Accident type, without injuries

Unintentional roof fall discovered. Fall located on No. 4 track, No. 12 block, 1 block left of station 350. Previously supported with 6' torque tension bolts and screen. Fall measured approximately 2'L X 18'W X 2'H. Area developed on or about 01/31/2005. Overburden about 260'.

Accident type, without injuries

Unintentional roof fall discovered. Fall is located at 11 Zone, 21 Block, No. 2 entry, 1 block inby station 829. This area was mined on or about 10/18/2006 and was supported with 6 foot resin grouted roof bolts. Overburden is about 180 feet.

Unintentional roof fall discovered during inspection. Fall location is #2 Belt, #6 hdg, 11 block, inby station 5102. Fall measured approximately 20'L X 18'W X 7'H. Previously supported with 6' torque tension bolts and was developed on or about 04-10-04. Overburden approximately 300'.

Unintentional roof fall discovered this date while examining escapeway. Fall measured approx. 50'L X 17.5'W X 6'H. Previously supported with 6' full grout resin bolt. Area developed on or about 6/15/08. No injuries. No property damage. Escapeway not blocked. Approximately 290' overburden.

Unintentional roof fall discovered at 11 Zone, 29 block, No. 1 entry, 1 block to left of station 919. Fall measured approximately 40'L X 6'H X 18'W. Area was previously supported with 6' full grout resin roof bolts. Area was mined on or about 02-02-07.

Unintentional roof fall discovered this date. Fall measured approximately 18'W X 60'L X 10'H. Area previously supported with torque tension roof bolts and cable bolts. Area was developed on or about 4-08-05.

Continuation of roof fall reported 5/12/08. Fall location: 2 Lt section, 27 break, 8 entry Xct rt, inby stations 4315 and 4342. Area mined on or about 3-21-06. Fall measured approx. 30'LX18'WX8'H. Previously supported with 6' resin grout roof bolts and 10' cable bolts. Overburden approximately 260'.

A previous fall had been supported with a tunnel liner. More rock fell from the previous fall cavity and caused the existing liners to bend, blocking the secondary escapeway. Area was last traveled at or about 5:00 AM.

Unintentional roof fall discovered on above date. Location of fall is No. 2 section, 6 heading, 46 to 47 block, station 5260, Did not block escapeway, no injuries, or property damage. Fall measured 18'W X 50'LX approx. 10'H. Previously supported with 6' full grout resin bolts. Cover approximately 225'. Mined on or about 1/27/07.

A face ignition occurred on the 003 mmu at approx 12:10 am. No heat of concussion was felt by witnesses. It self extinguished in seconds. No injuries to persons or property. Sandstone was present.

Unintentional roof fall discovered this date. Fall location: 2 Lt section, 27 break, 7 entry, inby stations 4315 and 4342. Area mined on or about 3-21-06. Fall measured approx. 36'L X 18'W X 8'H. Previously supported with 6' resin grout roof bolts and 10' cable bolts. Overburden approximately 260'.

Unintentional roof fall discovered this date. Location of fall is North West Mains, left of station 4667 in crosscut between 1 & 2 entries. Fall measured approximately 18' wide and 40' Long. Height is undetermined. Overburden in this area is approximately 320'. Previously supported with 6' full grout resin bolts. Area mined on or about 6/11/06.

Unintentional roof fall discovered this time and date. Location of fall: No. 2 Section (MMU 002-0), No. 1 entry, station 2618. Fall measured approximately 110'LX18'WX6'-8'H. Area previously supported with 6' full grout resin bolts. Overburden is near 160'. Area mined on or about April 10, 2008.

Unintentional roof fall discovered 1/9/08 during weekly inspection. Fall located in No. 11 Zone, 18 Block, Xct 8-9 one block inby station 680, No. 9 entry. Fall previously supported with 6' full grout resin bolts. Area mined on or about 10/21/06. Fall measured approximately 17'W X 6.5'H X 15'L.

Two employees were entrapped on the elevator when it stopped in the shaft due to the cold conditions not allowing magnetic switches to opperate properly. When these switches do not ""make up"" the safety features of the elevator shut it down. the men were removed from the elevator without harm at 10:09 AM

Struck by powered moving object

The victim was walking in an outby direction on the outby side of the scoop, which was being trammed over the victim resulting in fatal crushing injuries.

Struck by flying object

The injured was making a belt splice and was not wearing his safety glasses when a piece of a metal hammer broke off and struck him in the left eye.

The injured, along with 2 others, were installing a mechanical splice in a piece of mining belt when a scoop struck them.

Over-exertion (Not Elsewhere Classified)

The injured was moving the continuous miner cable out of the way when he felt a stinging sensation in his groin; over several days it got progressively worse. He was diagnosed with a genetic weak abdominal tissue which allowed this condition. This is being classified as an occupational illness and not an injury.

Caught in, under or between a moving and a stationary object

The injured was operating the radio remote continuous miner when he was caught between the cable handler and the coal rib resulting in crushing injuries to his left leg and thigh.

