An unintentional roof fall was discovered during the weekly airway exam. The fall was located in Old 4 zone at 49 block in the No. 8 entry near spad 1539. The fall measured 16 feet wide, 25 feet long by 5 feet high.

All incidents in 2012
On 10-14-12 at 6:28 a.m. the Elevator went down. There was no one trapped. The Bodie Brake malfunctioned and would not release. Shindler Elevator Service was called to the mine and replaced the part. The Elevator was back in service at 6:15 p.m. 10-14-12.
On 10-14-12 at 9:23 p.m. the Elevator went down. There was no one trapped. There was an electrical connection causing the Bodie Brake to read bad. Shindler Elevator Service was called to the mine and repaired the connection. The Elevator was back in service at 12:50 a.m.
After taking the first lift to the left in the No. 6 entry, the continuous miner was backed out of the lift the bolted roof in the entry fell approximately 7 to 8 feet high, entrapping the continuous miner and 2 MRS's. The unbolted roof in the lift was not immediately affected.
Outby foreman went to build a stopping in this area and located the fall
An unplanned roof fall occurred outby the idle #2 section on 7/16/2012. There were no injuries or equipment damage due to the fall and there were no escapeways blocked. The fall was in the return and an alternate airway has been established. This is an idle section and the equipment has been moved outby to the #16 tailpiece.

An unplanned roof fall occurred at the Imperial Mine on 4/26/12 on the #2 section in the face of #5 entry intersection and through the crosscut 5-6 at spad #8803. There were no injuries or equipment damage due to the fall. The fall will be cleaned up according to the company fall clean up and securing plan. Imperial will also follow guidelines put in place by MSHA & the WVOMHS&T.
The date and time of fall is unknown it was located by the weekley examiner during his travel of that air way. It will not be cleaned up and does not effect Air Flow.

During the weekly air way exam the examiner encountered this roof fall in the number 7 entry South West Mains 2. This fall is located in a worked out area and did not effect the air way it will not be cleaned up.

An unplanned roof fall occurred 3/28/12. There were no injuries, blocked escape ways or eq. damage due to fall. The fall dimensions are 85' L. X 7' H. X 18' W. The fall area has been dangered off and all approaches to the fall have been cribbed and timbered. Loc. of fall Old #5 zone, 2blks inby spad #8097 # 8 entry 41-42 blk.
While mining in the number 3 Entry the Face ignited and went out it did not travel outby
An unplanned roof fall was discovered 2/20/12.The loc. of the fall is #5 zone #36 blk in #3 entry in the intersct. There were no injuries, Eq. damage or escape ways blocked. The area around the fall has been supported by cribs & timbers and all approaches have been dangered off. The fall dimen. are 27' L. X 17' W. X 8' H. This area was dev. on 2/27/11 with approx. 180' of cover.
An unplanned roof fall occurred at Imperial on 2/13/12. Location of the fall is #5 zone #5 belt #37 blk to #38 blk. There were no injuries, equipment damage or escapeways blocked due to the fall. All approaches to the fall have been dangered, cribbed and timbered off. 15' outby spad #8069. Fall dimensions: 50'L X 18'W X 6 1/2'H.
During the production shift The intersection on the Number 1 Section Number 8 entry fell in the last open break.
Weekly examiner located the fall during his exam and reported to management when he reached the surface.
Employee was cleaning front windshield on the loader when he went to take a step and foot slipped and fell. Foot slippped off 90 degree fitting on hoses in center of loader. His foot slipped down about 1' and badly twisted right knee.
When removing the drill steel from the drill chuck. The steel became fouled, extra force was used to wiggle and jerk upward and pain was felt in Right Shoulder.
Employee stepped off the back of the roof bolter into a water hole and twisted his right knee and heard something pop. Incident occurred on 11/1/2012, became restricted duty on 2/7/2013 when employee was placed on restricted duty.
Principal placed glue and top end of roof bolt into the drilled bolt hole. As he was placing the head (or bottom) end of the bolt into the drill chuck a piece of rock fell striking his left hand and catching the thumb oh his hand between the rock and the bolt plate resulting in a laceration to his thumb.
Individual was letting down cable. As he released the rope cable hanger, it caught on the cable and swung upward. The hook on the cable hanger caught his nose and caused a laceration. Individual was treated by the Emergency Room Doctor and received four ""Sutures"". Individual was released to work with no restrictions.
Individual was removing the line curtain from the face area of the number 11 entry. When he stepped backwards with curtain, his foot landed on a rock that was covered by water. This caused him to fall backwards injuring his left foot. X-rays revealed a fracture to the small bone in his foot.
Individual was struck by a piece of rock that slid off the loaded shuttle car and into the canopy area striking him on the right hand and causing a laceration the required 6 sutures.
Rock Truck bed safety latch came back and hit EE in the mouth.
Employee was hosing the floor in the breaker building and as he started out the back side doorway that had a piece of curtain hung for a door he caught his left forearm on a jagged piece of metal hidden by the curtain causing a laceration which required 8 stitches to close.
The injured employee was cleaning off the miner when a rock fell from the top striking him in the head above the right ear causing a laceration that took 5 staples to close. The employee also struck his right elbow on the machine and x-rays were taken and results were negative.
Individual received laceration to his left hand, while making a splice to the shuttle car cable. His hand slipped resulting in a laceration to his middle and ring finger from a utility knife.
While operating the continuous miner in the #3 entry on the #3 section; employee, was struck in the left side of the neck by a piece of metal that appears to have come from a bit lug or a miner bit.
