Showing 50 of 67During the removal of a sheave from a pump assembly, using a block of wood as a protective buffer while applying force with a sledgehammer. EE A was holding the block of wood against the sheave while EE B struck the block with the sledgehammer. During one of the swings, EE A attempted to reposition the block of wood, & the finger was pinched/smashed between the wood and the sheave.
Effected employee (EE) was helping load a grease barrel on a fuel truck when another employee was on top of the truck pulling the top of the barrel causing the bottom of it to slide out and pinch EE's left thumb under the barrel and deck of the truck. The barrel was 16 gallons and weight approx. 120 lbs. Weather conditions were gloomy and misting rain.
Operator in Training (O1) was tasked with wash down duties on the north side of the Mill Washer Building. While performing the assigned task O1 used both hands to close a 2" ball valve with an oval handle. While closing the valve, O1 felt a pop in their left ring finger. O1 was evaluated at an offsite facility, where imaging revealed a chipped bone in the finger.
EE reports low back pain that began while working on a pump this morning. EE describes middle of the back pain that is keeping EE from being able to move or bend in any direction. EE states unable to work with the pain. EE reports previous low back problems that normally resolve with prescriptions and rest.
EE's right hand was caught between the belts & the sheave while in the process of changing drive belts on final product pump. EE had to roll sheave in the opposite direction to remove the stuck hand. EE visited Clinic for after-hours treatment & returned to Clinic on 7/14/25, had decreased grip strength, swelling to thumb, ring & little finger. Hand was obviously painful to move.
EE called Clinic to report neck strain that occurred while pulling hose, prevented return to work the next morning. EE reports stiffness in neck that prevents turning of head left or right. EE was instructed to visit the Clinic for evaluation by company MD. EE evaluated and referred to clinic for evaluation and treatment.
Nutrien Employee was in the process of loading a mines power cable PLM head into the reel of the reel loader. The cable snagged, causing NE to drop the PLM head. The head fell onto NE's right ring finger pinching it against the reel.
EE reported left knee pain after working in the dragline tub replacing tub paper. EE states that the knee was hurting too bad to straighten all the way and that it hurts too much to bear weight on it. Upon presentation in clinic, there was notable swelling to the front of the left knee.
EE was taking apart a lube line and twisted wrist. After a few minutes EE reported pain in wrist and was checked out by EMT on shift.
EE called and reported left knee pain while getting on and off a D8 Bulldozer. EE states that the incident happened on Friday night and that the pain has increased since becoming bad enough that it prevented EE from being able to work Sunday & Monday night.
EE reported pop sensation in lower back during climb into boom truck. EE states left shop and drove to work area before realizing that low back was too painful to allow standing up straight.
Employee alleges that EE strained EE's lower back while performing repairs on an excavator. EE did not notify management until 2/10/2025. EE states that EE was handling a started when the event occurred.
EE reports packing #9 lift on the #2/4 matrix line closing the matrix bleed-off valve to release the line back into operation when low back pain was experienced. EE reported the injury the next morning by calling as the EE was unable to get dressed to return to work. EE requested off-site evaluation and was sent to Company Care for evaluation and treatment.
Mill Maintenance Mechanic (M1) bent down on their right knee to observe the rotary gate valve at the #6 calciner. M1 began feeling pain in their right knee as they attempted to stand back up from the kneeling position. M1 was transported to the onsite clinic and was later transferred to an offsite facility for additional evaluation.
EE called Clinic on 11/18/2024 to report getting a blister from new work boots on 11/14/2024. EE missed work on 11/15/2024 and went to personal doctor. PMD treating wound for infection and provided prescription antibiotics. Followed-up with PMD and received instructions to remain off work and follow-up with podiatrist.
EE reports something pop in right heel followed by pain with a burning sensation while in the process of climbing onto a dozer in the process of completing pre-shift inspection.
