Elevator showed a brake limit switch fault. The brake adjustment had come loose.

An unintentional roof fall occurred on the ME beltline at crosscut 48. Earlier in the month the area outby was found with a crack in the roof. The said area was cribbed, and rail barred. It is believed that the pressure in the top shifted and caused the fall at crosscut 48.

An unintentional roof fall happened in the face of entry 8 located between crosscut 21 and 22. There was a slight start in elevation change and the coal seam was starting to pinch off.

An unintentional roof fall has occurred on the S3B beltline between crosscuts 37 and 38. The fall measured approximately 46 feet in length, 17.5 feet in width and 7 plus feet high. The roof fall occurred on the idle weekend. An action plan has been submitted and approved to start rehabilitation of the affected area.

An unintentional roof fall has occurred on the 1 North A at xc41 in entry 6. The fall was approximately 40' X 17 1/2' approximately 12'-14' high

During a non constructive test, a distortion was discovered on one of the ropes. The hoist was removed from hoisting men underground.

Elevator went into alarm for a fault, after further investigation one of the brake pistons was not actuating properly. At that time the elevator was taken out of service and the repairs began to correct the problem.

Injured was found in entry 11, caught between the left rib and the horn of the miner.

Caught in, under or between running or meshing objects

While helping to get #14 mantrip started, the injured was resting hand on top of the radiator. When the mantrip started the fan severed the right index finger.

Caught in, under or between a moving and a stationary object

Injured was unlatching fence hook on ATRS pad and got fingers caught between ATRS pad and hook handle. The ATRS was being raised when the pad came in contact with the top and the hook latch smashed fingers

Injured was climbing out of a coal hauler when injured's feet slipped and felt pain in right inner thigh. Injured was diagnosed with a groin strain on 6/27. Injured was seen again on 7/14 and 7/28 for same injury. On 7/28 a CT scan was ordered. On 8/9 it was determined that the injured needs hernia surgery.

Injured was attaching two drill steels together. The steels were not completely connected when hydraulic pressure was applied. This caused the steels to come apart and strike the injured on the hand.

Injured was cutting down 3"" waterline to reclaim. When IE cut the wire holding up the waterline the roof fell out around the bolts, landing on IE pushing IE to the ground. IE cut right hand open and fractured L5.

Injured was getting the next hog panel to install that was leaning against the rib, a rock came loose from the edge of a previously installed panel and stuck injured in the back of the head.

Over-exertion in lifting objects

Injured was building cribs in the intake on Unit #2 xc42 when they felt something pull in the stomach area. On 1/26/2023 a CT scan was done and showed the injured had a hernia. Surgery is scheduled for 2/22/2023.

Struck by falling object

Injured was tramming a roof bolter on unit #2 entry 3 crosscut A. As they turned the corner the roof bolter became a little unstable from a drop in the mine floor. As the injured reached for the top of the canopy to help with their balance a crossover pad fell from the canopy and struck injured on top of the hand. This resulted in a fracture of the lower part of the middle finger.

Injured was working in the beltline when EE stepped in a hole and twisted EE's knee. On 12/06/2022 MRI results showed a meniscus tear in the right knee. Surgery is scheduled.

Injured was lifting discharge line to the mine roof. The pipe shifted and the injured lost footing causing a popped feeling in the knee. An MRI on 12/2 showed that the meniscus in the right knee was tore. Surgery is scheduled for 12/16/22.

Injured was loading 6' timbers into a scoop bucket when they felt something awkward in the shoulder area. After weeks of physical therapy, the ortho suggest that rotator cuff surgery will be needed. Surgery is scheduled for 5/5/2023.

While setting timbers in the M3 Intake, the injured stepped in a hole and strained the right knee. On 11/22/2022 an MRI showed a torn medial meniscus of the right knee.

Over-exertion in lifting objects

Injured was at the M11 seal area building walls. At the end of the shift, EE felt a slight pain in the abdomen area. The next day there was swelling in the abdomen. On 11/22/2022 a CT scan showed EE had a large left inguinal hernia.

