Over-exertion (Not Elsewhere Classified)

Operator was walking to EE's work area and felt a ""pop"" in EE's fourth toe on EE's right foot. Employee was taken to the physician and was later determined o have a hairline fracture in EE's right foot.

Absorption of radiations, caustics, toxic and noxious substances

An employee was sprayed with hydraulic fluid while operating a forklift in the warehouse area. The employee was traveling when a weakened portion of a hydraulic line burst, spraying the employee's head and torso area. Fluid spray contacted the forehead and moved down towards the left eye. Doctor prescribed antibiotic eyedrops. Returned to work same day.

Over-exertion in lifting objects

The injured worker was cleaning up & lifted the old wire (6 gage) & began to walk up the pond bank & felt back ""crunch"", but did not really hurt at that time. The injured worker finished cleaning up with a Sun Chemical Maintenance worker & sat in the office to cool. When the worker went to get up later, back was locked up & could hardly walk.

Struck against a moving object

Trucks hauling wet mica material onto stockpile. Trucks had to be pushed out by the dozer. Dozer was pushing truck 2362 Truck caught traction causing a gap. Dozer operator was unable to stop the machine, and the blade of the dozer made contact with the dump trailer. This contact caused a jar to the truck driver, aggravating sciatic nerve

The employee was attempting to dislodge stuck pallets from a dump truck by vibrating the tail gate when he cut his middle finger on a piece of sharp metal on the tail gate.

At 8:45 am on 7/16/2012, a maintenance technician received a laceration to the right thumb requiring 5 stitches to close. The technician was attempting to remove the insulation on an electrical lug nut in preparation for changing out a centrifuge motor. As the technician was cutting the insulation, the knife slipped, making a one inch cut on his thumb.

A maintenance technician at the Hartwell site received a laceration to his right index finger requiring 10 stitches. The technician was removing a 6 inch diameter x 3 foot long rubber boot from a 48? Dewco vibratory screen. The technician was taken to the Hart County Emergency Room for treatment and released.

EE WAS PERFORMING HOUSEKEEPING DUTIES ON A 9"" ELEVATED PLATFORM-AS HE LEANED TO LEFT SIDE OF WORK AREA, HE LOST BALANCE AND FELL-HE EXTENDED HIS LEFT ARM TO BRACE AGAINST THE IMPACT AND STRUCK STEEL BASE PLATE WITH LEFT HAND. CONTACT WITH PLATE CAUSED A 3"" LACERATION TO HIS LEFT PALM. EE TRANSPORTED TO CO DR AND 13 SUTURES WERE REQUIRED TO CLOSE THE WOUND.
