Erecting scaffold in cyclone when a clinker fell from an overhead section of the cyclone. The clinker landed on the employee's top right side of the hard hat. It also struck his right shoulder, right hip and left thumb causing a fractured shoulder and wrist.

Employee stated that he slipped and fell between the loader cab and catwalk while reaching for a water hose.
The elevator in the plant dropped momentarily when a piece of scaffold touched the elevator counterweight. The piece of scaffold bounced on the outside of the employee's foot just behind the steel toe which fractured his small left foot toe.
Employee stated that while he was climbing up on the water truck his right knee hit the bumper.
Employee stated that he was welding inside of the raw mill while standing and leaning on an aluminum ladder. The welding cable came into contact with the ladder and as a result, he sustained an electric shock.
Employee stated that he was traveling over a speed bump when the basket of the manlift he was using began bouncing causing him to fall on top of its bolt head.
Employee stated that he was cleaning a cooler when he backed up off the platform he was standing on and fell 2 feet to the floor landing on his right foot.
Employee stated that he was using a hammer and chisel to break open a chain when a piece of steel flew out and lodged in his leg.
Employee was welding and a piece of welding slag fell into his ear.
Employee lost his footing and fell from the same level, landing on some steel, resulting in a Contusion/Abrasion to the knee.
Employee stated that he was knocking down material buildup in the cooler with a high pressured water gun when vapor came out via the cleaning port and traveled thru his glove.
Employee stated that he was straddling a pipe to put fasteners in and as he leaned over to align it, the pipe sprung up and struck him in the face.