Fall onto or against objects

Injured was walking toward the M10 belt drive when injured tripped over a belt block that was left behind. As injured lost balance the shoulder came into contact with the coal rib. An MRI on 11-10-2022 showed a tear to the rotator cuff. The injured was placed on a 25lb work restriction. On 1/16/2023 they scheduled surgery for 1/18/2023.

Struck by powered moving object

Injured had reached for roof plates on the bolter when the operator trammed forward. The left foot was in front of the tire as the bolter moved forward. Injured was diagnosed with a fractured left great toe.

Injured was connecting steel to steel while roof bolting. The steels were not properly aligned when pressure was applied. The steel popped the injured on the back of their hand. EE was diagnosed with a fracture at the base of the thumb.

Struck against a moving object

Injured was driving a mantrip on the haul road. Injured was pulling into a crosscut to get out of the way of an oncoming piece of equipment. They had their hand/fingers through the opening of the steering wheel when the tire ran over a rock and the steering wheel spun. Injured was diagnosed with a 4th and 5th metacarpal fracture.

Struck by falling object

Injured was standing beside the roof bolter when a rock fell out and pushed EE's face into the guarding of the side controls. Injured was treated for laceration to the lip and received 9 sutures.

Struck against stationary object

Injured was driving a coal hauler away from the continuous miner and ran over a hole in the road. Injured hit head on the canopy of the hauler and was diagnosed with a mild concussion.

Injured was helping make a splice on the S4-A beltline. When the injured reached for the rivet plunger, another employee was in the process of swinging a 4lb hammer. The hammer struck the injured on top of the hand and broke the left index finger.

Fall from scaffolds, walkways, platforms

Injured was working on the Mine No2 fan. While EE attempting to open the mandoor at the back of the fan, EE lost EE's footing and fell off the concrete ledge (46"" high). Injured fell to the ground and landed on EE's left side fracturing four ribs.

Struck by falling object

Injured was outby the unit spotting hog panels. Injured had just finished hanging fence and injured's partner had backed the bolter up when a rock fell out striking injured in the foot. The injured had a displaced fracture of the fifth metatarsal bone.

Over-exertion in lifting objects

Injured was lifting a 6' timber when EE felt something abnormal in their arm. EE went home after EE's shift and applied ice. The next day EE was still in pain and was sent to the doctor. A follow-up appointment on 7/20/2022 determined that the right bicep had a tear and was going to require surgery.

Over-exertion in lifting objects

Injured was lifting a lid on a coal hauler when EE's right arm gave out and EE fell towards the ground jerking EE's arm. Injured felt pain but kept working and making adjustments to be able to function. On 07/21/2022 the injured was pushing backwards and the pain worsened. It was determined on 07/22/2022 that the injured needed the subscapularis tendon repaired.

Struck by falling object

Injured and EE's partner had just finished bolting the face of entry 5. They were both on the same side of the bolter discussing their next place to go when a rock fell from the corner of the rib and top striking the injured in the ankle/shin area. Injured was transported by ambulance with a broken fibula.

Struck against a moving object

Operator was driving the #2 Dapco down the slope when they encountered a slick spot due to the humidity. Operator slid until it was stopped by the safety barriers at the bottom of the slope.

Struck by falling object

Bolter operator backed up to put a hog panel on the ATRS. While putting panel on a rock fell from the top and hit the ATRS causing the rock to flip back and strike the hand between the canopy and the rock.

Fall onto or against objects

Injured bent down to pick up a saddle (belt structure). When injured stood up EE's shirt got caught causing EE to lose footing. EE fell and smashed EE's fingers between two saddles. This resulted in a fractured little finger with six sutures.

Caught in, under or between a moving and a stationary object

Injured was helping line the bearings up for the feeders gear box when the gear box slid off the forks and pinned EE's leg between the feeder and the gear box.

