Accident type, without injuries

Unintentional roof fall located on the Northwest Mains #2 Section, #6 entry, Station 5211. Fall measured approximately 18'L X 18'W X undetermined height. No injuries or equipment involved. Area previously supported with 6' fully grouted resin bolts and cable bolts. Overburden approximately 290'.

Accident type, without injuries

Unintentional roof fall discovered while checking dewatering pump. Fall measured 18'L X 18'W X 5'H. Area previously supported with 5' torque tension roof bolts. Area was mined 12/7/04. Fall was in 7 hdg, 3 belt, station 3479. Fall will be cleaned and supported.

Accident type, without injuries

Unintentional roof fall occurred on the North Mains Section, MMU 001-0, in Xct to the right of Station 731, #5 Entry, 86 Block. Fall measured approx. 8'H X 30'L X 18' W. Area mined on 12/12/06. Overburden approx. 220'. Area previously supported with 5' fully grouted resin bolts. No injuries or property damage. Called fall in at 10:30 AM to MSHA and WVMHS&T

Unintentional roof fall. Area was heard to be working prior to falling. Fall measured 40'L X 18'W X 10'H. Area was mined on11/14/06. Overburden is 360' in this area. Previously supported with 6' resin grouted roof bolts, supplemented with additional 6' bolts and jacks. Fall is located on #2 Section , 3 entry,Spad 5181. Reported to MSHA at 3:40A (G. Fetty)

Unintentional roof fall discovered at 5:50 AM. Fall measured 6' to 10'H X 19'W X 65'L. Originally supported with 6' full grout resin bolts in entry and 6' full grout and 10' cable bolts in intersection. #5 track between 36 & 37 block, Main Hdgs, to right of stations 4441 - 4448. Reported to C. Mosley at 5:55AM 11/13/06.

Unintentional roof fall on abandoned #1 Section. Fall measured 75'L X 18'W X 10H. Area supported with 6' resin grouted roof bolts and 10' cable bolts. Fall location: Sta. 4985 to sta. 5000. Fall was discovered while examining intake and return airways.

Unintentional roof fall discoverd on abandoned #1 Section. Fall measured approximately 70'L X 18'W X 10'H. Area previously supported with 6' resin grouted bolts and 10' cable bolts. Fall was from #1 entry to #2 entry just left of station 4981. Fall was discovered during examination of intake and return airways.

An unintentional roof fall was discovered on the old #1 section, #10 track, 18 break, 1 break inby spad station #4914. This area is non-producing and equipment has been moved from the area since Sept. 2006. District office was contacted 3:05 PM on 10-17-06.

An unintentional roof fall was discovered on the #1 section, 1 break inby spad station # 5029, from #7 entry thru #9 entry in the last open X-cut. The fall measured 90' long x 17' wide x 11' high. This miner section has been withdrawn and ceased mining.

An unintentional roof fall was discovered outby the #1 section between entries #10 and #11, across from spad station #4982. The fall measured 45' long x 18' wide x 10' high. This fall was outby the working section.

While operating a torch cutting metal on the 4th level, the workmen ignited a small methane bleeder in the concrete wall 7 feet above the floor level. The flame was approx. 2 inches in length, burned 5 to 7 seconds, and was extinguished by using a gloved finger to smother out the flame.

An unintentional roof fall was discovered on the #1 section, #6 right entry to #7 entry (face) 1 block inby #4936 in #6 entry. The fall measured 40 ft L x 18' wide x 6 1/2 ft high.

An ignition of methane occurred in the underground bin area during the demolition of the facility. Methane which was liberating through the joint in the wall and floor ignited when cutting torch blew sparks and slag against the location. Flame duration was 5 to 7 seconds and was extinguished by smothering it out. No injuries occurred.

At approx. 1:20 pm on the miner unit 003-0 the continuous miner cut into an unchartered gas well there was an inundation of 7% CH4, 17.3 02 and 105 PPM Co.

The mine elevator went down due to the system not reading one of the safety switches on the cage rails in the shaft. Not in operation until 11:00 AM. Repaired by Industrial Elevator.

Sycamore Mine #2 had a roof fall on the mains section at cross-cut 4 to 5 in the last break. The fall measured 8' high x 15' long x 16' wide.

(2ND OF 2 ROOF FALLS THIS DATE.) An unintentional roof fall has occured in B1A panel in #4 entry at spad 13175. It is in old works and does not affect passage or escapeways. Height - 7'; Width - 20'; Length - 40'. Support - 5' fully grouted resin bolts. Developed 12/3/04. No injuries or equipment damage.

UNINTENTIONAL ROOF FALL. FALL MEASURED 20' LONG X 18' WIDE X 40' HIGH. AREA MINED 2/22/06. NO INJURIES OR PROPERTY DAMAGE. ROOF PREVIOUSLY SUPPORTED WITH 6' TORQUE TENSION AND 6' DOUBLE LOCK BOLTS. ONLY ENTRANCE WAS DANGERED OFF AND BROW SUPPORTED WITH 3 CRIBS. FALL MOST LIKELY WILL NOT BE SUPPORTED OR CLEANED.

Elevator went down due to movement of rails caused by temp. change. Sensors were adjusted - running @ 9:40 am. This is a new system and is still having a few ""bugs"" worked out.

