EE WAS SPOT BOLTING TRACK ENTRY WHEN A PIECE OF ROOF FELL STRIKING EE IN EE'S LEFT ARM AND HAND
EE WAS BOLTING. A ROCK CAME DOWN HITTING EE'S HAND ON THE ROOF BOLTER FRAME MASHING THE MIDDLE FINGER ON RIGHT HAND.
The EE was hanging ventilation curtain when a piece of shale became dislodged striking the EE on the top of EE's Right hand. The resulting injury required stitches.
Operating Continuous Miner. A piece of rock, 2'x 4'x 6" fell from the roof and landed on EE's right foot. Fractured EE's little toe.
EE was operating the miner in the #5 entry. EE was cutting #5 to #6 break. A rock measuring 3'x4'x2" to 10" thick fell from the corner striking EE on the back. EE was knocked down to the mine floor and pinned with the remote box underneath EE.
EE was cutting a timber off so it could be reset. A piece of draw rock fell and hit EE's hard hat first then it slid off EE's hat and struck EE's left hand. EE sustained a laceration requiring stitches to close the wound.
Tramming bolter; rock fell from top striking IW on left shoulder
EE was tramming the roof bolter from the side controls when stone fell from the rib line and the bolt knocking EE to the ground, then more stone fell and struck EE on the right leg and ankle causing a fracture to leg and a laceration to ankle requiring 2 stitches to close the wound.
EE had right hand on the drill pot on the roof bolter and while reaching to pick up a roof bolt with left hand a piece of draw rock fell striking EE just above the wrist. The draw rock measured 6" x 8" x 2" thick. EE sustained a laceration requiring stitches.
Injured swung boom of roof bolter out to put up outside pin. The steel struck a piece of top coal. The piece of coal fell and struck the injured on left foot. Employee suffered a fracture.
On 7-20-16 EE was walking across # 1 unit and walked through a wing curtain in # 4 entry hitting a brow breaking hard hat. Reported accident, but employee lost no work days until 8-13-2018. Employee had medical treatment related to injury and now off work.
Injured got glove caught between pinner steels. EE applied rotation and EE's left hand, wrist and arm were injured when the steel rotated.
While knocking cement block out of an old regulator, the employee was struck by a piece of stone measuring approx. 3'x 4'x 3". The stone knocked the employee to the ground and the employee received fractures to the sternum and back.
While installing the center bolt in the face area of an entry, a piece of stone fell from inby the T-bar and rolled backwards striking the employee on the back of the right foot as he attempted to move out of the way. The incident resulted in fractures to the employee's foot and toes.
While drilling a hole for a roof bolt, a piece of stone broke off and struck the employee on the outside of the left leg bending the leg to the side. The employee continued to work but he had surgery done on his left knee on 11/24/2015 due to the injury.
While installing roof bolts in the x-cut 5-6 room, the employee installed a rib bolt and then swung in to install the middle bolt. When he swung the bolter head in, a piece of stone approx. 24"x12"x1" fell from between his last bolt and the rib and struck him on the calf of the right leg. The accident resulted in a non-displaced fracture to his right fibula.
While operating a double boom Fletcher roof bolter in the #9 entry bolting a turned left on the #1 unit, while installing the outside rib pin on the opposite operator side, the injured was struck by a piece of draw rock that fell out from between 2 pins. The rock fell out and struck the right ankle. He suffered a hairline fracture that required a boot cast be worn.
Injured employee stated he was preparing to drill a bolt hole, put the drill steel against the roof and as he was withdrawing his hand a piece of rock fell and struck him on the right hand resulting in a laceration to his index finger.
Injured employee stated he was swinging the twin boom roof bolter boom to install another bolt when loose material fell and struck him on the back of his head and neck.
Employee stated he was roof bolting, changing a drill steel when a rock fell and struck his right middle finger.
Injured employee was walking through a x-cut when a piece of rock fell and struck him on the back of his head.
While scaling down a piece of stone, the employee was struck on the right foot by a piece that he did not see falling. This resulted in a fracture to his right little toe.
The employee was reaching over to get a roof bolt, that was leaning on the rib, and was struck on the top of the hand, above the glove line, by a piece of stone that fell between the bolt and the rib. The stone caused a laceration that required 4 stitches.
The continuous miner had started into a left hand turn on the second lift off the pillar. A roof fall approx. 22'x 25' ranging in thickness from 12" to 22" occurred, striking the employee causing injury to his right knee.
While walking in the neutral B entry of the 5 left off 4 north section, the EE heard a cracking sound in the roof. As he turned to observe a piece of stone fell and struck him on the left leg fracturing his ankle.
While observing the roof bolters bolting a cut, the supervisor laid his on top of the bolter near the mid section to rest himself. A piece of stone approx. 12" X 12" X 2" fell and struck his right thumb causing a laceration requiring stitches.
Employee was roof bolting in our 5 North Section when a stone fell out of the top striking his R hand and wrist, cutting his middle finger and ring finger and also cutting his R wrist. A total of 11 stitches were required to close the cuts.
EE just finished bolting a cut when he went to set down on the bolter he heard a cracking sound as he looked up a stone fell from the roof and hit him in the face, which caused a laceration to the bridge of his nose and to the left cheek requiring 2 stitches in each cut.
While bolting the bolter operator put his first auger against the roof and hit his rotation. A piece of stone broke off and struck him lacerating his lip and chin requiring 5 stitches.
EE was using starter steel to start hole for roof bolt when a piece of draw rock fell and struck his left forearm causing a laceration requiring 5 stitches.
Employee was installing cable bolts when a piece of draw rock fell from between installed roof bolts striking him on his back requiring stitches.
While kneeling down a stone fell out from between the bolts and hit him on the back and knocked him to the ground.
wHILE TALKING TO CO-WORKER THE INJURED PERSON HEARD A CRACK IN THE TOP AND HE TURNED AND LOOKED UP AND A STONE FELL FROM THE TOP STRIKING HIM IN THE LEFT CHEEK AREA AND LACERATING HIS CHEEKBONE WHICH HE RECEIVED 4 STITCHES
Miner was installing the last row of bolts in #6 rt. While pulling a piece of rock in the face, the rib brow on the left side of the entry fell, hitting him on the rt. ankle.
EE was struck by a piece of roof rock that fell on roof bolter and slid down his knee. Injury resulted in miner treated and required to ambulate on crutches. This report is being generated under protest to terminate citation #9006089.
A brow fell and hit employee while he was operating a miner resulting in pain in the center of his back.
Rock fell from the roof while swinging the drill arm on the bolter. The rock struck the miner on the right elbow resulting in a laceration.
