Associate was shoveling rock off of our 30" silo belt. EE was going to go to break and started walking down the belt. As EE was walking down the belt, EE slipped and fell onto a metal chute. The associate landed on the right side rib cage, breaking four ribs. A contributing factor was that it had started to rain, which caused the belt to become slippery.

Showing all 44Associate was attempting to dispose of an air filter, from a 988 FE Loader, when ee went to lift it into the dumpster ee heard shoulder pop. Upon examination from a doctor it was determined that the injury was a torn rotator cuff requiring surgery to repair.
Employee was helping change a flight after watching and being instructed how to do so. While laying one end down, employee's finger was pinched between the steel flight and steel chain the flight fits into to pull material through crusher. Finger was cut through glove. Received 8 stitches and returned to work.
Employee was helping powder and stepped on rock rolling an ankle and falling on their buttocks and left hip area. Employee didn't feel they needed medical evaluation. Two weeks later employee started feeling sharp pain down left leg. This became reportable on Oct. 27, 2017 when employee was taken to Chiropractor for a bulging disk in back. Received treatment and realignment.
Employee was walking with a 50 lb. bag of Anfo and EE's back went out.
Quarry Truck Driver had gone to Rotan Gin to check loaded weight of truck and trailer. He got out of his truck went to the window of the scale house to check weight; once checked he was going back to his truck and stepped off a raised concrete porch that was uneven lost his balance and fell to concrete hitting his knee.
Employee was walking around pick up truck, tripped on a dirt clod and to keep from falling he grabbed the tailgate and cut his left ring finger.
EE slipped while going from steps of the loader to the steps of the truck causing him to hold on with one hand to prevent fall to ground. Resulting in hurting his shoulder.

Unknown, Employee is required to wear hearing protection. Annual hearing test indicated Employee had a standard threshold shift in his right ear. Conditions outside of work could have contributed to this condition also.

Employee got off the scraper and was walking to the shop. He stated he stepped on a clod of dirt, twisted his knee and fell.

