EMPLOYEE WAS HANDLING STEEL, MOVING & SORTING, WHEN A PIECE CAUGHT HIS GLOVE ON A SHARP EDGE, CUTTING HIS SMALL FINGER ON HIS RIGHT HAND.
EE was attempting to pull an empty 5-gallon hydraulic can out of the frozen ground. As he was pulling on the handle of the can, one end of the handle broke loose from the can and cut his little finger (6 stitches required). EE was wearing a pair of insulated leather gloves at the time of the incident.
The employee bent down to pick up and move the miner cable. He felt a sharp pain in his lower back and couldn't move.
While installing a timber in the breaker row, individual felt pain in lower back. Individual went to Chiropractor and found to have a back strain. He is receiving treatment for this and has not been released to return to work.
EE was dropping a bolter cable from the mine roof, as he unhooked the cable and it fell to the ground the j-hook snagged the EE's hand causing a laceration on his right hand.
After EE ran shear for the shift, he felt a slight irritation in his eyes. The next morning when he woke up, his eyes were swollen shut. EE went to his eye doctor and was examined and given 2 presciptions.
While installing a water hose to the section water manifold, the manifold broke free from the water line and struck individual in the left shoulder and side of face. This chipped his bottom left wisdom tooth and caused a hair line fracture to jaw.
Employee was finishing the repairs on the continuous miner. He lifted the cover to recover tools and/or parts. The cover slipped out of his right hand and fell onto his left hand. The cover smashed his left thumb causing a laceration and fracture. Employee sustained a fracture of his left thumb and required 5 stitches.
EE was assisting the miner operator in clearing some cap rock off the head of the miner. As they pushed, the cap rock broke and EE's hand slid along the edge of the rock. The sharp edge cut through EE's gloves and cut his left thumb. EE was transported to the hospital where he received 5 stitches.
Employee was attempting to remove 7 roof screens from a bundle of screens that were leaning against the rib when the entire bundle tipped over striking ee in the left leg.
While pulling a wire mesh roof screen over his head and onto the ATRS of the roof bolter, EE felt pull in lower abdomen. Doctor visit on 7/12/05 diagnosed with hernia. Surgery secheduled for 7/19/05.
The Employee was adjusting the feed gate on a belt. The wheel handle gave all at once and pinched his fingers between the wheel handle and the brace/bracket.
EE WAS PULLING A WIRE ROPE ON TOP OF NEWLY INSTALLED BELT STRUCTURE. THE ROPE GOT HUNG UP ON A ROLLER AND AS EE YANKED ON THE ROPE, THE ROPE FREED ITSELF & EE's ARM SWUNG AROUND HIS BODY STRIKING A PIECE OF 2" PIPE.
While loading timbers into the bucket of a scoop felt pain and pulling in lower back. Determined to be an acute lumbar strain. Unable to work for 10 days.
EE was attempting to open a mandoor between the intake and the return when he felt a sharp pain in his right shoulder.
EMPLOYEE WAS ATTEMPTING TO UNPLUG A 1&1/2 WATER HOSE. WHEN THE OBSTRUCTION FREED ITSELF THE HOSE STARTED WHIPPING AROUND & STRUCK EE IN THE FACE.
EE WAS SHOVELING COAL ON A BELT. HE WAS IN A BOX CHECK AREA AND THREW A LUMP OF COAL ON THE BELT. IT HIT A CABLE AND BOUNCED BACK OUT AND LANDED ON EE'S FOOT.
While adjusting the surface loadout sensing magnet on the gates, individual pulled his hand away from magnet and struck a piece of stationary metal causing a cut to right index finger, which needed stitches.
Employee lifted a top belt roller when he felt a pull in his groin. Employee worked for an additional 5 days before seeking medical assistance.
THE EMPLOYEE LIFTED A SACK OF ROCK DUST AND HIS SHOULDER POPPED OUT OF IT'S SOCKET. HIS SHOULDER HAD BEEN PREVIOUSLY DISLOCATED.
THE EMPLOYEE ATTEMPTED TO TRAVEL A STEEP ROCK RAMP AND SLIPPED ON UNEVEN FOOTING AND REACHED AND GRABBED A SCREEN TO REGAIN HER BALANCE. HER SHOULDER WAS STRAINED.
Employee was pulling ventilation tube from scoop bucket using a nylon strap hooked to roof mesh. The strap gave striking him on the left palm causing a laceration.
The ee was unloading trash. He pulled on a bent roof screen and an unknown object hit him in the mouth.
