A roof fall occured on the 2nd West Main, spad 0+80 in the supply road. The fall is 20'x20' height unknown.
A roof fall occured in the #4, 2nd East Panel. The fall is 180 feet long, 20 feet wide and 10-15 feet high.

While examining the maintenance shop for day shift, ee stepped off a bank which caused his knee to twist. A MRI was performed and found he has a torn meniscus.

Pressure washer had quit. Worker climbed into back of truck, fixed the problem by securing a wire, turned around & fell off the truck. Initially, this was not a reportable accident because worker did not seek med. treatment & no time was lost. Worker did not seek med attn until 9/14/10 & was placed on restricted duty. Effective 9/20/10, he was off work completely.
Was pulling cable off cars so they could be inspected by MSHA. He did not have pain until the next day,and filed a report on the 13th. He said he woke up with pain on his right side/hip/and leg.
While pinning the employee was struck on the bridge of the nose that caused a cut requiring stitches.
Employee was removing a leaf spring from a mantrip. After removing a bolt the spring dropped onto the employee's right pinky finger causing a laceration.
Employee was holding a piece of curtain for another employee to cut. The blade came through the curtian cutting the injured employee's finger.
EE was cutting a piece of bucker rubber for repair on the vibrator. While sitting down, he was holding the rubber in his left hand and in his right hand he was using a belt knife to cut the rubber with. After cutting through the hard surface, he entered the soft spot part with extra force, causing the knife to slip and cut his upper leg.
While pinning in #4 right a piece of coal struck the employee in the left rib and back area.
The employee was getting roof bolts off the pinner when the operator moved the pinner forward striking the injured employee's foot.
Employee was removing a door when the frame hit his third digit on his left hand.
While driving in an insert on the continous miner, the employee hit his left index finger with the hammer he was using causing a fracture.

Was breaking a water cuppling loose on a 6 inch water line when his hammer slipped striking him in the lower lip.
While starting his pin steel in # 10 entry, a rock fell out, cutting him above the knuckle on his right thumb.
While bolting in # 1 entry, on # 3 unit, EE was reaching for a pin plate when a small rock fell hitting the tip of his little finger on his left hand.
He was putting his pin steel in the chuck on the bolter, when it came loose and struck his thumb. This tore the skin between the right thumb and his index finger , which required stitches.
He had his hand on top of the boom and was trying to put his steels together and a small piece of rock fell and caught the end of his finger.
He bent over to pick up a cat head and felt a pain in his lower back and hips.
He was pulling a shipping band out from a bundle of pins. When it came out faster than he thought it caught his finger between a pin board and the roof.
