EE was trying to get the handle to slide shut on the back door of the container. It was not closing so EE was hitting the handle with right hand. After a few times, EE's hand missed the handle and hit the container.

Miner was injured while attaching metal pull cable to haul truck stuck on the dump site. A frayed wire on the cable struck and lacerated EE's right leg. This resulted in 7 staples to close the wound.

EE was prepping containers, when EE grabbed a liner bag off of a stack of liner bags to pull the bag to forks of the forklift, the wooden part of the liner caught the side of the door frame and pulled EE back causing a pain in EE's right shoulder.

An employee was walking down the incline ramp to retrieve tools in cart and experienced sharp pain in left knee. Medical was contacted and employee seen by contract doctor. After going to see medical provider, employee returned to duty. Restricted duty was not relayed to management at time of return.

Operator attempted to pull 50lb bag from top of completed stacked pallet. Bag fell from top jerking employee's arm down. Employee worked without restriction until 2 Feb when they went out of work for what was told was unrelated work injury. 8 Apr was notified that the injury was work related.

Brick layer was assisting to remove refractory debris from the kiln. They reached for a piece of debris and a second piece of debris shifted onto the 5th finger, pinching the tip. Employee was wearing full face respirator, hard hat, steel toe shoes and leather gloves.

Mechanic reported back soreness on 11/7/18 after placing motor into bracket and was seen by site medical with first aid treatment. However, on 11/15/18 had follow up with site medical with work restrictions prescribed and follow up evaluation with back specialist ordered.

A Lab Technician experienced a skin injury when EE spilled Nitric acid from a test tube while performing standard lab test under a lab hood. The spilled liquid contacted EE's neck and arm creating small first degree chemical burns on skin. The Doctor prescribed antibiotics and prescription burn cream. The employee returned without restrictions the same day.

Installing 11 gage stainless steel plate on 11G spray driver. While lowering the plate through the scaffolds, plate hung up on the 2nd or 3rd scaffold, plate shifted and hit employee.

Injured employee was helping to demolish and old fence when a piece of angle iron fell striking employee on the upper right arm. The arm was not broken but there was a soft tissue injury. The employee has returned to work on restricted duty as of 10/28/2016.

Mechanic was exiting a dryer control room. The mechanic placed his left hand on the door to control the door. Fifth finger (pinky) was pinched between door and frame causing laceration above finger nail bed. Employee received stitches and prescription for antibiotics and pain. Returned to work with restricted duty.

While attempting to climb on machine employee stepped on bottom step and reached up and grabbed hand rail while pulling up felt a pop in left shoulder experiencing pain in L. arm. Diagnosis no use left arm/must wear sling, muscle in left shoulder damaged pending Arthrogram.

Mechanic was cleaning out a six-inch drain pipe in a concrete pit. The gate valve in the pit is designed to leak minimally as it opens and closes. The mechanic was cleaning the pipe with a water hose by running it up the line. As he was pulling the hose back out of the drain pipe, his elbow ran across the edge of a piece of expanded metal being used to guard the nearby gate valve.

MidGA - Gordon. A Gordon employee received an injury when opening the #7 filter vacuum pump valve. The handle broke lacerating his left index finger through leather work gloves worn as proper PPE for the task. The employee was transported to the Medical Center in Macon where he was treated and received four stitches along with a prescription for antibiotics.

On 3/28/11 at 12:15 am the 2C big bag operator was stepping down in to the loading room from the operator station when he felt a pain in his right knee. The step is about 8-9 inches.

Employee was shoveling surface soil into drum when he felt a sharp pain in in shoulder. Employee was transported to local ER for treatment. ER physician determined employee had a muscle spasm. Employee claimed he was in severe pain. Employee received injections for pain control and muscle relaxer. Employee was released to return to work his next shift without restrictions.

The employee was loading product into a bulk truck. In the process of lowering the product chute into the opening at the top of the truck, he pinched his left ring finger between the handle of the chute and the platform railing. It was originally diagnosed as a contusion. An x-ray later revealed a tiny fracture.

A Translink operator was pulling a filled 50lb bag from the packer spout to place on the conveying system when he lost his grip on the bag. In an attempt to prevent the bag from hitting the floor, the operator tried to regain control of the bag and felt a pain in his lower back. Late reporting of injury due to causation investigation. Injury cause was determined on 11/01/10.

Employee pumped liquid kaolin into a tanker trailer without venting the trailer. The trailer consequently pressurized itself. When he attempted to vent the trailer while unloading, he unscrewed a lid on the top of the trailer and the pressure inside the trailer caused the lid to pop off and hit him in the nose. Sustained a laceration to the nose.

Prior to accident & during crane steup, operator damaged auxilary lifting sheave on crane. He failed to report or to take action to repair damage. After job completeion EE was helping operator prepare crane for removal from job site. At this time damaged auxilary sheave fell from crane striking and injuring EE. Accident was direct result of EE's misconduct.

The employee was investigating an operational problem. While he was investigating, dilute silicate solution (high pH) splashed around the rim of his safety glasses and ran into his right eye. The silicate solution came from a sample valve that had been left open on the previous shift.

EE was assisting drilling crew in connecting cables & pipe, stepping up & down from the drill rig platform (repetitive motion). He felt a slight pain in lower back. EE has a previous back injury dating back to 2004 with surgery & restricted duty. He remained on restricted duty & had reoccurence in 2008 & now another & is scheduled for Dr. appt. 1/29/10.

Operator was disconnecting a hose from a tank. The hose was depressurized and believed to be empty, but apparently had residual recovered sodium silicate. As the operator was disconnecting the hose, the chemical splashed onto his face and under his safety glasses, resulting in irritation to the eyes.

A Mechanic suffered a strain to his left shoulder while he, along with another employee, supported a gas valve assembly that was being removed for replacement during a Calciner outage. The physican diagnosed a muscle strain. The Mechanic continued to have pain in his shoulder. The employee had a MRI which revealed a small tear in the rotator cuff.

EE was looking for a bucket to collect a sample from a stockpile. He lifted an equipment rim to pull the bucket out from under it. His hand slipped off the rim and it fell on his finger crushing it. He went to the emergency room in Gallup New Mexico where he was treated. His injury required two stitches and a splint to protect his finger.

#4 Silicate Concentrator area sump pump not pumping material. While investigating, pump discharged liquid & struck ee in face; knocked off safety glasses, liquid went into eyes. Flushed eyes at eye wash station. Examined by site nurse before being transported to see local opthamologist who prescribed antibiotic ointment, pain medication & released employee.

Two operators were changing out a roller on a filter belt. One operator was supporting the roller. The other operator was unbolting the bracket that holds the roller in place. The operator supporting the roller, stopped supporting it. The bolt broke, the roller fell and struck the right middle finger of the operator who was unbolting the roller.

A mechanic worked on a dust collector bag change on 5/22. The next Tues,he called in with complaints of back pain, but did not indicate a work related injury. While receiving medical attention, he reasoned that the bag change must be the cause of injury. On 6/13, he met with supervision to make a W/C claim. An investigation is in process. Awaiting assessment by physicians.

Employee was assigned to install a manway cover and valve on Tank #22 and was instructed that a co-worker would assist him. However, EE lifted the manway cover alone and states hurting his lower back. EE sought medical attention by MD on 9/4/07.
