An unintentional roof fall occurred on the S1L at the 15xc in the #7 entry in the last open XC. The fall was discovered by the examiner during the pre-shift examination for the on-coming shift. The fall was approximately 20' x 20' x 10'
An unintentional Roof fall occurred on the S1J at the #2XC. The fall was approximately 30-foot long by 30-foot wide and over 10-foot high. The fall was discovered by the examiner.
At approximately 4pm the hoist man received a call from the work deck to bump up on #2 work deck winch. The hoist man reportedly heard a noise coming from #2 work deck winch, EE checked the winch and seen that the 1"" cable was unspooling from the drum and falling into the shaft. The cable completely unspooled to the work deck below due to mechanical failure of coupler.
The elevator would not run in inspection mode in the up direction. During the troubleshooting the elevator became inoperative at 11:40am. During the investigation it was found that the wires in the jacketed cable to the pendant controller were broken.
The emergency hoist was being inspected during the quarterly exam and when the examiner engaged the down function on the hoist the hoist malfunctioned and started unwinding while still sitting on the surface platform. During the investigation it was discovered that the cage rest sensor was bad.
Injured went to pick up a Roof jack off of the mine floor to install along the travel way of the M14 and when EE picked up the jack EE felt a pop in EE's forearm. Injured continued working and the following morning had a knot in the forearm of EE's arm. Injured was sent for medical attention and then sent for MRI. For possible bicep muscle tear.
Injured was cleaning walkways along the M8 belt line and bent over to pick up rail and strained lower back. Injured was taken for observation and placed on restrictions for a lower lumbar strain.
Injured was lifting a drill feed cylinder on 4/27/21 and claims they strained their back and then on 5/12/21 injured reported that they had hurt their back a few weeks ago and wanted to be seen by Doctor. Injured was seen and was put off work.
Injured said they hurt their back on 4/9/21 but did not report the accident until 4/19/21 and then, was sent for observation and placed on restrictions the same day for a back strain.
injured was installing bolts in two left blow through, when installing the outside bolt of the last row the bottoms which the foot jack was sitting broke apart causing the canopy to come down approximate 8-10 inches and striking injured on EE's head. Injured continued to work but neck become sore. Injured was taken for observation and placed on restrictions
Injured was checking fluids on EE's security truck. EE slipped/tripped on the ground and landed on EE's arm.
The injured was unbolting the shaft from the fan, when the injured twisted and felt a pop in the left knee causing a tear in the lateral meniscus. Injured was placed on restrictions on 1/5/2022
Injured was driving down travelway over corrected and front right bumper connected coal rib causing mantrip to come to stop, causing injured nose to come in contact with the steering wheel. This caused a laceration and fracture of the nose. Injured received stitches for the injury.
Injured was building cribs on unit 2 in entry 6 when Injured picked up crib block & strained Right Shoulder. Injured was sent for an MRI on the shoulder on 12/8/21, then on 12/15/21 MRI Results revealed a tear of Rotator cuff.
Injured was walking to hook up trailer on the pickup truck and strained knee. Continued to have pain and then on 11/24/21 MRI results revealed a torn meniscal in EE's right knee and was placed on a restricted duty.
Injured fell and hit elbow on the coal rib and the mine floor causing elbow to become dislocated. Injured was transported for medical attention and and elbow was put back in place.
Injured was swinging sledge hammer while installing bit inserts for the miner. Injured said the next morning EE had trouble lifting EE's arm. On 10/11/21 injured filled out accident report and was set up for appointment to be seen for shoulder and on 10/13/21 injured was placed on restrictions.
Injured was putting crib blocks under the miner because it was stuck in the intersection of the #10 entry. While putting the cribs under the machine EE smashed two fingers between the crib block and the ground. X-rays revealed EE had fractured both fingers.
Injured was bending over and putting waterline together and when EE stood up said EE felt a pop in EE's knee. Injured continued working and on 8/27/21 injured reported the incident and was set up for doctor appointment on 8/30/21. On this date injured was placed on restrictions and is awaiting an MRI.
Injured and another employee were putting covers back on the Roof bolter and when they went to pick up the third cover injured said EE felt something pull in EE's left bicep area. Injured was taken for medical attention and then placed on restrictions pending MRI results.
Injured was walking the M2 belt line and EE's knee locked up and EE fell to the ground and when EE fell EE's left knee hit a piece of C-Channel. The employees left knee was x-rayed and it was discovered EE had broken EE's knee cap.
