Over-exertion in wielding or throwing objects

Employee was exploratory drilling at property adjacent to HM Booneville site. Employee was operating drill connecting continuous flight auger stems. This involves insertion of a metal clip/pin into the connecting points on the auger stems using a hammer. Weight of the hammer is 14 oz ball type. In the back swing motion of hammering employee felt pain in right elbow.

Employees were removing pipeline off the water from the dredge. Employee was removing bolts from the pipeline flange, after the bolts were removed employee made sure everyone was clear and swung the pipeline behind him and one of the employees walked forward and was struck in the right ankle.

Employee was disconnecting dredge pipe on the shoreline. Employee was standing next to pipe removing the last bolt from the flange. When bolt was removed the pipe shifted and struck the employee in the lower leg causing contusion.

Fall to the walkway or working surface

Employee slipped in the parking lot while on the way to his vehicle and fell onto his right side/shoulder.

Employees moved snow off of a concrete pad and it exposed Ice underneath the snow. Employee was heading to use the skid loader to spread out some sand to cover the Ice for traction and to help melt the ice. Employee walked across the ice to go get the loader and slipped and fell on his right shoulder.

Fall from machine

Slipped on dozer tracks dismounting machine. Tracks were snow packed due to weather conditions that day - Cut lip - scratches on back left side and two broken ribs left side.

Employee tripped when he stepped between the floating pontoons.

Caught in, under or between a moving and a stationary object

The crew was re-attaching the counterweight to the 390 cat excavator and employee got this left Index finger pinched between the bolt head and the washer.

EE was using an electric grinder on a wheel loader bucket. He was grinding rust away to install a new cutting edge. While grinding the grinder wheel caught a rise in the metal and the grinder kicked back into ee hand. His index finger was caught between the grinder wheel and the wheel guard causing a deep cut.
