not affected. Stoppings were intact, area is dangered off on all sides and will not be cleaned up. Size 20ftX30FtX10Ft high.

Employee was bent down making a cable splice with back turned to the rib. A rock fell hitting the employee in the back, resulting in a fracture to back. The size of the rock of 2' x 3' x 9""-14"" thick. Incident occurred in the 3rd South Panel Entry 5 Crosscut 13.
Employee was assisting to flip over a piece of belt to roll up when the employee's channel locks slipped off belt, employee felt a strain to the right shoulder.
Employee slipped and fell while carrying a roller frame. The roller frame fell on top of the employee causing a fracture to left rib.
While hanging structure, the employees fifth digit (pinky finger) on left hand got caught between the belt and belt chain, causing a laceration requiring stitches. Employee was not wearing gloves at the time of incident.
Employee let drill pot down and drill steel fractured finger.
