EMPLOYEE WAS HOLDING THE HYDRAULIC DRILL WITH HIS LEFT HAND & REACHING DOWN TO THE MINE FLOOR TO PICK UP A WRENCH WITH HIS RIGHT HAND & FELT A CATCH IN HIS MID TO LOWER BACK AREA.

EMPLOYEE WAS OPERATING THE CONTINUOUS MINER WHEN ROCK FELL & STRUCK HIM RESULTING IN A FRACTURE TO THE RIGHT LEG.

EE WAS DRIVING SUPPLY MOTOR. HE WAS TRYING TO EMPTY THE CAR & COULDN'T GET THE HANDLE MOVED. HE STRAINED SOME MUSCLES & BROKE A RIB.

CREW WAS MAKING A BELT MOVE. AS BELT WAS BEING PULLED THE BELT HUNG IN THE FRAME WORK. THE WIRE ROPE BROKE & FLEW BACK STRIKING HIM ON BACK OF HIS LEGS.

Caught in, under or between a moving and a stationary object

HE WAS PUSHING SCOOP CHARGER OUT OF SCOOP BUCKET UNHOOKING CHAIN FROM RAM OF SCOOP. OPERATOR WAS INSTRUCTED TO PUSH RAM OUT. INSTEAD HE PULLED IT IN AND CAUGHT HIS HAND BETWEEN RAM AND BUCKET FRAME. CHAIN WAS HOOKED ON THE OPPOSITE SIDE OF SCOOP.

Struck against stationary object

EE WAS WALKING IN THE BELT ENTRY HELPING MOVE CABLES. HE RAN INTO A BELT HANGER AND RECEIVED A CUT ABOVE THE EYE BROW.

THE MINER CABLE AND WATERLINE WERE LOOPED ON RT.SIDE OF NO.3 ENTRY. EE SAW THE BOLTER WAS ABOUT TO GET ON THE CABLE. HE STEPPED UP TO MOVE THE CABLE AND HIS LIGHT CORD CAUGHT ON THE SIDE OF THE BOLTER. THIS TWISTED HIS BODY AND CAUSED HISLT. FOOT TO STEP IN FRONT OF THE TIRE. THE TIRE RAN OVER THE EDGE OF HIS LT. FOOT CAUSING A CHIPPED BONE AT END OF HIS LITTLE TOE.
