Accident type, without injuries

An examiner found a roof fall at x-cut 11, entry number 1, M4 zone. The roof fall was 25' long, 20' wide and 10' high. No miners were hurt or trapped and ventilation was not affected. The area was supported with 6' fully grouted roof bolts prior to the accident.

A roof fall was found in numbers 1 and 3 entries at cross cut 20 in our M4 zone. The falls were approx. 20' X 20' X 15' high. The falls did not affect ventilation, passage of miners or cause injury. The falls occurred in the intersections only. The area had been supported with 6' fully grouted roof bolts. The MSHA hotline was notified of the accident.

During an examination of the intake escape ways by a company and federal ee a roof fall was found, in the pillar line at x-cut 39-40 on the M3 zone. It was supported with 6' fully grouted roof bolts. The fall was 20' long, 17' wide and 8' high.

A roof fall was found by the asst mine manager at 3:18am and reported to MSHA at 3:30am. Fall is located at X-cut 43, Entry #3 of the M-3 zone. It is 20' side, 25'long, and fell 12' high. The area was supported with 6' fully grouted roof bolts and was developed on 8/15/09. This fall did not affect ventilation, passage of miners or cause personal injury to any miner.

Accident type, without injuries

Roof fall was found while examining the return air course on MMU-001. The fall is located at X-cut 20 #3 entry of the M3 zone. The is 40' long 17' wide 7' in thickness. Area was supported with 6' fully grouted roof bolts and developed on 6/29/09. No injuries, disturbance of air floe, or blocking of passage occurred with this roof fall.

Struck by falling object

The EE was helping the continuous miner operator set up for the next cut. A piece of rock dislodged from the mine roof, striking EE in the head.

Struck against stationary object

The employee was attempting to dislodge rock dust from a machine rock duster. During this procedure the employee slid his hand across a metal burr cutting his hand.

The employee was helping the miner move to the next cut and a rock fell from the mine roof striking the employee in the head. The piece of mine roof measured app. 4"" x 10"" x 12"".

The employee was eating lunch at the section transformer. A miner cable fell from the top and struck the employee twisting his back. He worked for two weeks and went to the dr on 5/19/09.

On 2/28/09 the EE was getting off a mantrip and twisted his knee. It has since started causing him pain and had minor surgery on 5/14/09.

The employee was changing a boom lift hose. A wrench slipped smashing the employee's finger.

The EE was helping the continuous miner operator pull up miner cable. The operator failed to stop when flagged and pulled the cable down striking the ee knocking him to the ground.

The ee was supporting mine roof with a twin head bolter. He had his hand on the starter steel and used the hyd. to insert the pusher steel. The steel snapped back striking the employees hand.

The employee was changing a hydraulic hose on a continuous miner. The head pin plate was in an upright position to allow access to the damaged hose. The head pin plate fell on the employee's hand.

The employee was installing roof support. He dropped the boom down on his knees.

The employee was assisting other miners who were advancing the section belt up during a non-producing shift. The miners were in the process of pulling a new piece of mining belt with a 3/8"" wire rope and a battery powered scoop. The chain that was holding the wire rope broke causing the wire rope to sling back striking the employee in the hand.

The employee was spotting roof bolts in the haul road. While installing roof bolt in the mine roof he caught his hand between two wrenches causing a laceration.

EE was entering a diesel scoop and bumped his knee on the cab.

The ee was pulling up miner water hose. A piece of mine roof dislodged from the mine roof and struck the ee on the foot.

The employee was running a continuous miner. He had positioned himself between a battery coal hauler and the rib. The Hauler slid over and smashed his hand.

The employee was installing roof bolts. A piece of mine roof dislodged from the roof and struck the employee

Struck by flying object

The employee was cleaning a dryer with a water hose. A piece of coal struck the employee in the eye leaving a small partical of coal embeded in his eye.

The EE was installing a man door in a recently built brattice. The man door slipped and as the ee grabbed it a sharp edge cut his hand.

The employee was installing floor to roof support. As he was securing a cap piece in the support his hammer slipped and hit his thumb.
