Miner walked down the walkway ramp to the boat dock and stepped from ramp to dock (an 8" tall riser ht.)Felt back pain. Did not slip or fall . We took him to E.R. Diagnosis was muscle spasms or sprain. Adm. and injection, 2 Rx's , told to stay home today and apply ice. Can repot to work on 7/21/11 if feeling ok.

Showing all 22Removed 1 section of dredge pipe (mounted on a pontoon) and pushed it to the bank of the lake with motorized work boat. The pipe ran into the bank causing pipe to slip over edge of work boat and pin miner's left leg between the pipe and bench seat of the work boat.
Walking near the booster pump and stepped into a depression or uneven area of the ground. EE went to a chiropractor on 10/31/08.
Miner was tightening packing gland on cyclone pump. Wrench slipped oof of nut, hand struck a hose clamp. Miner was wearing gloves, however, the clamp cut through glove and lacerated right thumb. Required 3 stitches, tetanus vaccination and Rx anitiodic.
2 men working on a dredge sleeve, in process of bolting up the sleeve to pipeline the dredge sleeve fell and EE grabbed one end of it to keep it from going into the lake. The suddent jolt caused an acute strain of his left shoulder
Miner was removing pump packing from a booster pump, when he lost balance & caught himself before falling and then felt something wrench back. He later felt back pain and will be going to a chiropractic clinic for treatment, as soon as he can get an appointment.
UNLOADING JUNK IRON AND A PIECE FELL ON EE'S FOOT. NO BROKEN BONES, BUT FOOT WAS BRUISED. PHYSICIAN INSTRUCTED EE TO STAY OFF FOOT 2 DAYS THEN REPORT BACK TO WORK.
Affected miner & two other miners were lifting a cable block from the rear of a mobile crane. Before the block was completely lowered to the ground, the other two miners let go of the block, which caused the affected miner to feel a jolt. After feeling discomfort for 1 week, the affected miner went to a chiropractor and received one treatment on 10-28-05.
MINER WAS TIGHTENING A CLAMP ON A DREDGE SLEEVE. THE CLAMP BROKE, CAUSING MINER TO LOOSE BALANCE AND FALL AGAINST A BRACE, RESULTING IN MINER'S BUTT STRIKING AGAINST THE BRACE, RESULTING IN FR ACTURED TAILBONE.
EE WAS HELPING REMOVE A COLLECTOR CHUTE FROM THE SCREENING PLANT AND WHILE IT WAS BEING LOWERED IT SWANG AND STRUCK THE EE ON THE RIGHT SIDE OF HIS HEAD. LACERATION REQUIRED 7 SUTURES.
EMPLOYEE BURNED LEFT FOREARM ON MUFFLER ON A SMALL BRIGGS & STRATTON MOTOR. WHILE OPERATING A WORK BOAT.
AFTER SHUT DOWN FRONT-END LOADER,EMPLOYEE WAS DESCENDING THE STEPS TO LEAVE THE MACHINE.HE SAID HIS LEG GAVE OUT CAUSING HIM TO FALL.EMPLOYEE FELL AGAINST THE WALKWAY OF THE SCALE HOUSE,STRUCK HIS HEAD ABOVE RIGHT EYEBROW CAUSING APPROX 1/2INCH LONG CUT.DEPTH OF THE CUT REQUIRED THREE SUTURES TO CLOSE THE WOUND.
EMPLOYEE WAS HELPING OTHER EMPLOYEE WORK ON THE PIPELINE AND BECAME ILL.
WALKING ON DECK OF DREDGE, SLIPPED & TWISTED BACK. S-RAY REVEALS ARTHRITIS AND CALCIUM DEPOSITS ON HIS SPINE. AT THIS TIME IT IS NOT DETERMINED WHETHTER BACK PAIN IS DUE TO INJURY OR IS DUE TO ARTHRITIS. CURRENTLY EE IS NOT WORKING, AND IS TAKING DAILY PHYSICAL THERAPY TREATMENTS PER DR.'S ORDER.
EMPLOYEE BACKED DUMP TRUCK UNDER LOAD-OUT BIN OF SCREENING PLANT AND STEPPED OUT OF TRUCK CAB ONTO A PLANK WALK-WAY. THERE WAS ICE ON THE WALKWAY WHICH CAUSED EMPLOYEE TO SLIP AND FALL, STRIKI NG HIS LEFT FOREARM, BRUISING THE MEATY PART OF HIS FOREARM.
WALKING DOWN THE PIT BANK TO DO SOME REPAIR WORK ON THE DREDGE HE SLIPPED AND TWISTED AND HYPERETENDED HIS RIGHT KNEE.
EMPLOYEE WAS WITH 2 OTHER MEN HAVING JUST COMPLETED REPAIRING THE GRAVEL PUMP ON THE DREDGE AND NOTICED A WATER LEAK ON CONNECTION OF HIGH PRESSURE WATER HOSE TO THE GRAVEL PUMP. EMPLOYEE TOUC HED THE HOSE TO EXAMINE IT AND IT BLEW OFF FROM THE CONNECTION CAUSING THE CLAMP TO CUT EMPLOYEE'S HAND BETWEN THUMB AND FOREFINGER (LEFT HAND).
EMPLOYEE WAS TALKING TO DREDGE OPERATOR ON 2-WAY RADIO ADVISING HIM TO SHUT DOWN OPERATIONS BECAUSE A THUNDERSTORM WAS ADVANCING FROM THE DISTANCE. LIGHTNING STRUCK SOMEWHERE OFF MIEN PROPERT Y AND APPARENTLY FOLLOWED POWER LINE TO MINE PLANTSITE. EMPLOYEE WAS KNOCKED DOWN BY THE LIGHTNING.
BANK SLIDE INTO LAKE CAUSED DREDGE TO ROCK. OPERATOR WAS STANDING IN CONTROL CABIN ON DREDGE. ROCKING MOTION CAUSED OPERATOR TO LOOSE BALANCE AND STRUCK WINDOW WITH RIGHT ARM GLASS BROKE AND C AUSED LACERATION ON RIGHT ELBOW, REQUIRED 7 SUTURES.

PULLING ON A WRENCH, WRENCH SLIPPED OFF. EMPLOYEE LOST BALANCE AND STRUCK BACK OF HEAD ON ANGLE IRON, CAUSING A LACERATION. REQUIRED 8 SUTURES.
JUMPED OUT OF TRUCK TO GET BULLDOZER TO PUSH SAND AND RAN INTO VEHICLE BUMPER (EMPLOYEE'S AUTOMOBILE) STRUCK LEFT KNEE ON BUMPER.
