Accident type, without injuries

During the examination of the Hanson hoist rope, there was damaged strains discovered that surpassed the allowable limit. The hoist was shut down and miners were withdrawn from the affected area. The Hanson hoist rope was replaced.

There was an unplanned roof fall in the #4 - 5th North spad 1+00 to 2+35.

Contractor was hauling soil from the back side of gob hill. As EE was returning for another load, EE heard a pop in the engine and started smelling and seeing smoke in the cab. As EE stopped to make a check, the truck was on fire. EE tried to put the fire out with a fire extinguisher EE had along with another one from another truck. Fire Dept was called. No injuries.

Struck by powered moving object

The victim was fatally injured when EE was struck by a battery powered scoop which had trammed through a translucent line curtain in the #3 Entry of the #6 Unit (MMU 005) just outby the working place where the continuous mining machine was being operated. Co-workers observed the victim in a sitting position stretching EE's legs prior to the accident.

Over-exertion (Not Elsewhere Classified)

Employee was hooking a steel rope up to pull belt off framing, when employee felt pain in lower right back. Employee started missing work on January 2, 2020.

Struck by falling object

Employee was pinning in # 9 left entry. When top coal fell out between the coal rib and rib pin striking employee on the right leg twisting right knee. The twisting of the leg tore EE's ACL in EE's knee.

Employee was kneeling down beside the miner operator, when a rock fell out between pins, hitting employee on the right knee, calf and ankle. Employee has compartment syndrome in calf.

Over-exertion in pulling or pushing objects

Employee was rehanging miner cable and pushed cable up with left hand, tied it with right hand, felt a pop in left shoulder. On 9/16/19 MRI revealed a full thickness tear to the rotator cuff. Employee started missing work on 9/16/19.

Fall to the walkway or working surface

Employee was taking bolts out of a cover, when the bolt came loose employee slipped and twisted falling backwards left foot did not move. Resulted in twisting the knee. MRI reveals Near complete to complete ACL tear.

Employee was walking and tripped over loose coal. EE's left leg gave way on the loose material, and employee fell. AN MRI was done and confirmed Left Quadriceps tendon rupture.

Absorption of radiations, caustics, toxic and noxious substances

Employee was plastering the top of brattice, when a piece of plaster fell into employee right eye.

Employee was driving down 5B road when a rock fell out between pins striking employee in the head.

Employee had drilled the first steel into the mine roof, when the employee let the roof bolter boom down, the boom then came into contact with the second drill steel, which was located in the employee's left hand, causing the steel to penetrate the injured employee's left foot.

Over-exertion in pulling or pushing objects

Employee was pulling down a clear curtain and felt something pull in EE's left upper arm. Employee started missing work on 7/29/2019.

Over-exertion in lifting objects

Employee picked up the miner cable and was moving it into the face of #3 entry and twisted upper body and felt right knee in a strain.

Over-exertion in wielding or throwing objects

Employee was using a sledge hammer to hit a coupling on a water line to break it apart. Employee felt pain in right upper arm.

Caught in, under or between a moving and a stationary object

Employee caught right index finger between the pinner chuck and the mine roof. This happened while employee was attempting to get a drill steel out of the mine roof. The right index finger was fractured.

Struck by falling object

Employee was capping off the side of a brattice, when piece of coal rib fell off and smashed EE's right thumb into the block. The abrasion took four sutures.

Employee was marking up #2 left entry, when wet coal fines that were stuck to the mine roof fell on EE's safety glasses & forehead. Employee took off the safety glasses to clean them, coal fines got into EE's right eye. Employee got a prescription for eye drops.

Employee was pinning and a rock fell out and hit left hand and fractured the tip of middle finger.

Employee was drilling up top steel, free hand was on the handle of the canopy, when a thin piece of rock came down the steel and slung off striking EE on the right arm above the elbow. The laceration required 4 stitches.

Rubbed or abraded

Employee had plaster and dust on their hand, went to wipe sweat from their eye area, and got plaster in the eye. Employee got prescription eye drops.

Struck against a moving object

Employee ran over loose rock with the stinger ride and the steering ball on the steering wheel hit employee on right little finger and torn ligaments. On 10/1/19 employee had surgery to put a pin in right little finger. Back to work, on 10/2/2019 no restrictions.

Struck by falling object

Employee was walking to side controls to drop the ATRS on the roof bolter, when a rock fell from between the outside pins striking and cutting EE's right arm. The laceration required stitches.

Struck by flying object

Employee was tightening straps on steel beams, and a piece of metal went between the safety glasses and got into EE's right eye.

Fall onto or against objects

Employee was pushing a rock off the continuous miner when the rock broke, causing the employee to fall unto the broken rock cutting EE's right forearm. The laceration required seven stitches.

Struck by falling object

Employee was installing a sun gear into the planetary axle on a continuous miner, when the employee's hand slipped off the sun gear it fell to the mine floor smashing EE's left middle finger. The finger was cut and required 5 stitches.

Struck by rolling or sliding object

Employee was moving a rock away from the coal rib, when the employee started moving the rock the rock slid down the coal rib smashing right middle finger. The finger was cut and required 5 stitches.

Caught in, under or between a moving and a stationary object

Employee was reclaiming belt framing and placing it on trailer, when ee got left middle finger in between two piece of belt framing cutting finger. The laceration required stitches.

Contact with heat

Employee was pinning and got over heated, EE had muscle cramps and nausea.

Employee was roof bolting and got dehydrated.

Employee was helping on the roof bolter and got dehydrated.
