Helping another maintenance employee lift and position the metal refractory door to the combustion chamber to the No. 1 Hammer Mill, he strained his lower back. Treating physician issued work restrictions for one week. Employee (Maintenance Electrician) continues working (light duty) and has not lost time.

Employee was moving a metal bottom elevator idler (spare part) in the Store Room and strained his lower back (right side). He did not report to Management until August 20. Doctor issued work restrictions. Employee continues to work on light duty.

Injured employee (Maintenance Oiler) was helping another maintenance employee lift & position the product pipe on top of Roller Mill #2 and twisted his left knee. Pre-existing condition is the primary contributing factor. He was evaluated by an orthopedic doctor on July 2 who placed him on restricted work duty (no lost time).

While re-installing a mesh guard, the maintenance mechanic's right hand's forefinger was lacerated (stitches were needed to close the wound).

Unclear. Employee's statement is that as he was stacking 50-lb. bags at the end of the conveyor belt from No. 3 Bagger he felt a sharp pain in his right wrist. Employee's wrist was not immobilized (cast) with work restrictions until April 10.

While positioning No. 1 Roller Mill's 150-hp fan motor with forklift boom, chain, & cable, the motor became wedged. Three maintenance mechanics physically pulled & pushed and the motor came free, swung, and crushed one mechanic's left forearm (pinned between the gear box & drive coupling hub). The one injured mechanic required emergency medical treatment & surgery.

Injured employee was physically pulling the vacuum hose of the HiVac with 2 other employees and alleges he felt pain in his back. Begain losing time due to alleged back pain on November 30. Interestingly, employee has a history of pre-existing back problem conditions (non-work-related minor league baseball and alleged work-related injury reported on July 6, 2004).

While turning a wheel valve, employee caught his right hand's ring finger. Employee continued working. Employee did not require medical treatment until July 26. Employee was able to return to work and his job functions allow him to continue working without violating his issued work restrictions. Treating physician did not immobilize the finger until August 3.

Treating physician diagnoses as an aggravation to his pre-existing illness, which is non-work related. Employee has been placed on light duty until circumstances are resolved.

Injured employee was bagging at the No. 5 Bagger and brushed his right elbow up against the bagging machine's loading spout, which was sharp due to the natural wear of the product, causing a laceration that required four stitches. Employee returned to work full-duty.

Injured employee was stacking 50-lb. bags on a wooden pallet. After laying a bag on a pallet he raised himself back upwards and felt discomfort in his back. The discomfort continued so he was taken for medical examination and diagnosed with a lower back strain. Treating doctor issued work restrictions.

Injured employee was stacking 50-lb. bags on a wooden pallet. He reached down to adjust a bag on the wooden pallet and alleges he felt a burning pain on his Right Side. Treating physician at the Hospital suspected a Hernia; however, Specialist confirmed as Groin Strain on April 26, 2005. Employee has work restrictions and has returned to work on Light Duty.

Injured EE, #1 Loader Operator, was helping the Mechanic replace the #1 Loader's battery. Injured employee's right, little finger was accidentally caught between the new battery (weight about 50-lbs.) & the battery box on the loader. X-rays indicated fracture of employee's right, little finger and work restrictions were issued. Employee returned to work.

Injured employee was checking product bin levels outdoors when he felt something in his left eye. Employee could not specify when & where his eye became discomforted. Employee alleges that some fugitive dust entered his left eye. Employee was wearing appropriate safety glasses. Treating physician prescribed antibiotic eye drops & an eye patch for the day of injury only.

While removing plates in the 30" x 42" Austin Westin jaw crusher, one welded eye disconnected as the crane was lifting the plate. The welded eye struck the injured employee in the upper left chest area breaking #6 and #7 ribs.

EMPLOYEE WAS LIFTING A 48 X 48 WOODEN PALLET WHEN HE FELT A TINGLE IN HIS BACK. ON THE WAY HOME THAT EVENING HE EXPERIENCED PAIN AND NUMBNESS IN HIS BACK AND LEG.

Employee was pushing rods into rod mill & felt a sharp pain in his neck & back. Employee was instructed to see the doctor on 6-23-04. Doctor placed him on restricted work duty. Employer put him out of work because he could not meet the physical job requirements. Employee will be allowed back to work when his doctor has lifted these restrictions.

EMPLOYEE WAS LIFTING AN ITEM IN AN AWKWARD POSITION AND STRAINED HIS BACK. THE ITEM WAS REPORTED NOT TO BE VERY HEAVY. EMPLOYEE WAS LOADING A RAIL CAR. HE WAS M0VING A LOADING HOSE FROM ONE HOLE TO ANOTHER. IT APPEARS THAT HE LIFTED AND TWISTED WITH HIS BACK AT THE SAME TIME. RESULT WAS PULLED MUSCLE.

EE was tightening bolts in jaw crusher whe he felt something pop in his left shoulder. It was determined that the trapezius muscle in left shoulder was pulled. As of 3-30-04, ee has returned to work on light duty.

EE WAS HEATING SHRINK WRAP AROUND PRODUCT. THE SHRINK WRAP FLAMED UP. EE STRUCK SHRINK WRAP WITH BACK OF HAND & PLASTIC STUCK TO HIS HAND CAUSING A BURN. THE DR PLACED EE ON ONE HANDED WORK UN TIL HAND STARTED TO HEAL.

ANOTHER EMPLOYEE WAS ON UPPER LEVEL. SOME MATERIAL WAS KICKED OFF THAT LEVEL AT THE SAME TIME THE INJURED EE LOOKED UP. MATERIAL FELL INTO EE'S EYES AROUND HIS SAFETY GLASSES.

EE WAS REPLACING LINING IN A 6X 10 ROD MILL. EE ALONG WITH ANOTHER EE WAS TRYING TO WORK A PIECE OF LINING INTO PLACE WHEN THE LINING MOVED SUDDENLY, PINNING THE EE'S FINGER BETWEEN ANOTHER PI ECE OF LINING. THE EE WAS TAKEN TO THE DR. EE HAD A CUT, AND A BONE CHIPPED ON END OF FINGER AT THE FIRST JOINT. EE HAD 8 STITCHES AND A SPLINT.LIGHT DUTY.

EE HAD JUST CLEANED 2 TROUGHING IDLERS. EE TOOK LOCK AND TAG OFF AND STARTED BELT TO SEE IF THE BELT WAS GOING TO TRACK, ONE OF THE ROLLERS ON THE IDLER WAS NOT TURNING, EE REACHED THE TOP ROL LER WITH HAMMER TO START IT TURNING. WHEN EE DIDHIS GLOVE WAS CAUGHT BY THE BELT AND HIS LEFT HAND WAS PULLED INTO THE ROLLER AND BELT.
