Miner was lifting a roll of shrink wrap to replace an empty roll and felt small popping sensation in right arm.

As the employee was finishing barring out a crude hopper, they went to put the bar away and felt a pop in right shoulder, and pain afterwards.

Was performing end of shift housekeeping duties. Pinched thumb in between cleaning rake and metal frame of conveyor system.

Employee was repairing a broken airline on top of tank #6. Employee was cutting new airline to fit in the location. Knife employee was using went through the airline and contacted the employee's left thumb with the open blade.

Standard threshold shift to left ear

Employee cut left middle fingertip when attempting to cut an airline hose. The injury required 5 sutures. There were no restrictions.

An employee was sitting in the packing chair of the #4 Palletizer system when they stopped, twisted, and looked over their left shoulder to check how many pallets were left on the lot and a lower back strain occurred.

Employee cut index finger on sharp edge of a guard while trying to open the guard to check the drive belts.

Employee was stung on right wrist while walking to time clock.

Employee was climbing a fixed ladder and foot slipped off the fourth ground causing ee to fall 10ft.

Employee pinched finger between tool and railroad tracks

Employee bends over to loosen some bolts on a mill and felt a strain in his mid to lower back.

Employee was taking a bearing off on #1 mill in the tower when bearing fell off the shaft and contact employee arm giving him a laceration on the right arm.

Employee was clearing jam up top on top belt of #1 packer and forgot to shut the stop switch off up top and when bag was cleared belt turned back on while he was on it.

Employee was going to check on a plugged auger on the mill. While walking by the auger the bolts on the motor mount broke and struck his right leg.

Employee left hand contacted a piece of metal resulting in a laceration that had to have 3 stitches.

Sprained wrist resulting in restricted duty

EMPLOYEE WAS WALKING UP A RAMP OUTSIDE THE MAINTENANCE SHOP CARRYING MATERIAL TO A JOB SITE AND FELT LEFT KNEE POP AND GET VERY WARM. KNEE CONTINUED TO POP IN AND OUT THE REST OF THE DAY. EMPLOYEE DECIDED TO SEE A DOCTOR ON 6-12-12. EMPLOYEE CONTINUED TO WORK FULL TIME THRU 6-27-12. HE WENT IN FOR KNEE SURGERY ON 6-28-12.

The employee struck his calf of his right leg on a piece of metal, causing a blood vessel to burst in his calf muscle. This required two incisions to relieve the pressure to his lower leg.

A lab tech. was taking a sample out of a pipe that was loading a railcar. The force of the flowing material took the bag from his hand. It startled him and he jerked his hand back hitting his index finger of his right hand on metal. This caused a laceration on his finger requiring five stitches to close.

Employee was climbing a ladder to an elevated platform, he missed a step and hit his right knee. This resulted in four stitches.

Hand contacting auger.

Employee had his hand on a chain to keep tension on it, the chain pulled back and pinned his hand between the chain and an elevator. This resulted in stitches and a broken pinky finger.

Employee was lifting a rail car door with a forklift. He got off the forklift and pushed on the rail car door. The fork came loose and hit his chin, cutting his chin and inner lip requiring seven stiches.

Employee was filling bags with bentonite and got his ring finger pinched between the fill spout and the rubber bag clamp. When he pulled his finger back, it tore the skin. It required three stitches.

Employee was removing an old hooder above a box car and realized he was tied off to the hooder when it broke free knocking him off the top and sending him to the ground. He was able to disconect before he hit the ground. He sustained damage to his pelvis, right shoulder and left knee. No surgery or bracing of any kind was needed.

Gave OK to start up piece of equipment, then placed arm in to get chunks out and feeder grabbed his glove and cut off small part of index finger.

Forklift backed and hit employee while trying to ask driver question

Closing doors on manual grizzly that kept opening and a rock wiggled loose and fell and landed on his right foot.