This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been investigated. The dimensions are 18'W x 75'L x 8'H.

This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been fully investigated. The dimensions are 19'W x 70'L x 8'H.

This fall was discovered & reported during the recovery effort by Mine Rescue Teams to the surface command center. It is unknown when the fall occurred but it has now been fully investigated. The dimensions are 40'L x 19.5W x 7.5'H.

Employee was in a standing position operating a Joy 14-15 radio remote controlled continuous miner. The immediate roof began to fall between the roof bolts and the employee turned to move from the rock when his feet became lodged in the muddy bottom causing a strain to his lower back. Did not miss work until 11/9/06.

Struck by falling object

The injured employee was drilling a hole in the mine roof when a small/med piece of rock fell next to the rib, striking the injured on top of right shoulder & back of right leg.

Over-exertion in lifting objects

The injured was assisting hanging a miner cable in the #4 entry on the #1 section when he felt a ""pop"" & pain in his right shoulder.

While pulling charger cable, injured caught foot in another cable and fell, pulling groin muscle.

Struck against a moving object

While tramming shuttlecar, injured hit a hole in the bottom causing him to bounce out of the seat and strike his head on bottom of canopy, hurting neck.

Injured was working at the #6 belt drive when he slipped in a thin layer of mud twisting his right knee. Did not become reportable until 6/11/06.

Bending roof bolt while installing into top, injured felt pain in lower back. This became a lost time accident 5-23-06.

Fall to the walkway or working surface

Injured was walking along #2 main belt conveyor when he stepped on uneven mine floor causing him to fall on left knee. This did not become LTA until 6-1-06.

Injured was using a torch overhead on the belt line to remove bent bolts. The mine floor was wet with some water in the area he was standing. As he back away from his work,still looking toward it, he tripped backwards over the 6"" water line causing him to strain his rt. knee. The injury became reportable on 4/8/06 when he had surgery on the knee.

INJURED WAS DRAGGING 2 BOXES OF RESIN UP #6 ENTRY ONE BOX CAUGHT IN THE BOTTOM AND CAUSED A STRAIN TO LOWER BACK.

Walking in #7 entry towards power center, walked into wire screen, cut left ear and left side of neck. He has ringing in ear and stiff neck.

While tramming miner into the #3 entry, injured stepped into a hole, twisting his right knee.

AT 6:31 AM AN EXPLOSION OCCURED BEHIND THE OLD 2 LEFT MAINS SEALS, RESULTING IN CARBON MONOXIDE POISONING TO THE INJURED EMPLOYEE WHICH REQUIRED HOSPITALIZATION. MSHA, STATE AND COMPANY HAVE INITIATED INVESTIGATION BUT RESULTS NOT YET AVAILABLE.

Struck by falling object

Employee was in an upright position roof bolting. He had completed drilling the rib side hole in the roof and was reaching back to get his roof bolt when a piece of rock measuring 25""LX18'WX2""T fell from the roof and struck his right forearm. X rays negative. Employee later fainted making it an emergency.

Fall to the walkway or working surface

While installing roof bolt, loose top started falling, injured started to move out of the way and tripped on a rock striking his elbow on the bottom.

Injured was operating the helpers side of the roof bolter in the # 6 entry, he pinched the palm of his right hand between the pusher and the finisher steels, causing a laceration. Injured received 3 stitches.

Caught in, under or between running or meshing objects

Injured was bolting wire screen to mine roof. He placed a bent drill steel against the top while his hand was on the drill steel. He started the drill rotation and the steel bent and struck him on the hand and finger.

Struck by falling object

Injured was installing 5'W X 15'L with 4"" opening wire screen against the mine roof, with a roof bolter, when a small piece of shale fell from the roof and struck him above the right eye causing a laceration requiring 9 sutures to close. Injured was released to normal duties beginning 11/14/2006.

Injured was installing roof support over the slope belt when a rock, 8"" by 10"" by 1"" thick fell striking him on the right ear causing a laceration which required 3 sutures to close.

Injured was cutting a 2"" plastic water line while it was laying across his leg. The utility knife slipped out of the pipe and cut him causing a laceration which required 4 sutures to close.

Injured was building a Kennedy stopping, as he lifted a metal stopping panel it slipped through his hand, the edge of panel cut through his glove and lacerated his right thumb. He received 6 stitches.

Struck by falling object

The injured was installing a roof bolt into a drilled hole when bolt being installed slid against edge of hole causing small piece of slate to break away from mine roof striking injured on upper right lip. Injured returned to work same shift.

Struck against stationary object

Pulling roof bolt off of roof bolter, bolt suddenly came loose causing injured to strike bolt and hand against rib. Injured received 3 stitches in left little finger.

Struck by flying object

Was removing a bearing from a shaft by using a chisel and hammer. As he struck the hammer, a small piece of metal came from the sharp end of the chisel and lodged in his chin. It was surgically removed on 2/8/2006.

Injured and co-worker were taking slack out of a continuous miner conveyor. The injured was securing the chain with a sledge hammer. Co-worker was striking the chain with a sledge hammer. Co-worker swung at the chain but struck the injured in the right hand. Fracture was not identified until 1/27/2006