Trailer bed was raised to disconnect it and when it was lowered again, employee had his hand on the trailer brace and his thumb was crushed.
EE reported on 7/9/07 that his right hand/fingers were going numb. He states it's from operating the front end loader. He indicated it started on 6/1/07. He did not report it until 7/9/07. Dr. issued him a wrist splint. He was allowed to go back to work immediately with no restrictions or lost work time. He has a follow-up appointment on 7/20/07.
Employee was loading a truck from a bin. The gate stuck on the chute he was using so he used a pipe wrench to help close the gate. The pipe wrench slipped and smashed his index finger on left hand between the wrench and guard.
Employee was walking down slope to bottom of B2 when he slipped and twisted right knee. Doctor's appt on 6-7-07 indicated he recommends a right knee arthroscopy.
Employee was helping blaster tape on fuses and get ready to blast. While walking back to pickup he stepped in a hole and wrenched his back.
Employee was hauling rock with a Cline truck out of underground mine. When he got to the portal he hit a section of rough road. This caused the truck to bounce and jarred his back.
Employee was getting out of his vehicle in the parking lot. As he placed his right foot on the ground, it slipped out from under him. He caught the door of his vehicle preventing a fall, but felt a pain in his back. The parking lot was covered with hard-packed snow.
EE WAS GUIDING BELTS ONTO A MOTOR AT THE SCREEN HOUSE WHEN HIS FINGER SLIPPED BETWEEN THE BELT AND THE SHEAVE CAUSING LACERATION AND FRACTURE TO HIS LEFT SMALL FINGER.
WHILE STEPPING UP INTO A TRACTOR THE EMPLOYEE'S LEFT HAND SLIPPED OFF THE HANDLE AND HE FELL BACKWARDS TO THE SHOP FLOOR. WHILE ATTEMPTING TO CATCH HIMSELF HE INJURED HIS LEFT WRIST.
EE WAS ON HIS KNEES MATCHING UP GEARS & AXLE ON JCI LOADER. WHILE GETTING UP FROM THE PAD HIS LEFT KNEE TWISTED.
2 EMPLOYEES WERE PERFORMING CLEAN UP BY PICKING UP SCRAP STEEL & PUTTING IT IN THE LOADER BUCKET SO IT COULD BE MOVED & DISPOSED OF. A PIECE OF STEEL FELL OUT OF BUCKET ONTO EMPLOYEE'S FOOT.
EE WAS CLEANING WINDOW OF HIS TRUCK. AS HE WAS DISMOUNTING TRUCK HIS FOOT SLIPPED CAUSING HIM TO LAND ON BOTH FEET RESULTING IN BACK PAIN.
EMPLOYEE WAS CLOSING A DOOR ON TRAILER. ANOTHER EMPLOYEE WAS GETTING OUT OF PICKUP TRUCK TO CLOSE THE TAILGATE, THINKING HE PUT THE TRUCK INTO PARK. TRUCK WENT INTO REVERSE AND THE TAILGATE HI T THE FIRST EMPLOYEE IN THE BACK OF LEGS AND HE HIT THE BACK OF THE TRAILER WITH THE FRONT OF HIS LEGS.
EE WAS WALKING FROM WASH HOUSE TO PORTAL WHEN HE SLIPPED AND FELL ONTHE SNOW. PULLING RIGHT HAMSTRING.
EE REPORTED HIS WRISTS WERE HURTING & HE WAS HAVING NUMBNESS IN HIS HANDS & LOWER ARMS. I WAS INFORMED ON 1/9/02 THAT HE HAS CARPAL TUNNEL SYNDROME IN BOTH WRISTS WHICH MAY RESULT IN LOST TIME IN THE FUTURE.
EMPLOYEE COMPLAINED OF ELBOW PAIN ON 10/29/01. WAS REFERRED TO ORTHOPEDIC DOCTOR AND RECEIVED CORTOZONE SHOT ON 11/27/01. RESTRICTED DUTY FOR 3 WEEKS.
EE FELL A MINE PORTAL & HIT KNEE WENT TO DR ON 11/27/01 & NEEDS TO WEAR KNEE SLEEVE. HAS APPT WITH ORTHOPEDIC DR ON 12/07/01
THE OPERATOR DESCENDED FROM THE CAB OF A 769C HAUL TRUCK ON THE PASSENGER SIDE. HE STEPPED ON A FAULTY STEP AND FELL, WHILE HOLDING THE LADDER WITH HIS HANDS, TO THE GROUND. THE INCIDENT RESUL TED INA STRAINED KNEE.
EMPLOYEE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME. EE REPORTED THAT USE OF DRILL CONTROLS STARTED SYMPTOMS. SURGERY ON 6-21-01.

WHILE DRIVING A HAUL TRUCK HE HIT A BUMP AND JARRED HIS BACK.
THE EMPLOYEE WAS IN THE CRUSHER HOPPER ATTEMPTING TO UNPLUG IT. HE WAS PUTTING A CHAIN AROUND A ROCK WHEN THE ROCK SLIPPED PINCHING HIS RIGHT LITTLE FINGER, FRACTURING THE TIP OF THE FINGER.
DRIVING HAUL TRUCK IN A STRIPPING OPERATION, APPLIED BRAKES, CLAIMS TO HAVE HAD SEAT IMPROPERLY ADJUSTED CAUSING FACE TO HIT STEERING WHEEL.
EMPLOYEE STEPPED OFF OF LADDER AND HIS FOOT HIT CYLINDER LAYING ON FLOOR CAUSING HIM TO FALL ON DRIVE SHAFT LYING ON FLOOR FRACTURING A LEFT RIB
EMPLOYEE WAS SETTING UP A SUMP PUMP IN PIT WHEN HE SLIPPED AND FELL STRAINING HIS LOWER BACK. HEDID NOT WANT MEDICAL TREATMENT AT THIS TIME, BUTWANTED TO SEE A DOCTOR LATER. HE WAS SENT FOR MEDICAL TREATMENT ON 8/31/00.