Greasing the loader and when done was climbing back in to loader up the stairs. Grease was on the sole of my boots and I slipped on the top step of loader and was holding the handrail when I fell. Felt something pull in shoulder.

Closing car door, when handle got a little hard to turn, so he turned it a little harder and the handle went free and his hand jerked and caught a peice of metal that was sticking out on the door.

while closing the box car door worker slipped on some mud and lost hold of cheater bar. Cheater bar came back and hit employee in the elbow causing a laceration that required 3 stitches

Mega tex was running. Shut if off and boot of mega tex slipped into pipe, mega tex was winding down, reached down to grab sck in pipe and mega tex pinned left arm.

Holding a pin on bobcat bucket, bucket raised too quickly and arm caught.

Changing bushings on a millroll and adjusting a pinion shaft. Thurned the top of the roll to get a better angle, then the roll fell and smashed finger between the roll and spider gear.

55 gallon of oil came off the dolly when getting it off the truck smashing left ring finger.

After rainstorm he stepped out of loader to wash windsheild and slipped off of the fender and fell to the ground.

Employee got caught in the guard rail at hooder. He was pinned between the hooder arm and the guard rail with his head facing toward the palled that was in the hooder. The stainless steel can was directly below him and the chain still wet when other person came.

Removing bulk loading hood from top of bulk car, hood caught on the when it came down he tripped on the track trying to get away.

burning pallets day before and wind created spark starting fire.

Shoveling wet bentonite with a scoop shovel and as he lifted something popped in his back, like a pulled muscle.

Stepping out of the bobcat and hit in the nose with the handle of a shovel.

Had a rock stuck in the hopper in chips causing the belt to jam, took a big pipe to break out the rock while hitting the rock the pipe slammed back into the palm of my hand causing swellin and pain.

Restacking pallets, walked over to get a 50 lb bag and carried it to pallet and stepped wrong hurting lower back

Twisted right ankle while crossing tracks in wet conditions.

Holding a new roll on the reroll line as a roll was being taken off by a stinger, as the stinger was setting down the roll it bounced a bit and came too close to her, noticed it was coming down at her at the wrong angle and was only able to get her right hand out of the way

Carrying harness across R/R cars over his arm. Started to get down, harness caught on knuckles. Lost balance and caught himself with hands and twisted ankle.

unplugging pipe on the old granular system above the screens. ON my way down the ladder I dropped a hammer on top of an auger and tried to grab it out but thumb got caught in between the auger and the auger cash.

EE was throwing bags and other worker was packing. Pallet was full and co-worker was told to stop but did not and injured got hit in the left knee with the bag.

Repetitive action throwing bags caused the pain, employee received a cortisone shot on this date.

Climbing off dozer w/fuel nozzle in right hand grabbed handle w/left hand to lower self to ground. Got arm stuck in handle when stepped to ground.

Opening doors on bulk rail cars to be loaded

Fell in dark, without safety equipment.

Switching an air valve to the closed position, he had to remove a rock from the valve before switching the air line. As soon as the rock was removed the valve closed on his hand.

Handling specialties bags for mixing. Bending & twisting to mix grout in specialties room & strained lower back.

LEFT FOOT WAS PINNED BETWEEN THE FORKLIFTS COUNTER WEIGHT AND GROUND. FORKLIFT ROLLED BACK ON THE FOOT AND FOLDED IT.

THROWING 50# BAGS ONTO PALLET, BENT DOWN TO DROP THE BAG. VERTEBRA IN LOWER BACK BECAME DISPLACED AND PINCHED THE NERVE.

TRUCK DRIVER PICKING UP LOAD ON SITE. (NOT MINE EMPLOYEE - EMPLOYED BY CONTRACTOR.) DRIVER WAS TARPING THE LOAD AND FELL OFF ONTO THE GROUND. DRIVER WAS NOT USING FALL PROTECTION HARNESSES WHICH ARE PROVIDED.

Hit on nose by broken high pressure pipe sustained 3.0 cm laceration and concussion.

