EE was changing heater hose on a loader while doing so he hit his hand on a hose clamp and cut his hand. The EE received 2 stitches on the top of his hand. This constituting as an MSHA reportable accident.
EE was operating the box erector putting a empty box in machine. While doing so the EE felt a sharp pain in lower back region. The doctor put him on restrictions of not lifting over 20 pounds until Jan 28th, thus constituting as a MSHA reportable accident.
EE was changing clutch on a drill truck while doing so the ring gear slipped off the crank shaft and he cut his finger. He received five stitches on his right index finger thus constituting in an MSHA reportable accident.
EMPLOYEE WAS WALKING UP A RAMP OUTSIDE THE MAINTENANCE SHOP CARRYING MATERIAL TO A JOB SITE AND FELT LEFT KNEE POP AND GET VERY WARM. KNEE CONTINUED TO POP IN AND OUT THE REST OF THE DAY. EMPLOYEE DECIDED TO SEE A DOCTOR ON 6-12-12. EMPLOYEE CONTINUED TO WORK FULL TIME THRU 6-27-12. HE WENT IN FOR KNEE SURGERY ON 6-28-12.

A lab tech. was taking a sample out of a pipe that was loading a railcar. The force of the flowing material took the bag from his hand. It startled him and he jerked his hand back hitting his index finger of his right hand on metal. This caused a laceration on his finger requiring five stitches to close.

EE was putting hook of car puller onto rail car when his index finger was cut, stuck in between hook and car. Took EE to doctor and doctor removed nail, put 2 stitches in and prescribed antibiotics, thus constituing MSHA reportablity.
EE was throwing 100 pound bags. Felt pain in wrist at the end of shift. Went to doctor on 11/30 and was told EE had tendonitis. Restrictions of not lifting anything over 10 pounds was put on EE resulting in MSHA reportablity.

Employee was shoveling at tail end of a conveyor. As he was turning to dump the scoop in the wheelbarrow he strained his back. A doctor prescribed him muscle relaxers constituting MSHA reportability.
EE was stretch wrapping a pallet of 50# bags of bentonite, when he stepped beween the forklift stop and the moving stretch wrapper. The pallet that the bags were on, pinned his left leg between the forklift stop and the pallet of bags, causing bruising of the leg and light duty work.

The EE had to walk sideways through a 23 inch walkway to go to the maintenance shop. The employee was side-stepping to the right. As the EE was free to sqare up his body, the next step is where the EE heard and felt the pop in his right foot. The EE fractured his right 5th metatarsal. A splint and restricted work was the outcome, thus resulting in the MSHA reportable.

EE was setting up a 6" water pump to pump water from pit. EE was in process of setting the suction hose in the pit. When he picked up the hose and turned to place in water, he twisted his back. He saw a chiropractor on 7/6/07 and missed work 7/7. He worked full shifts 7/5 & 7/6 and sought chiro. after work. He was not scheduled 7/8 and returned 7/9 w/o restrictions.