Employee was pulling 12/4 cable and it caught on a lump of coal, as he pulled harder the cable came loose and his right knee gave. He was examined by x-ray and an orthopedist and no medical treatment was given. EE was later re-evaluated and a scope was performed on his right knee on 5/7/04.
WHILE INSTALLING AN "I" BEAM TUNNEL LINER SUPPORT, THE "I" BEAM ROLLED AND PINCHED HIS RIGHT RING FINGER CAUSING A SMALL LACERATION THAT NEEDED STITCHES.
THE EE WAS CHECKING A WATER PIPE SYSTEM FOR LEAKS AFTER IT HAD BEEN REPAIRED. IT CHANGED FITTING BROKE AND THE PIPE STRUCK HIM ON THE LEFT SIDE OF HEAD.
THE EE WAS CARRYING A JACK HAMMER OFF THE LONGWALL FACE. HE SLIPPED AND THE JACK HAMMER FELL ON HIS HAND AGAINST THE TOE OF A SHIELD AND SEVERED HIS LITTLE FINGER ABOVE THE SECOND JOINT.
THE INJURED EE WAS JOINING TWO LENGTHS OF WATER HOSE. A SMALL ROCK FELL FROM BETWEEN OPENINGS IN A WIRE MESH MAT AND STRUCK HIS RIGHT HAND CAUSING A CUT ON THE LITTLE FINGER ON THE RIGHT HAND. SUTURES WERE REQUIRED.
DURING THE PROCESS OF PULLING THE CONVEYOR BELT TOGETHER, AN UNIDENTIFIED OBJECT STRUCK EE IN THE SAFETY GLASSES AND CAUSED A SMALL LACERATION THAT REQUIRED SUTURES.
WHILE SERVICING THE CONT MINER, INDIVIDUAL LIFTED A NEW BUCKET OF MINER BITS ONTO THE TOP OF THE MINER AND FELT SOMETHING PULL IN HIS LEFT CHEST. DIAGNOSED WITH POSSIBLE DISLOCATED RIBS FROM S TERNUM. TOLD TO STAY OFF WORK FOR 2 WEEKS.
THE INJURED EMPLOYEE WAS A MECHANIC WHO WAS SENT TO REPAIR A FLETCHER ROOF BOLTING MACHINE IN USE IN THE EAST MAINS. AS HE WAS REMOVING THE HYDRAULIC FILTER HOUSING HIS HAND STRUCK THE FRAME O F THE MACHINE CAUSING A LACERATION OF THE RIGHT HAND. SUTURES WERE REQUIRED.
THE EE WAS PUSHING COAL WITH A CAT D9 BULLDOZER, WHEN THE FAN BELTS BROKE, HE PARKED THE DOZER AND ATTEMPTED TO RELIEVE THE PRESSURE ON THE RADIATOR, WHEN HE REMOVED THE CAP, HOT ENGINE COOLAN T SPRAYED ON HIS ARMS AND THE RIGHT SIDE OF HIS CHEST.
WHILE LOOKING FOR A LEAK IN A HYDRAULIC HOSE ON A ROOF BOLTER, INDIVIDUAL WAS HOLDING HOSE BY DAMAGED AREA. THE HOSE WAS PRESSURIZED FOR IDENTIFICATION AND A SMALL AMOUNT OF HYDRAULIC OIL WAS INJECTED INTO TIP OF RIGHT INDEX FINGER REQUIRING THE FINGER TIP TO BE LANCED TO ALLOW THE OIL TO DRAIN.
INDIVIDUAL SAT ON A RELAY BAR THAT WAS BEING SUSPENDED FROMT HE MONORAIL BY CHAINS. AS HE SAT ONT HE BAR THE CHAIN BROKE AND THE RELAY BAR FELL ON HIS LEFT LEG FRACTURING THE LOWER PART OF HIS LEG. THIS BAR WAS ON THE BELT LINE OUTBY THE TAILPIECE.
INJURED EE WAS SETTING A TIMER. THE CAP PIECE FELL OFF TIMBER AND STRUCK HIM IN MOUTH, CAUSING A LACERATION OF THE UPPER LIP. THE LACERATION REQUIRED SUTURES.
THE INJURED EE WAS INSTALLING A TRANSMISSION FROM AN ISUZU P.U. TRUCK LOCATED IN THE SURFACE SHOP. AS HE WAS LIFTING THE TRANSMISSION IN PLACE, IT SLIPPED AND FELL AND PINCHED HIS RT. HAND BET WEEN THE TRANSMISSION AND THE CONCRETE FLOOR. THE LITTLE FINGER OF THE RT. HAND RECEIVED A COMPOUND FRACTURE AND DISLOCATION. AS A RESULT OF THEINJURY FIRST JOINT OF LITTLE FINGER AMPUTATED.
