DISCOVERED UNPLANNED ROOF FALL IN OLD NO.1 ENTRY OFF #4 PANEL. ""OLD #3 BELT"", 1 CROSSCUT INBY SPAD 9189. AREA DOES NOT IMPEDE TRAVEL OR VENTILATION & HAS BEEN DANGERED OFF.

Accident type, without injuries

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED ON 10/05/05 IN THE 1ST S-E PANNEL AT SPAD 10180. THE FALL MEASURED 26'LONG X 20'WIDE X 6""-2'HIGH. THE AREA HAD BEEN BOLTED WITH 4 RESIN BOLTS.

Accident type, without injuries

AN INITENTIONAL ROOF FALL WAS DISCOVERED AT THE #3 ENTRY AT THE DRIFT MOUTH ON 1-5-05. THE FALL MEASURED 20' LONG X 20' WIDE X 3 TO 4' HIGH. THE AREA WAS SUPPORTED WITH 4' AND 6' FULLEY GROUTED ROOF BOLTS.

Injured was helping a co-worker remove a piece of stuck drill steel from the bolter drill pot when a piece of metal struck him in the eye.

On december 15,2005EE was splicing miner cable, while stipping miner cable. He cut his little finger on left hand.

Struck by falling object

Employee was installing a roof bolt when a piece of rock fell on the top of the canopy. The rock busted into several small pieces. A piece approx. 12"" x 12"" x 1/4"" thick landed on his arm causing a small laceration requiring 3 stitches.

Caught in, under or between a moving and a stationary object

He was installing tram motor on shuttle car. He had his hand on top of tram motor while trying to line up pin holes. Forklift was shut off when it drifted allowing motor to spring up against bottom of buggy smashing hand.

EE WAS DRILLING HOLE WITH A 54"" STARTER WHICH DUE TO HEIGHT CUT FOR FOR PONY DRIVE HE COULD NOT GET HIS HOLE DEEP ENOUGH FOR A 4"" BOLT. HE THEN TOOK STARTED OUT & PUT A FINISHER IN HOLE AND WAS TRYING TO PUT PUSHER & FINISHER TOGETHER WHEN HE BROKE FINISHER AT JOINT CAUSING DRIVER TO HIT HIM IN THE CHIN.

ON 10/17/05, EMPLOYEE WAS PUTTING BOLT IN TOP. HE WENT TO LINE THE BOLT HEAD UP WITH THE DRILL HEAD, BUMPED THE ROTATION & HIS GLOVE CAUGHT THE BOLT & TWISTED HIS HAND AROUND THE BOLT BEFORE HE COULD SHUT THE MACHINE OFF.

Struck against a moving object

Injured was riding in a vehicle leaving mine property when the vehicle he was in was struck head on by a second vehicle entering the property. This accident happened in a curve.

WHILE MOVING SHUTTLE CAR FROM #8 ENTRY TO FEEDER WHEN EE MADE TURN IN #8 ENTRY OUTBY WHEEL CLIMBED UP ON RIB. WHEN IT DROPPED OFF IT THREW EE AGAINST THE CANOPY

Struck against stationary object

Injured was tramming roof bolter into face no.2 entry when a section of roof measuring 25' x 19'x 26"" fell on the front of bolter forcing the injured head and shoulder to strike the canpoy in the operator compartment.

Absorption of radiations, caustics, toxic and noxious substances

INJURED WAS PLASTERING A STOPPING IN THE RIGHT RETURN WHEN THE PLASTER SPLASHED ONTO HIS FOREHEAD JUST OVER HIS RT. EYE, RUNNING DOWN BEHIND HIS SAFETY GLASSES INTO HIS EYE.