Injured was installing roof bolts and while bring pot up to match with roof bolt bent the bolt and struck injured on the left wrist. Injured was taken and diagnosed with a left wrist strain and was placed on restricted duty.
Injured was scaling the roof in entry 5 and scaled a piece down then the piece in front of the piece that was being scaled fell also causing the first piece to bounce and strike the injured below EE's knee. This caused hematoma and leg contusion EE's right leg.
Injured was recovering belt structure and while carrying a belt saddle and slipped and fell into standing support along the belt line. This caused them to smash a finger between the standing support and the belt saddle which after being checked caused a small fracture at the tip of the index finger.
Injured was walking and stepped on uneven surface at a battery station twisting a knee. On 5/14/21 injured wanted to have knee looked at, so was set up to have MRI done on knee and was placed on restrictions on this date.
Injured was pulling on miner cable on the date listed above and continued to work and then on 5/18/21 injured had an MRI done and it was detected that the injured has sustained a small infraspinatus tear and was referred to an Orthopedic.
Injured pulled on piece of tie wire that was overhead and when doing so the tie wire did not give causing the tip of finger to break. Injured was diagnosed with a non-displaced spiral fracture and placed on restrictions.
Injured hit head on canopy of battery hauler after hitting an uneven surface while operating the hauler. Continued to work until 7/12/21 when injured was brought outside with neck pain. On 7/13/21 injured was placed on restrictions for the injury.
Injured was installing roof bolts and when EE pushed bolt into the roof EE hurt stomach area. On 4/29/21 injured had a CT scan and Hernia was discovered. On 4/30/21 injured was placed on Restrictions.
Injured was pulling into a crosscut on a battery transporter. EE was hit by a plastic pipe causing contusions to the leg.
Injured was lifting a timber and hurt EE's left shoulder. On 4/1/21 injured was seen and placed on restrictions and is awaiting an MRI.
Injured reported on 3/16/21 that EE had fallen and hurt shoulder but could not remember when it was when it had occurred exactly. Injured was sent to dr and on 3/24/21 was reported EE had a torn rotator cuff.
Injured was stepping over a concrete slab and stepped awkwardly coming down and strained left leg. On 12/9/21 injured was seen by doctor and placed on restrictions for knee strain and sent for MRI then on 12/14/21 MRI results revealed a tiny peripheral tear of the meniscus.
Injured reported on 12/2/21 that EE had strained EE's back on 11/16/21 while operating a battery scoop. Injured has made an appointment for 12/6/21 and on this date was placed on restricted duty.
Injured was lifting on a piece of belt and strained lower back. Injured was seen on 11/5/21 for the injury and placed on restrictions for this.
Injured claims hit head on the canopy of the mantrip as the ride was coming out of the mine. Injured reported the incident the following day. Injured was sent for observation and diagnosed with Myofascial strain and placed on restrictions.
Injured was roof bolting on the #1 Unit in the #9 Entry and while raising the boom to connect the pusher steel, EE hit their hand with the steel. Injured was seen the following day and diagnosed with a hand contusion and placed on restrictions.
Injured had just parked the bolter and was getting out to walk up to front controls and a piece of draw rock fell and struck in the Shoulder. Injured was placed on restrictions for a bruised shoulder.
Injured was installing 8' bolt and bent and smacked hand. This caused a sprain to EE's right hand. EE was placed on restrictions on 8/10/21.
Injured was taking side cover off of the miner and lost grip on the cover and smashed finger between the cover and the machine. This caused a non-displaced fracture on EE's finger.
Injured was driving in a battery lift pin on battery hauler and while driving it in EE felt a pop in EE's shoulder. Injured had an MRI and results revealed EE had torn articular tear.
Injured was picking up a bucket of miner bits off of the miner and said EE felt a pop in EE's back, and continued to work but started experiencing upper back pain and on 4-5-21 injured was placed on restrictions for a herniated disc.
Injured was using a pipe wrench to tighten a fitting and strained left shoulder, on 1/20/21 injured was placed on restrictions from doctor and reported the incident to the safety department
Injured was pulling a roof bolt from the working tray when they came in contact with the edge of a roof plate. This resulted in a laceration the the left forearm, which the injured received stitches for.
Injured was changing a final on a D7 dozer. As they assisted in removing the axle it fell on a finger. Employee received sutures for a small laceration.

Injured was doing light fabrication work under a work truck. While welding EE got irritation to EE's eyes. A prescription medication was used to flush EE's eyes.
