Accident type, without injuries

An unintentional roof fall has occurred on the M12 at the 32nd Xc of the number 4 entry. The area was in the process of being re-supported when the failure occurred the fall was approximately 35' x 40' x 12'. A site specific plan was submitted and the area is having additional support installed.

An unintentional roof fall occurred at the Xc-31 in the #10 Entry of the #2 unit. The fall was approximately 40' x 18' x 12'. The fall was barricaded off and a site specific plan to rehabilitate the immediate area was put in place.

A roof fall, in an examination line of travel, was found during a pre-shift examination in our 6W Mains. The fall was located at X-Cut #5 in Entry #8. The fall did not impede passage of miners, affect ventilation or cause personal injury to miners.

An unintentional roof fall has occurred on the P1-2 Belt line at the 26th Xc of the #5 Entry. The fall was approximately 25' long by 18' wide and 18' high.

An unintentional roof fall has occurred in the #9 Entry at the 6XC of the S1C. The fall was approximately 30 feet in length and 18 feet wide by 14 feet high.

The encoder shaft that has electronic transmitters to send back to the PLC to let the hoist no its location inside the shaft was damaged and would not allow the transmitters to relay information back to the PLC for the hoist to operate.

On the morning of 1/12/2018 the area had a winter storm with ice and snow and the facility power kept flickering back and forth causing the PLC on the hoist unit to scramble its brain so to speak. The utility company supplying the power checked their lines and they discovered a line that had been hitting together.

The elevator for the south portal was inoperable due to a piece of ice on a governor rope which caused the car brake to not release. The elevator was fixed after approximately 45 minutes.

Struck against a moving object

A mine mechanic was operating a diesel mantrip along the M5 Travelway when it appears the vehicle struck the coal rib and rolled on its side. The mechanic was partially ejected from the ride and the canopy fell on ee chest.

Caught in, under or between a moving and a stationary object

Injured was loading trash in a trailer. Foot was under the rub rail on the trailer. The trailer was pulled forward causing a small fracture to EE foot.

Struck by falling object

Injured was struck by a piece of draw rock approximately 3' x 4'x4"". Injured was taken to hospital for medical treatment and was discovered EE suffered fractures in EE's back and foot.

Fall to the walkway or working surface

Injured was turning to walk and twisted ankle and fell down while in the #8 entry of the #1 unit. Injured was taken to have ankle looked at and was originally told it was a sprain. On 6/15/18 x-rays were reexamined and it was discovered ee had a fracture.

Fall from machine

Injured was stepping out of EE's D10 Dozer at the end of EE's scheduled shift. As EE stepped out EE lost EE's balance, EE's foot slid between the track and dozer frame, EE fell backwards fracturing EE's left leg below the knee.

Over-exertion (Not Elsewhere Classified)

Injured was hand dusting the roof and ribs when they stepped on a rock and twisted an ankle. Injured continued to work and the following day was still experiencing discomfort in the ankle. Was sent for x-rays and was discovered they had an Fibula comminuted fracture, distal, non-displaced and tibia avulsion fracture non-displaced.

Struck by powered moving object

Injured was roof bolting. As ee swung the boom to install the rib pin, the boom hit leg and twisted ankle. A fracture was found in ankle upon review of the x-ray.

Caught in, under or between running or meshing objects

Injured was bolting in the #2 entry of the P1-F, while they were installing the middle bolt, injured dropped the drill pot on top of their right foot. Injured continued working and when arrived on surface the foot had began to swell and injured was taken to ER for evaluation. It was discovered with x-ray they had small fractures on top of the foot.

Struck by falling object

Injured was struck by a piece of draw rock on the left forearm while bolting on the #2 unit.

Caught in, under or between a moving and a stationary object

Injured was moving screen wire to place on the Roof bolter and their hand was smashed causing a fracture of a finger and thumb.

Struck against stationary object

Injured was a passenger in a man-trip which struck a roof jack which was installed in concrete along the travel-way. The corner of the bumper caught the jack causing the injured to strike EE's knee against the dash of the mantrip.

Over-exertion (Not Elsewhere Classified)

Injured was walking and twisted their knee after stepping awkwardly, continued to have pain. On 2/26/18 had an MRI and discovered has a medial meniscal radial tear in the left knee. Was placed on restrictions on this date.

Over-exertion in wielding or throwing objects

Injured was throwing cable over and felt a pop in their lower back. Injured continued to work but was experiencing discomfort in the lower back and leg area. Injured went for evaluation at Doctor on 12/6/18 and was placed on restrictions and given medication for the injury

Struck by powered moving object

Injured was connecting split steels and brought the drill pot up to reconnect and caught the drill steel and caused the steel to bend and strike the palm of the injured. Injured was taken the next day for evaluation and x-rays and it was discovered they had suffered an avulsion fracture at the base of the thumb.

Injured struck hand with the hammer while constructing a stopping on the S4G. Injured was taken for x-ray and revealed EE had fractured the thumb on left hand.

Over-exertion (Not Elsewhere Classified)

Injured reported on 6/11/18 that ee had sprained wrist while changing out an insert on 6/8/18. Injured was taken for medical treatment and was given prescription for the injury and placed on restricted duties.

injured was removing a miner bit from an insert and struck EE's thumb with the hammer. Injured continued to work and on 6/7/18 went to Doctor and had x-rays and discovered there was a fracture above the knuckle of the thumb.

Struck by falling object

On June 1, 2018 Injured was repairing a battery powered scoop. As EE was replacing a steel cover it slipped and hit EE's right pinky finger. EE went to the Doctor on June 28 and was placed on restricted duty due to a fracture found in that finger.

Fall onto or against objects

Injured fell when EE tripped over the power cable and fell on EE's back and landed on EE's SCSR. Injured was placed on restrictions for a lower lumbar strain.

Injured was cutting a flapper off of a piece of belt. The knife slipped and ee cut thumb. EE received two sutures to close the wound.

Injured hit self in the mouth with drill steel chipping front tooth out.

Struck by flying object

Injured was doing maintenance work on the outside of the preparation plant. As employee was filling up a tank, their right eye was hit by unknown material. The physician prescribed medicated eye drops for the damage to the eye.

Injured was roof bolting, as they installed the pusher steel into the starter steel it slapped back and hit their hand. Employee was taken to the hospital and received 4 sutures.

Injured was splicing a cable and lacerated left pointer finger with a utility knife. The injured received 3 stitches for the injury.

Struck by falling object

Injured had a man door which was on a pallet fall over strike right calf and caused an avulsion on calf. Injured was taken for medical treatment and received stitches for injury.

Caught in, under or between a moving and a stationary object

Injured was moving the fork of the telehandler and pinched EE's finger between the fork and the headache bar of the machine. Injured suffered a laceration as a result of this and received stitches for this injury.

Injured had a foreign body in eye which caused a scratch to eye. Injured was given a prescription for the injury.

Injured was making a splice on a cable, when the knife slipped and cut left hand. Injured was taken to hospital and received 6 stitches for injury.
