Unintentioinal roof fall discovered this time and date. Fall located in 22 Zone, 13 to 14 block, no. 9 hdg. Fall measured approximately 70'L X 18'W and undetermined height. Area mined on or about 1/17/2007. Roof was previously supported with 6' full grout resin bolts. Overburden between 160' and 180'.

Unintentional roof fall discovered this time and date. Fall located in 22 zone, 11 block, No. 9 entry. Fall measured approximately 6'H X 18'W X 60'L. Area mined on or about 1/15/07. Roof previously supported with 6' full grouted resin bolts. Overburden between 160' and 180'.

On 8-31-09 the mine examiner found a roof fall in the return on the 2 B panel at 17 blk in the old belt heading.

Unintentiional roof fall discovered on the above date and time. Fall measured approximately 20'LX10'HX20'W. Location of the fall is No. 11 Zone, 18 block, 9 entry, 1 block outby station 837. Fall location previously supported with 6' full grout resin and 10' cable bolts. Overburden approximately 180'-200'. Area mined on or about 1/11/07.

At approximately 6:35 am on 8-4-09 the mine examiner found a roof fall on the # 2 section return J panel.

On 8-3-09 the shift foreman discovered a roof fall on the old # 1 section. This section of the mine has been abandoned.

On 7-22-09 at 8:35 pm a roof fall occured on the # 2 section in the # 7 entry.

An unintentional roof fall occurred at the above time and date. Fall located on 11 belt, 19 block, 5 hdg, station 814. Fall measured approximately 18'W X 50'L X 10'/15'H. Continuation of fall to the the left of current fall. Previously supported with 6' full grout resin bolts and cribbing. Area mined on or about January 2007. Overburden at or about 180'.

Unintentional roof fall discovered the above date and time. Fall location is: Old 16 zone, No. 5 block, No. 5 entry, 1.5 bloccks inby station 3554. Fell in entry between intersections. Location mined on or about 10/30/08. Previously supported with 6. full grout resin bolts. Fall measured approx. 16'L X 17'W X 7'H. Approx. 240' overburden. Abandoned area.

At approximently 12:35 pm on 3-26-09 a roof fall occurred on the #1 working section in the last open crosscut.

At approximately 5:30 am on March 22nd 2009 a roof fall occurred on the # 1 section in the # 9 entry in the last open crosscut.

Unintentional roof fall discovered this date. Area previously mined on or about 10/18/06 and was supported with 6' full grout resin bolts. Fall measured approximately 18'W X 75'L X 6'H. Fall location is 15 Zone, 11 block, Xct 8-9 & 9 Intersection 2 blocks inby station 2490. Overburden is at or about 280'.

Unintentional roof fall discovered on this date by company and MSHA officials. Fall was located at North West Mains, #2 hdg, 1 block outby spad 4471, block 8-9. Fall measured 20'L X 7'H 18'W. Area previously supported with 6' full grout resin bolts. Area was mined on or about 01/20/2006. Overburden about 260'.

Over-exertion in lifting objects

The employee was building a ventilation stopping when he bent down to pick up a block & he strained his back.

The injured was putting on his boot in the bathhouse when he felt pain in his right side. The cause was later determined to be genetic weak abdominal tissue which allowed this condition. This is being classified as an occupational illness and not an injury.

Fall to the walkway or working surface

End loader operator had got off the loader to move the coal stacker. When he was walking back to the loader he fell on the frozen, slick ground. When he was uprighting himself, he struck his head on a metal storage rack causing a strain to his neck.

Struck by falling object

The injured was operating a radio remote controlled continuous miner when a rock fell inby the last row of roof bolts, it rode through the last row in a manner that it struck the injured as he truned to get back, resulting in an injury to his neck and a fracture to his left ankle.

Over-exertion in lifting objects

The employee was taking off a rope of miner cable, when he felt a pop in his shoulder.

Fall onto or against objects

Injured was standing in an upright position and was reaching above his head to hang a ventilation line curtain, when he fell back against the continuous miner which caused a pain in his back. Injured continued to perform his normal duties, without restrictions, and began to perform restricted work on 12/28/2009 making it reportable on that date.

Fall to the walkway or working surface

Employee was in an upright position operating a Fletcher DDO-15 Roof Ranger Roof Bolter, when he slipped on a piece of rock on the mine floor. In an attempt to break his fall, he extended his right arm out and when he landed, he received a strain to the right wrist.

Struck against stationary object

Employee was extending the ventilating curtain in the working face. He was reaching upward to attach the line curtain to a curtain hanger that was attached to a roof bolt that was through a pizza pan. As he hung the curtain on the hanger, the back side of his left ring finger came in contact with the hanger causing a laceration to the finger requiring 5 sutures to close.

Employee stepped from a rubber tired mantrip onto a belt idler roller. The roller turned, causing him to sprain his right ankle.

The employee was moving power, when he bent down to move the miner cable he experienced pain in lower back.

Struck by falling object

The employee was roof bolting when a piece of roof rock fell, striking him on the left hand, fracturing the 3rd metacarpil bone and lacerating the index finger.

The employee was opening a drill steel bit bucket with a utility knife, when the knife slipped, lacerating his left hand.

Struck by falling object

Employee was operating a roof bolter on the right side. He had completed drilling a hole and was bringing the drill steel out of the hole when a small piece of roof rock fell and struck him on the right side of the upper lip. The wound required 3 sutures to close. He returned to work on his next scheduled shift without restrictions.

Struck by falling object

The employee was moving a piece of line curtain away from the rib, when a small piece of roof rock that was laying on top of the curtain, fell lacerating him on the right hand. The wound took 7 sutures to close.

Caught in, under or between a moving and a stationary object

The employee was roof bolting and while installing the bolt into the drill head he inadvertently hit the steel isolater lever squeezing his left hand resulting in 14 sutures.

Struck by flying object

The employee was operating a dual boom Fletcher roof bolter when a piece of roof rock fell, striking the drill pot causing it to fly up, striking the indivual in the mouth. The laceration required 6 sutures to close the wound.

The employee was operating a fletcher roof bolter when he inadvertingly hit the steel isolator control closing it on his right hand causing a fracture.

The employee was operating a battery mantrip when he leaned up hitting his head on an overcast.

Injured was cutting conveyor belt with a utility knife when he cut himself on the right thigh resulting in a laceration which required staples to close. Injury was to a contract laborer, operator was made aware that staples were administered on 4-2-09

The employee was cutting tape off a set of drill steels, when the utility knife slipped, striking him in the left hand requiring 3 sutures.

Struck by falling object

The employee was building a stopping when the block fell striking his left pinky finger requiring 12 sutures.

Fall onto or against objects

The employee was in an upright position walking down the mine pit access road, when he stepped on a piece of ice, causing him to slip. He was carrying a plastic dinner bucket and when he fell, his left chest area fell upon the dinner bucket, causing him to fracture the 5th and 6th ribs on the left side of his chest.

Employee and co-worker were removing broken pins from feeder/crusher shear pin plate with punch and hammer. When injured attempted to strike punch, being held by co-worker, he missed the punch with the hammer and struck the punch with the index finger of his right hand, causing a laceration that required 9 sutures to close. He returned to work on his next scheduled shift.

Rubbed or abraded

From working on his knees during the course of his employment he developed Cellutisis as diagnosed by his physician.