Employee was lowering a 14"" diameter pipe into place when it became caught. He kicked it with his right foot to free it. The pipe broke free catching the side of his foot in a pinch point causing a laceration. The employee was wearing metatarsals, but it caught just behind steel toe and below metatarsal.

EE was installing a 5 foot resin roof bolt with 8""x8"" plate & a metal curtain hanger. In an effort to align the hanger he attached a nylon rope to it, when he applied rotation to the bolt his hand & arm became caught and twisted by the rotating roof bolt resulting in fractures to his left arm, wrist, ring and middle fingers as well as a laceration to the palm of his left hand

Struck by falling object

EE finished installing new cable and danger signs on the legs under the emergency refuse bin. He was getting into the passage the pickup truck when he was struck on the top of his hard hat by a falling rock (2""x 8""x 10""l) causing him to strike his face and head on the rearview mirror of the truck resulting in 3 broken bones in his neck, 6 staples in his nose and 2 on his head.

Injured was watching the helper's side operator re-attach the hose to his drill head on a Fletcher DDR roof bolter. The helper raised the mast to attach the hose and was unaware of the injured, who was standing on the outside of the machine, placing his hand on the mast. After attaching the hose the mast was dropped and the injured fingers were caught in the mast.

The injured was installing a resin roof bolt in the roof. As he was pushing the bolt up with the head of the bolt machine the bolt bent and the drill wrench broke striking him in the face, causing a laceration to his nose and lip which required sutures to close.

EE was getting a bag of rock dust from the scoop in an attempt to rockdust the corss-cut while the scoop was operating, when he struck the back portion of his knee resulting in a bruise and one day of restricted duty.

Over-exertion in lifting objects

The injured was lifting a panel box cover for a continuous miner when he experienced pain in the lower right side of his back.

Fall to the walkway or working surface

The Employee was hanging curtin when he slipped and fell. The employee stuck his right arm out to break his fall hurting his right shoulder.

Struck by falling object

Rock fell pinning his hand against the bolter.

Employee stepped on a rock and sprained his ankle.

Individual was participating in our company wellness screening, conducted by Principal Health group. After doing a flexibility test while seated on the floor, as he got up, he stumbled and sprained his rt. knee.

Struck by falling object

Injured was struck by a falling rock, measuring 42 iches by 16 inches by 1 inch, while dismantling a ""kennedy"" stopping.

Fall from machine

Injured was walking in the center walkway toward the back of the DDr roof bolting machine to exit it when he tripped over a five gallon oil can which was left in the walkway. This caused him to fall to the pavement from the back of the machine.

Over-exertion in lifting objects

The employee was in a bent over position, with his knees slightly bent, getting ready to pick up the continuous miner cable, when he felt a pain in his lower back. The employee had not begun to lift the cable. He told his co-workers and supervisor that his back was hurting and continued to work the remainder of the shift.

Struck by flying object

The employee was standing on the right side of the Fletcher DD0 15 Roof bolter. As the bolter trammed past him to set up to begin a row of bolts, the right front tire ran over the side of a rock that was laying on the mine floor, measuring approximately 10? Long by 10? Wide by 5? thick, causing the rock to become an airborne projectile and struck the employee.

Injured was operating a DDR Fletcher roof bolter. As he stepped down from the walkway to the drill head, he sprained his foot / ankle. The step to the pavement was approx. 14 inches. The pavement was even and hard with thin mud on it.

Employee was roof bolting. As he began to drill a piece of rock fell from the area of the drill steel and struck the canopy then struck the employee on the right shoulder.

Struck by rolling or sliding object

While the employee was pulling up bolter slack, a piece of the rib fell out, striking him in the left arm that required 4 sutures.

The employee was cutting belt when he slipped cutting his arm.

The injured was drilling a hole for a belt anchor, when he let the head of the bolt machine down the drill steel stuck in the mine roof, momentarily, and then fell out of the drilled hole and struck the injured on the chin resulting in a laceration which required 2 sutures to close.

Employee was operating a Fletcher Roof Ranger on the left side. Upon completion of drilling he was in the process of removing the drill steels from the hole when a small rock fell from the roof and struck him atop the left hand causing a fracture to the metacarpal bone.

Employee and co-workers were installing a 400? piece of mine belt to the number 11 belt. The employee was in a squatted position, on his knees, cutting the belt by pulling the knife toward him. As he pulled the knife, it came from the belt and struck him in the right thigh causing an incision.

Employee was replacing a bit insert on the cutter head of the number 4 continuous miner. He was striking the bit insert with a hammer when a sliver of metal came from the metal insert and lodged behind his right knee cap. He was treated at St. Joseph's Hospital in Buckhannon, WV. The metal was removed by St. Joseph's Hospital

The employee had an acute grief reaction after the scoop he was operating ran over and caused fatal injuries to a fellow employee. This is being classified as an illness and not an injury.