Struck by rolling or sliding object

Injured was loading bolts onto the roof bolter and the bundle of bolts slid and struck the ring finger on the right hand. This caused the end of the finger to deglove and fracture at the tip.

Over-exertion in pulling or pushing objects

Injured was trying to break a bolt loose on a scraper. When pushing on the wrench, the injured felt a pop and then pain in shoulder. An MRI on 4/29/2022 showed a shoulder supraspinatus partial thickness tear.

Struck by flying object

injured was putting 2 drill steels together. The steels were not all the way together when pressure was applied. This caused the steels to pop and strike injured in the hand causing injured to receive stitches.

Struck against stationary object

Injured was raising the bolter pod to insert an extension for the drill steel and popped hand resulting in a broken thumb.

Fall onto or against objects

Injured was setting the miner over. While walking backwards EE lost balance and fell. A rock struck EE's face and caused a laceration. Injured also strained ACL on right leg.

Caught in, under or between running or meshing objects

Injured was roof bolting on unit 1 in entry 6. The drill steel slipped from the left hand and landed between the top of the left foot and the drill boom that was being lowered. This put enough pressure on the foot to cause a fracture of the 2nd metatarsal.

While roof bolting in entry five, the injured connected the pusher and the starter steel together. The bolter chuck and the steels were misaligned when pressure was applied and the steels popped the hand. The injured fractured the proximal phalanx of the right index finger.

Injured was drilling the inside hole. The 2nd steel became clogged causing it to bend. Injured attempted to bring said steel back down, breaking the steel to steel insert and spinning the steel catching the injured across right forearm causing a laceration to the right arm.

Over-exertion in lifting objects

Injured was lifting the hood of a MT995 tractor to service the a/c when the shoulder felt like it tore. Injured went to the clinic on 8/8 and they treated and ordered a MRI. On 8/17 the MRI showed that there was a tear in the right rotator cuff. Injured is scheduled for surgery on 9/2.

Struck against stationary object

Injured was going down the slope when the Dapco started to slide. Injured started tapping on the brakes but could not get it under control. The Dapco came to a stop when it hit the barricades at the bottom of the slope. Injured received stitches above and below the right eye.

Struck against a moving object

Injured was a passenger in a 2 person mantrip. The driver ran over a timber in the road and caused the mantrip to run into the rib. The injured was brought back outside with right ankle and knee pain. MRI on 3/16 showed torn ligament on side of ankle and fracture in back of ankle.

Injured had just drilled a hole in the top. As EE twisted to grab the glue and roof bolt, the knee popped and dislocated.

Fall to the walkway or working surface

Injured walked out the door of the prep plant going to the foreman's pickup. Due to the weather conditions the whole area was a solid sheet of ice. The injured slipped and fell hurting the right shoulder as the injured tried to catch self. The injured was still in discomfort on 4/1/2022 and went to the doctor. On 4/6/2022 and MRI showed a tear in the rotator cuff.

Injured was helping mechanic work on the drill feed pot. As they were lining up the holes to install a bolt, the pot shifted and smashed the tip of finger. The injured received 3 sutures to the tip of the right hand middle finger.

Injured was joining the starter and the pusher drill steels together. When the pressure was applied the steels did not align correctly and the steels popped the injured on the hand. The injured received 4 sutures between the thumb and the finger.

Injured was helping build cribs in entry 2. A curtain board (2X4) was broken and hanging from the top in the way. Injured was prying on the board when it broke off and hit injured in the mouth knocking a tooth loose. A dentist ended up removing the tooth.

Injured was placing jacks in the beltline. When injured went to lift a jack from the pile it became unbalanced and fell. Injured grabbed for the falling jack and it smashed injured's fingers between the falling jack and the pile of jacks. Injured received sutures to the left index finger.

Injured was bending a stamped 8' roof bolt on the cab of the roof bolter. The bolt rolled and slipped off striking the injured in the lip. Injured received stitches to the top lip area.