Got hand and forearm caught between roll and rewind roll when rubbing finger tips on rewind roll. Roll took hand and forearm in roll.

EE WAS ON TOP OF THE INCLINE CONVEYOR TO UNPLUG A BAG JAM ON THE CONVEYOR. WHEN HE REMOVED THE BAG FROM THE PHOTO EYE, THE CONVEYOR STARTED PULLING HIS LEG IN THE HEAD WHEEL AND BREAKING HIS ANKLE.

TRYING TO SHUT VALVE - NOT ENOUGH ROOM.

EMPLOYEE WAS POUNDING THE CORE TUBE BACK INTO PLACE WITH A RETAINER THAT HOLDS THE BENTOMAT FIBER ROLL IN PLACE AND HIS HAND WAS BETWEEN THE RETAINER AND THE CORE TUBE. HE SMASHED HIS FINGER.

MOVING NEEDLE RACK BACK TO NEEDLE AREA.

TABLET MACHINE WAS STOPPED, UNPLUGGED, LOCKED OUT POWER. HE TOOK THE BALANCE WHEEL COVER OFF AND WAS TURNING IT BY HAND. HE HIT A SHARP OBJECT, CUTTING HIS RIGHT INDEX FINGER.

PICKING UP 100 LB BAG & FELT SHARP SHOOTING PAIN IN LOWER BACK.

LOOSENING CLAMPS ON LINCH CONDUIT AND FELL OFF OF THE LADDER. CLAMPS CAME LOOSE AND HIT LADDER MAKING HIM FALL.

FIXING BAGS ON EAST END OF PALLETIZER AND LOST BALANCE AND FELL ON GUARD ON RIGHT SIDE OF BACK.

LIFTING PIPE INTO BANDSAW. PULLED MUSCLE IN LEFT SIDE OF BACK.

DUMPING HOPPER IN THE PIT TRUCK THE HOPPER WAS IN THE AIR AND TIPPED ONTO ITS SIDE ON THE GROUND.

COMING OUT OF WAREHOUSE, ROAD WAS SLIPPERY, DID NOT STOP AT DOORWAY - SLID INTO TRUCK WHEN TRIED TO STOP.

DROPPED A BAR ON FINGER.

CAUGHT RIGHT FOOT ON A METAL STEP GOING DOWN AND TWISTED AND FELL AND HURT FOOT.

FINISHED SPOTTING A RAILCAR AND STEPPED OFF THE CAR TO SEE IF IT WAS WHERE IT SHOULD BE AND STEPPED ON A CAR JACK LAYING ON THE GROUND.

LITTLE MOBILITY IN LEFT WRIST BECAUSE OF PAIN. RIGHT WRIST TINGLES ALL THE TIME. THROWING BAGS HE BELIEVES IS THE CAUSE.

EE WAS PICKING UP A SHEET OF METAL OFF THE TWO SAW HORSES WHEN ANOTHER PIECE OF METAL ROLLED OFF ONTO HIS FOOT-LEFT

EMPLOYEE WAS WALKING DOWN STAIRWAY, TRIPPED ON STAIR & FELL TO CEMENT FLOOR.

EE WAS LIFTING A PARTIAL ROLL OF FIBER TO A LOWER LEVEL AND FELT A SHARP PAIN IN GROIN.

DRILLING HOLES WITH DRILL PRESS AND WIPED FILINGS OFF BIT WITHOUT TURNING POWER OFF. BIT CAUGHT GLOVE AND PULLED FINGER INTO BIT. RIGHT INDEX FINGER WAS LACERATED FROM TIP TO SECOND JOINT.

FIBER ROLL WAS BEING LOADED, EMPLOYEE DID NOT MOVE OUT OF THE WAY & WAS BUMPED BY THE ROLL.

EMPLOYEE WAS GRINDING ON METAL AND DID NOT NOTICE UNTIL THAT EVENING THAT HE HAD METAL IN LEFT EYE.

