Roof fall found during air way examination. Fell above anchorage.

Top fell during second mining pulling roof bolts out by Mobil roof supports.

Over-exertion in lifting objects

EE complained of pain in low back after uncoupling and handling belt structure. The accident occurred in the belt entry, there was no mining, and mining equipment has been removed due to the mine being permanently closed. A report was made online on 9/24/14 (attached). Upon reviewing 3rd quarter results it was noted that the report failed transmission due to an error of the writer.

Fall to the walkway or working surface

Operator was informed that medical treatment was provided on 6/6/2014 for injuries sustained to the shoulder when employee slipped on ice. The employee no longer works at this mine.
