THE CRUSHER TOP HAD BEEN SET ON THE GROUND BECAUSE WE WERE DOING SOME MAINTENANCE INSIDE CRUSHER. EMPLOYEE WAS CLEANING OFF THE BUILD-UP ON THE CRUSHER TOP WHEN THE CRUSHER TOP SLIPPED INTO TW O PIECES. (WE DIDN'T KNOW IT WAS 2 PIECES) AND CUT HIS RIGHT THIGH REQUIRING 24 STITCHES & A HIARLINE FRACTURE IN RIGHT ANKLE.

Employee was digging in the pit with the WA700 pit loader and hit a toe and it jarred him straining his back. He was wearing his seatbelt.

THE INJURED EE WAS ASSISTING A CO-WORKER IN TRYING TO LOOSEN A TIGHT BOLT ON A 4248 JAW CRUSHER. THEY WERE USING A LONG PUNCH & A HAMMER TO TRY TO DRIVE THE BOLT OUT. THE EE'S HAND SLIPPED JUST AS THE OTHER EE SWUNG THE HAMMER TO HIT THE PUNCH. THE HAMMER STRUCK THE EE'S RT.RING FINGER.

Employee was trying to open the #2 bin door for washed stone and pinched his left hand below the thumb requiring 6 stitches