EE states pulling a cable yesterday and experienced discomfort in right shoulder that has become worse throughout the day. States that the pain in shoulder is causing decreased range of motion, numbness and tingling in the fingers of the right hand. Restrictions were given that cannot be accommodated, off work until further notice.
Field tech knelt down on D6 dozer track looking for coolant leak and felt a pop in EE's (L) knee. EE stood up and had slight discomfort. Tech contacted immediate supervisor to notify them. EE continued to work but discomfort became worse and swelling began. Tech contacted work partners and was advised to ice knee and stay off of it.
EE states while lifting a bucket cap & pin, felt a sharp/stab pain in mid back & immediately dropped the bucket cap. Felt pain throughout the day but did not report as work related & was going to PMD on 7/8/24. On 7/9/24 EE reported the back injury as work related & was referred to Company Care. Restrictions were given that cannot be accommodated, off work until further notice.
Employee reports right knee popped while pulling on a rope with a twisting motion of body. EE was referred to Company Care for Evaluation. Restrictions were given that cannot be accommodated. EE is off work until further notice.
C1 was assigned wash down duties. C1 placed a nozzle on the hose and began opening the water valve. As water pressure increased the hose nozzle moved upward, striking C1 on the right side of their face.
Operator (O1) was tasked with assisting with repairs being made to a screen at the Washer Building in the Mill-Flotation area. As part of the assigned task O1 was installing a wooden wedge on the side of the 14 mesh screen. During installation, the wedge failed pinching O1's left thumb. O1 was transported to an offsite facility for evaluation.
EE, reported to Clinic on 01/30/2024. Employee-EE reports that after fueling a piece of heavy equipment on 01/30/2024 EE noticed pain in right shoulder and down to right elbow. EE was referred to Co. Care for eval. Restrictions were given by APRN, that could not be accommodated. EE is off work until further notice.
Employee EE, reported to Clinic on 01/16/24. EE reported right shoulder pain after climbing on and off crane on 01/15/24. EE was referred to Co. Care for eval. Restrictions were given by APRN, that could not be accommodated on EE is off work until further notice.
EE reported incident to Clinic on 09/19/23. EE did not report incident prior to reporting to Clinic. EE reports while walking a flag marker wire penetrated their lower left leg. EE developed a wound and felt ill with chills. EE was referred to Co. Care for eval, then was sent to LCMC for eval and is currently being treated there for Cellulitis. EE is off work until further notice.
On 9/11/2023 EE felt a pop in right forearm after throwing a dolly block in a man-basket in bucket truck. On 09/26 EE reported to clinic they had no pain just swelling and tightness in area. EE was referred to The Orthopedic Institute for eval. On 10/06 EE was seen by Ortho. On 10/09 we received the Ortho report EE has restrictions that cannot be accommodated and is now off work.
EE reported incident to Clinic on 09/06/23. EE was exiting dozer and the door swung shut striking EE in the mouth. Impact from door broke a portion of top right front tooth. EE was referred to dental for tooth evaluation and possible repair. EE was able to return to shift and does not have any lost time to report.
No adverse weather conditions present. Mind not on task.
EE was using motorized hose real on service truck. EE released button and real continued. EE grabbed hose to avoid being hit by hose. While grabbing hose it pulled on EE's left shoulder causing pain. EMT evaluated shoulder and determined no mobility in left arm with increase in pain with attempted movement. EE reports previous rotator cuff tear and surgical repair in same shoulder.
The technician was exiting the bed of service truck with a filter in one hand and sprained an ankle when their foot landed on the ground. Video reveals that they were not using 3 points of contact on descent and the area was muddy.
EE had pain in lower back after standing & twisting from bent over position while handing off hose to blow down parts on dragline. EE had worse pain sitting & much worse pain when standing from seated position & denies pain or numbness in lower extremities. EE was referred to Co Care for treatment.
Temp Employee (T1) was tasked with performing wash down duties in the Mill area. A sudden disconnect of the wash line caused T1 to lose their footing and slip/fall. T1 struck their head resulting in a laceration. T1 was transported to an offsite medical facility where they received sutures.
