Equipment Operator stepped off of excavator turned to walk away and stepped on a clay chunk on the ground, Stubbled but did not fall, upon catching EE's balance felt a pop in EE's left knee. Conditions in the bullpen area was dry. On 7/11/2025 Employee reported that EE's knee had gotten worse and was taken to the ER. Further treatment was given at ER EE was released to home.

Associate claimed to have fallen out mobile office and striking the metal steps that were not yet put in place for use, associate entered office by stepping across from steps that were placed 30 inches from office entrance until surface was leveled and ready for step placement, associate claims EE forgot and exited the office falling between the office and steps hitting left side

EE was removing a starter from an International truck. The bolts were seized up and EE was using a cutting torch to heat the bolts for removal. EE who is nearsighted, slid the prescription safety glasses up to ensure EE was heating the proper bolt when hot grease hit left cheek and splattered into left eye causing a chemical burn.

While performing a routine service of the D8R dozer, associate was lower the belly pan, to gain access to the drain plug associate was removing bolts holding belly pan in place upon removing the last bolt pan fell due to excess dirt in pan. associates hand was caught under the falling weight of belly pan.

While conduction preventive maintenance, associate was removing transmission filter from a John Deere 400D artic truck. The wrench he was using slipped and associate struck right elbow against bottom side of engine.

Associate was closing gate at equipment storage. While closing the gate a red wasp came towards him and associate's instinct was to jump back, when he did his left ear contacted the razor wire on the top of the gate. He received a laceration to his left ear that required 8 stitches.

Associate was dismounting equipment and to avoid stepping on a pile of mud he attempted to step over it and his foot gave way and twisted causing a stress fracture to his right foot.

Associate was helping with a repair on the dozer. He was standing on top of the pusher blade loosening a bolt to the lift cylinders. When the bolt loosened, associate lost his footing and fell. He hit his left side on the blade before he landed on the ground.

Miner stated he stooped down while talking and lost his balance. He reached out to catch himself and grabbed a ramp for loading equipment and the ramp fell hitting him in the forehead. Miner received 4 stitches to the upper forehead.

Miner was helping another miner to pick up an irrigation pipe and move it to the side when he felt a pin in his lower back. The footing was loose and his foot slip and caused additonal pressure to one side. Strained lower back area.

Associate was fueling dozer, fuel nozzle started to slip from the machine and associate reached for hose and hopped down to the ground (32"). He landed on a small hump of dirt causing a dislocation of the left ankle and a compound fracture of the lower leg bones. Surgery was performed on 1/6/15 and a no weight restriction was placed on associate for eight weeks.

Associate was unbolting a roller from a scraper without proper support and when bolt was removed the roller twisted and pinch the associate's pinky resulting in it being smashed and lacerated.

Operator was changing 7' cutting edges on road grader. Operator removed all bolts from cutting edge and found it in a bind. Operator removed 3 of the 4 bolts in the outside cutting edge when 7' cutting edge came free and fell striking him in the right shin and right foot.

The operator was dismounting his articulated dump truck after cleaning his mirrors/windows and had window cleaner bottle in one hand and a rag in the other. As he came down the steps, his muddy boots caused him to slip and fall to the ground as his hands were occupied with the items he was carrying. Cracked pelvis.

Employee was unloading scrapper teeth from a loader bucket that was lowered to the ground, and neatly stacking the teeth on a trailer. The employee was carrying one tooth (approximate weight 12.5 pounds each) in each hand, while setting down the tooth with the left hand, employee felt a "pop" in his left arm, that is when he noticed a knot on his arm and felt pain.

The miner walked down access steps from the Mining Service Trailer. His right foot slipped on the last step causing him to lose his balance. The miner suffered strains to his right hip and lower back. There was mud on the bottom of the miner's boots, the last step tilted downward approximately 5 degrees, and there was smooth metal framing the step.

Loader Operator asked Quarryman,injured EE, to move some hoses. Injured EE was busy at that moment so EE got down off his loader and walked over to injured EE. A disagreement ensued. EE then pushed injured EE to the ground. He landed on his right arm and hand. At that point, the disagreement ceased.

The employee was washing the motor on the 988 with the pressure washer while standing on the back bumper. He placed his right foot on the tire and it slipped off. He fell to the ground. His boot had oil on the bottom and he did not use a 3-point contact. A medical exam revealed that the employee had fractured his radius and wrist on the right arm.

EE was severely injured when his personal vehicle backed over the upper portion of his body while attempting to jump start it. In order to perform this task he had to place himself under the right front tire of the vehicle. Once contact was made, the vehicle rolled in reverse over EE.

Employee states that around April 28 or 29, 2004, his hand and wrist started getting sore and then later started swelling. Around that time he was running the scraper on short hauls which caused him to use the levers more often. The soreness and swelling continued and employee came in to report the pain and swelling in his hand and wrist on 5-10-04.

THE EE WAS LEANING BACK AND PULLING THE LINES. THE HOOK SLIPPED OFFTHE CABLE AND LINE CAUSING THE EE TO STUMBLE BACK A COUPLE OF FEET THEN FALLING DOWN HITTING HIS HEAD ON GROUND SURFACE.

THE EE WAS WORKING INSIDE THE MAINTENANCE SHOP DOING SOME WELDING ON THE WORKTABLE. HE WAS LEANING OVER THE STEEL SLIPS WELDING WHEN SOME OF THE HOT SLAG POPPED UP AND LANDED ON THE BACK OF HE AD. THE SLAG ROLLED DOWN HIS HEAD AND HIT THE INSIDE OF HIS SAFETY GLASSES AND A SLIVER OF SLAG BOUNCED OFF THE SAFETY GLASSES INTO HIS EYE.

2 EE'S WERE LYING UNDERNEATH A CAT 930 TIGHTENING DOWN THE BOLTS TO THE CENTER PIN. ONE WAS SPRAYING WD-40 WITH A SPRAY BOTTLE AND THE OTHER WAS USING AN AIR RATCHET. THE EE USING THE AIR RATC HET WAS ACCIDENTLY SPRAYED IN THE FACE WITH THE WD-40. THE LUBRICANT ENTERED EE'S EYES.

ON THE DAY OF THE ACCIDENT. EE SAW A DR. ON 1/08/03.
