Fall onto or against objects

An Operator was on top of a grate cleaning a crude bin when EE's foot was caught in the grate. EE's knee gave out and EE fell onto the grate spraining their knee and shoulder.

EE was cleaning screens and one of the screens fell off the box it was on and the EE reached out to catch it. EE felt a pop in the right bicep.

Struck by flying object

Employee unloading a tire from pick up and tire bounced and hit employee's leg in the knee area. Two weeks later the employee started to have swelling in the ankle and was looked at again and it found that employee had an abscess in the lower leg and had to have minor surgery to remove infected area.

Employee was doing some live work and placed employee's hand in a hangar bearing and the finger got pinched. Employee received 10 stitches.

Miner aggravated a bilateral hernia condition on their left side when lifting 50lbs lignite bags and stacking them on a pallet.

Employee was lighting coal furnace with diesel and employee was burned

Employee was manually handling 50 Lb bags all day and finished shift at 3:00 PM. At midnight EE woke up with back pain and went to the ER where the Dr prescribed EE medication and gave EE lifting restrictions.

A maintenance operator was changing a gearbox on a piece of equipment. A associated part was being pry bared out of the way when the pry bar slipped and the part smashed his finger.

Fall from machine

Employee was exiting water truck and slipped off of step. Employee landed on one foot and fractured leg.

EE was retrieving parts for maintenance repair and was traveling down steps when he twisted his knee. We took EE to the hospital where doctors found no injury and released EE for restricted duty and scheduled a follow up appointment in 1 week.

EE was pushing wheel barrow full of material. The wheel fell off the wheel barrow and EE tried to steady the wheel barrow to prevent material from falling out and felt pain in back.

Struck against a moving object

The operator pulled into the mine and noticed that there was some spillage that needed to be cleaned up. He then parked the haul wagon, got into the loader and proceeded to clean the mess up. While cleaning the mess he noticed that he was getting closer to the haul wagon when he ran into it cause some damage to the cat walk and hand rail on the haul wagon and nothing on the loader.

Worker was working on 20% slope metal roof patching small vent holes, the worker was not tied off and was moving material and tools into the work area from access point. In Transit a 30"" section of roofing gave way and worker fell through the roof, 14 feet into the building, striking some conduit on the way down, causing a laceration to his left knee.

While the operator was doing his hourly inspection he noticed some smoldering material on a ledge. When he was preparing to clean the material up around an exit light the light shorted and caused a small flame to come out and burn his face. The operator suffered 2nd and 3rd degree burns to his face and minor burns on his neck. He was kept in the hospital for observation.

Accident type, without injuries

Contractors on site to remove unused equipment, were cutting iron with torches to remove entrance to building. Contractors inspected area an hour after cutting for embers, saw none and left the area at roughly 5 pm. Our employees noticed fire at 10:45 pm and called the VFD and me. VFD arrived at roughly 11 pm and contained fire to Old MCC room and extinguished.

Forklift operator lifted a pallet from stretch wrap machine and rolled forward jamming the pallet into employee's leg and twisting his ankle.

Walking to front end loader on snow packed area after a rain when he slipped on the ice and fell.

Annual hearing tests on 9/14/06 showed EE had STS Shift of 10 or more but <25. Retested on 11/8/06 & received paperwork 11/29/06 indicating he now had STS >25. He's indicated hearing loss runs in family so I've scheduled evaluation by Audiologist 12/13/06 to determine if shift is work related. Amendment: Audiologist & Dr. determined hearing loss is result of blockage in ear canal.

Was exiting the 980 loader when he felt his back start to tighten up and then had intense pain where he had previously injured it at home on 8/9/06. As he climbed down off the ladder his foot then slipped off the ladder.
