Rock was lodged inside cone crusher. Employee got inside cone crusher and was twisted around to grab a rock. While pulling at a rock his back made a popping sound. Emploee was treated at local clinc; was diagnosed with a pulled muscle.
Injury occurred when employee stepped with left foot from primary shack and muddy boots slipped off the step. Right leg and foot got caught between building and first step, twisting knee and leg. Sprain of knee and leg.
Employee was assisting in removing the bushings from the bucket of the excavator (changing over to the hammer) and he cut his right index finger requiring 5 stitches.
Employee was grinding the end of a bolt and the grinder slipped, cutting his right hand, requiring 4 stitches.
