Showing 50 of 98While walking from EE's pickup to the shop, the miner felt something snap in EE's right calf. EE's leg gave out and EE fell to the ground. The miner suffered a right calf strain and placed on walking restrictions.
The employee was welding on a water truck frame when a spark went behind EE's welding helmet and glasses and struck EE's eye. The employee had 2 pieces of embedded metal removed from EE's right eye by the doctor. Employee reported injury 5/3/18. No time missed, employee was wearing welding jacket, helmet, safety glasses, steel toed boots, and leather gloves.
While removing a machine guard on the overland belt tail pulley, the miners left hand was pinched between the machine guard and frame. The tail pulley was filled with rock. A rock moved while removing the guard causing the guard to shift and pinch miners hand. The miner suffered a crushing injury/contusion to left hand. No open wound was caused and an x-ray revealed no fracture.
The miner was feeding a new belt onto a belt conveyor when the spreader board on a chain hoist skipped free. The board flew out and struck the miner in the chest.
The miner slipped and fell on EE's back and rolled 3 to 4 feet. The miner had right shoulder pain. Original diagnosis of right shoulder sprain on 4/24/17. The miner had no restrictions/full duty and has had no lost time from the accident. On 7/7/17, diagnosis changed to a torn rotator cuff and is scheduled for Right Shoulder Surgery on 7/12/17.
While moving screen mesh from storage, the screen mesh slipped and smashed the miners right middle finger causing a contusion. The miner was wearing gloves, hardhat, and safety glasses during the incident.
Miner was walking down a ramp and tripped on something causing them to fall down stairs of a cone screen. Miner said they were not sure what they tripped on and there was no debris on the walkway. Miner fractured right ankle after the fall. Miner fell approximately 4-6 steps from the landing. The miner was not carrying anything during the incident.
Individual was clearing dirt around the plant with a skid loader when loader hit a large rock. This pushed employee forward into the safety bar causing a bruise to chest on the left side. Individual was wearing a seatbelt.
When changing screens inside of screen deck pulled or strained neck muscle
When swinging a sledgehammer at a rock, missed the rock causing hand to contact the discharge chute that the rock was lodged in, causing a laceration to the finger and cutting the tip of the finger off.
TRUCK HAD MECHANICAL FAILURE, MINER WAS STRUCK BY THE OPEN DOOR KNOCKING HIM TO THE GROUND CAUSING LEFT KNEE, RIBS AND ANKLE PAIN. MINER WAS GIVEN Rx FOR PAIN AND SWELLING, ICE PACK AND KNEE IMMOBILIZER.