A contractor truck driver (O1) was fueling a haul truck at the Mine Services fuel island (Tank #6). O1 stated that while lifting the fuel nozzle up on the truck's catwalk above their head, O1 felt a sharp pain in their shoulder. O1 then contacted Supervisor. O1 was transported to local medical facility for evaluation.
EE experienced low back pain after spending several hours moving power cable in muddy ground conditions. EE reported to clinic and sent to an occupational medicine facility for evaluation and treatment. EE's restrictions cannot be accommodated. EE is out of work effective 2/15/23.
Haul truck operator (E1) used right knee to hold hose/nozzle in place while fueling haul truck. After completing fueling and resuming hauling duties, E1 began to feel pain and noticed swelling in right knee. E1 notified supervisor and was transported to local medical facility for evaluation.
EE reports right shoulder pain after an incident on 1/17. EE rapidly exited equipment during a noted ammonia event. EE did not report to work on 2/2/23 due to pain. EE was sent to company care on 2/3/23 and given restrictions which cannot be accommodated. EE is out of work effective 2/3/23.
EE was removing a front knuckle assembly from pickup truck. When pulling on front knuckle, EE lost grip causing left thumb to get caught between the knuckle & part of truck. EE was referred to company care and treated for pain in forearm, abrasion/pain in thumb. EE placed on modified duty.
A Mill Maintenance Technician was in the process of disassembling #1 & #2 ITV (Internal Transfer Valve) on the #1 Calciner. While performing the task dry calcine rock dust fell from overhead. The technician felt something in their left eye.
Employee was climbing on equipment & experienced a pulling sensation in left upper arm. EE reported to clinic, follow up visits with on-site Dr, EE continued to work with no restrictions. MRI on 12/15 confirmed distal bicep tear. Surgery scheduled for 1/4/23. LDW 12/21/22
EE was working with supervisor, EE bent over to lift the end of suction hose and experienced low back pain. EE reports having back pain prior to incident.
EE was attempting to move an approximately 3' x 1.5' x 1" thick piece of steel plate out of work area. They attempted to lift one side of the plate and slide it out of the way. As they attempted to slide the plate, they lost their grip and the plate fell on the metatarsal area of their left foot.
Building collapse is under investigation by MSHA.
Employee was in process of pulling instrument cable that was buried in the dirt when EE experienced a popping sensation in right shoulder. Employee developed increased pain in the right bicep. Employee has been placed out of work and will be evaluated by Orthopedic Institute.
Employee was working on #4 matrix line changing out section of pipe. EE felt aches & pain in lower back & left leg while working. Symptoms continued through the night. EE was sent to the onsite clinic and Company Care on 6/8/22. EE is off work due to restrictions which cannot be accommodated.
Mechanic (M1) and Contractor employee (C1) were tasked with replacing an expansion joint. M1 removed the last bolt, and pushed against the joint to dislodge and lower to the floor. The joint came out it snapped back pinching M1's wrist between the expansion joint and pipe flange.
While flying a welding machine down from an elevated platform, the load shifted. The employee's hand became pinched between the welding machine and an adjacent I-beam.
Employee reports was attempting to move a water cannon and experienced a sharp pain and pop in the right shoulder. Reports that the pain in the shoulder is a steady pain and has a buzzing sensation in the right hand. This is the same shoulder that was previously injured and may be a re-injury. EE was placed out of work 11/2/21, EE has a dr. appt. on 11/3/21.
WHILE WORKING IN MILL MAINT. SHOP EE WENT TO REMOVE A PIECE OF METAL SHAVING FROM THE VALVE AND IT CUT THROUGH EE'S GLOVE. EE SUSTAINED A CUT ON EE'S RIGHT HAND 4TH (RING) FINGER.
Employee was assisting in the loading of 18,000 lb gearbox onto a trailer bed when EE's thumb was pinched between the swinging equipment and the side of the trailer.