WHILE HELPING REPLACE THE HAMMER TIPS IN SURFACE CRUSHER, THE TIP OF THE INDIVIDUAL'S FIFTH LEFT FINGER WAS PINCH BETWEEN CRUSHER ROLLER AND HAMMER TIP HOLD DOWN ROD WHILE IT WAS BEING INSERTE D. THIS CAUSED A LACERATION AND SOME TISSUE LOSS TO THE TIP OF THE FINGER REQUIRING STITCHES AND DEBREDMENT ON END OF FINGER.
EE WAS COMPLETING REPAIRS ON THE CONTINUOUS MINER. HE HAD A COVER THAT WAS BENT & DID NOT FIT PROPERLY. HE ATTEMPTED TO STRAIGHTEN THE COVER WITH THE HEAD OF THE MINER & PINCHED HIS LITTLE FIN GER ON HIS LEFT HAND.
EE WAS TAKING A BAG W/ROCK DUST OFF OF A BATTERYSCOOP AND TWISTED WITH ROCK DUST CAUSING THE MUSCLE ON THE RT. SIDE OF SPINE AT BASE OF SHOULDERBLADE TO CRAMP UP AND HAVE A SPASM. HAS MISSED WORK TO ALLOW SPASM TO SUBSIDE.
EE'S BELLY DUMP TRUCK WAS STUCK WHILE TRYING TO UNLOAD A COAL PILE. AFTER THE FRONT END LOADER PULLED HER OFF THE COAL PILE SHE WAS PUTTING THETOW CABLE BACK ON THE LOADER WHEN SHE INJURED THE TENDONS IN HER RIGHT WRIST.
WHILE LIFTING A ROOF BOLTER DRILL BOX INTO PICKUP TRUCK. FELT SOMETHING PULL IN RIGHT SHOULDER. ROTATOR CUFF TEAR.
WHILE OPENING AN EQUIPMENT AIR LOCK DOOR, THE DOOR SLIPPED FROM INDIVIDUALS GRIP, CLOSING ON OTHER HAND. THIS CAUSED A LACERATION ON THE BACK OF LT HAND REQUIRING STITCHES.
EMPLOYEE WAS LOADING ROOF MATS ON A FLETCHER ROOF BOLTING MACHINE THE CORNER OF THE MAT PINCHED HIS LEFT HAND BETWEEN ANOTHER MAT.
ON 10/6/01 INDIVIDUAL FELT PAIN IN RT. SHOULDER WHILE LIFTING A 5 GAL CAN OF OIL. HE HAS NOW SEEN A DOCTOR FOR THE PAIN AND RECEIVED SURGERY FOR TORN ROTATOR CUFF. COMPANY WAS NOTIFIED OF SCHE DULED SURGERY ON 5/14/02 FOLLOWING DOCTORS APPOINTMENT AND MRI. THIS IS NOW A LOST TIME AND REPORTABLE INJURY. INDIVIDUAL RECEIVED NO TREATMENT UNTIL THIS TIME. SURGERY 5/20/02.
INJURED WAS OPENING A MAN DOOR. HIS RIGHT HAND WAS PINCHED BETWEEN THE DOOR AND THE DOOR FRAME.
EE FELT DISCOMFORT IN HIS SHOULDER DURING THE SHIFT. THERE WAS NO SPECIFIC TIME. HE SAW A SPECIALIST IN JAN 2002 WHO RECOMMENDED AN ORTHOSCOPIC PROCEDURE TO REPAIR WHAT HE DIAGNOSED AS A ROTAT OR CUFF INJURY. THE PROCEDURE REVEALED AN OLD FRACTURE AND CHIPS OF BONE THAT WERE CAUSING THE PAIN. THE PROCEDURE BECAME MORE COMPLICATED RESULTING IN LOST WORK DAYS STARTING 3/6/02.
EE WAS REMOVING A WORN BIT FROM A BIT BLOCK ON THE CUTTER DRUM ON THE SHEARER. HE WAS USING A CHISEL TO FREE THE BIT. WHEN IT CAME FREE THE CASE SLIPPED AND HIS PINKY FINGER AND RING FINGER CA ME INTO CONTACT WITH A SHARP EDGE ON THE BIT BLOCK.
