Struck by powered moving object

The victim was fatally injured when struck by a battery powered scoop which had trammed through a line curtain in the #6 Entry of the #5 Unit (MMU 009-0) outby the working place where the continuous mining machine was being operated.

Employee had a bottom drill steel hung in the chuck of the roof bolter. Employee was trying to pull the drill sreel out and went to drop boom and hit the rotations causing the drill steel to hit right thumb. Employee's thumb was fractured.

Struck against stationary object

Employee was walking around a shuttle car and hit EE's head on a pin board. Employee started miss work on 5/3/2021.

Fall to the walkway or working surface

Employee was walking outby in #4 entry and stepped in a hole in the mine floor and twisted EE's left knee.

Employee was pulling a metal band out from under timbers, when the band came loose it went under employee safety glass and struck employees right eye causing a laceration that took 12 stitches.

Fall from machine

Employee was standing on the slack pile of the scoop changer, when an employee pulled the cable tight causing EE to fall hitting EE's head and left shoulder. EE has a separation of AC joint and laceration to the scalp. EE is off work doing PT for the AC joint.

Caught in, under or between a moving and a stationary object

Employee was cleaning the backside of 1C belt. EE's foot was caught between the pump line and the mine water line causing EE's knee to twist. Employee has a torn meniscus.

Employee was removing a broke pinner steel that was stuck in the pinner chuck. The employee had a punch wedged between the flinger plate and the canopy post jack and hit the rotation lever, the steel came out of the chuck striking EE in the right jaw.

Over-exertion (Not Elsewhere Classified)

Employee was carry two buckets of miner bits and twisted right knee. Employee's Doctor has EE off until EE has an MRI on the right knee.

Employee stepped off a motor base at the 10-54 header and felt pain in back. Employee started missing work 5/18/20.

Struck against stationary object

Employee drilled a hole in the mine roof for a roof bolt. EE lowered the pinner steel and a rock was spinning on the steel. EE reached down to stop the rock from spinning and the rock cut EE's left middle finger. Resulting in three stitches.

Employee was hitting a bar with a sledge hammer trying to knock out a bushing, when they missed the bar and hit right index finger causing a laceration that required stitches.

Struck by flying object

Employee was working under the turn table of the clean coal stacker when the wind blew causing material to get into left eye. The material was embedded in the employee's left eye and was removed at the Dr's office.

Struck against stationary object

Employee was laying on the top belt in throat of tailpiece trying to back up on the belt. EE felt pain on EE's ankle and jerked cutting self through EE's leather glove on left index finger below second knuckle.

Over-exertion (Not Elsewhere Classified)

Due to repetitive motion the employee's shoulder developed tendinitis, bursitis and possible labral tear. Employee started missing work on 10/7/2020.