EE WAS CLEANING OUT A TABLET MAHCINE AND TRIED TO PULL A PAPERTOWEL OUT THAT WAS IN THE MACHINE AND THE MACHINE INJURED HIS LEFT POINTING FINGER.

EMPLOYEE HAD JUST CUT THE TOP ON A ROLL OF PRODUCT AND WAS MOVING THE ROLL WHEN IT SLIPPED OUT OF HIS HAND. HAND HOLDING THE KNIFE CAME BACK AND CUT HIS FACE.

WHILE EMPLOYEE WAS CHECKING TO SEE IF CHEMICALS WERE REACHING CLAY, HE REACHED INTO MOVING AUGER & CAUGHT MIDDLE FINGER OF LEFT HAND, PARTIALLY SEVERING FINGER TIP.

EE WAS HELPING MOVE A 4' X 4' X 21"" METAL PLATE. HE LOST HIS GRIP & THE PLATE SMASHED HIS LEFT PINKY FINGER.

EE SLIPPED WHEN MOVING SEAL GLAND ON MILL BASE. WHEN HE FELL, HIS RT. ARM BECAME WEDGED BETWEEN THE MILL BASE & A TRASH HOPPER WHICH RESULTED INTHE DISLOCATION OF THE RT. SHOULDER.

EMPLOYEE WAS PICKING UP A BAG OFF A BELT WHEN HE STRUCK AND SLICED HIS RIGHT THUMB.

EE WAS GETTING A PRODUCT SAMPLE FOR THE LAB. A BROOM HANDLE WAS ON TOP OF CATWALK, HE TURNED AROUND & TRIPPED OVER THE BROOM HANDLE CAUSING HIM TO HIT HIS KNEE ON THE CATWALK.

EE WAS OPENING A PACKAGE AND CUT HAND WITH SCISSORS.

TRUCK DRIVER WAS WALKING INTO WAREHOUSE, STEPPEDON SOME SLICK MATERIAL & FELL DOWN.

ANOTHER EMPLOYEE WAS CUTTING PRODUCT WRAP WHEN VICTIM PUT HIS HAND ON THE PRODUCT ROLL THAT WASBEING CUT. HIS MIDDLE FINGER WAS CUT.

ROLL OF PRODUCT WAS STUCK, EE TRIED TO GET IT MOVING AGAIN WHEN ANOTHER EE REVERSED THE MACVHINE CAUSING EE'S ARM TO BE ROLLED UNDER THE ROLL.

EE WAS MOVING DOORS ON A RAILCAR. HE WAS HOLDING THE RELEASE LEVER WHILE ANOTHER EE WAS PULLING THE DOOR W/ A FORKLIFT. AS HE PRESSED ON THE GAS, THE DOOR SWUNG UP AND LANDED ON THE LEFT HEEL OF THE INJURED EMPLOYEE.

EMPLOYEE WAS PUTTING PRODUCT INTO #3 MILL. THEREARE 3 STEPS TO WALK UP TO FILL THE MILL. HE DUMPED A BAG OF PRODUCT AND WAS STEPPING DOWN WHEN HIS SHOE CAUGHT ON ONE OF THE STEPS AND HE FELL TO THE CATWALK. HIS SHOE APPARENTLY CAME LOOSE FROM BEING HUNG UP CAUSING HIM TO BREAK HIS ANKLE.

A 6"" IDLER ROLL FELL ON TOP OF LEFT FOOT.

EQUIPMENT WAS NOT WORKING CORRECTLY. ROLLING DEVICE FOR CLAYMAX LINE WAS STUCK. EE STOOD ON THE ROLL OF PRODUCT TRYING TO HELP IT ROLL WHEN HIS FOOT WAS CAUGHT BETWEEN THE ROLLER AND THE ROLL OF PRODUCT THE LEFT LEG WAS ROLLED UP JUST UNDERTHE CALF.
