ROOF FALL IN BELT ENTRY AT INTERSECTION, 40' LENGTH BY 30' WIDTH (INTO CROSSCUT) BY 4' HEIGHT. NO. 33 CROSSCUT ALONG NO. 1 BELT.
FALL OF ROOF MEASURING FORTY-FIVE FEET LONG, EIGHTEEN TO TWENTY FEET WIDE, AND ZERO TO SEVEN INCHES THICK. FALL DID NOT RESULT IN THE BLOCKING OF THE INTAKE.

ON DECEMBER 6, 2001 EE WAS PULLING THE CABLE FOR THE LEFT CONTINUOUS MINER WHEN HE TWISTED HIS LEFT KNEE. HE COMPLETED THE SHIFT AND WORKED SATURDAY, DECEMBER 8. ON SUNDAY EXAMINED BY A PHYSIC IAN.
EE HANDED THE SLEDGE HAMMER ACROSS THE BOOM OF THE CONTINUOUS MINER TO A CO WORKER. A PIECE OF MINE ROOF FELL MASHING HIS ARM AGAINST THE BOOM.
ON WEDNESDAY, AUG 8, 01, EE PERFORMED HIS NORMALDUTIES SUCH AS ASSISTING SET TIMBERS, OPERATE THE LOCOMOTIVE AND FORKLIFT. HE STATED HE DID NOT EXPERIENCE PAIN HELPING SET TIMBERS, CLIMBING IN AND OUT OF THE FORKLIFT AND LOCOMOTIVE DECKS OR DURING OTHER DUTIES. HE SAID HE FELT A TINGLING FEELING LATER DURING THE SHIFT AND WAS EXPERIENCING PAIN AT THE END OF THE SHIFT.
EE PLACED A CRIB BLOCK OVER TOP OF A BENT SHUTTLE CAR REAL COVER IN AN ATTEMPT TO STRAIGHTEN IT. HE THEN MOVED SIX TO EIGHT FEET AWAY, AT THIS TIME THE SHUTTLE CAR OPERATOR MOVED THE SHUTTLE C AR TO TRY AND STRAIGHTEN THE COVER. WITH THE MOVEMENT OF THE SHUTTLE CAR THE CRIB CAME LOOSE STRIKING EE ON THE THUMB OF HIS RIGHT HAND.
EE IS A ROOF BOLTER OPERATOR. ON MAY 10, 2001 HESTATED HIS BACK CONTINUED TO TIGHTEN UP AS HE WAS ROOF BOLTING & HE NEEDED TO LEAVE THE COAL MINE. HE STATED HIS BACK HAD BEEN BOTHERING HIM FOR TWO OR THREE DAYS PRIOR TO MAY 10. HE COULD NOT RECALL A SPECIFIC INCIDENT CAUSING HIS BACK TO HURT.
EMPLOYEE AND CO-WORKER WERE MOVING A WEAR STRIP FROM THE ROADWAY USING SLATE BARS. THE SLATE BAR SLIPPED ALLOWING THE WEAR STRIP TO FALL ON HIS RIGHT FOOT.
EE WAS MOVING DRILL BOOM INWARD. HE RUNS HIS FRONT HAND ONT HE CANOPY POST. AS THE DRILL BOOM SWUNG INWARD THE CANOPY HIT THE MINE ROOF AND HIS RIGHT MIDDLE AND RING FINGER WITH INDTY INT HE S T OR THE CAOPY POST. BEEN MISSING WORK 2-21-01
EE WAS LIFTING/SLIDDING AN EIGHT FOOT TIMBER OUT TO SIDE OF SCOOP BUCKET AND MASHED RIGHT MIDDLE FINGER BETWEEN TIMBER AND TOP OF SCOOP BUCKET.
MENBER WAS OPERATING CONTINUOUS MINER. HE HAD HIS HAND ON REMOTE BOX AND A PIECE OF MINE ROOF FELL STRIKING HIS HAND (RING FINGER)
EE WAS GETTING OUT OF MANTRIP & TOOK A CATCH IN HIS LOWER BACK. PER DOCTOR'S DIAGNOSIS, NO TRAUMA INVOLVED - OCCUPATIONAL DISEASE.
EE WAS LIFTING RAIL & STRAINED HIS ARM
