EMPLOYEE WAS WALKING UP A RAMP OUTSIDE THE MAINTENANCE SHOP CARRYING MATERIAL TO A JOB SITE AND FELT LEFT KNEE POP AND GET VERY WARM. KNEE CONTINUED TO POP IN AND OUT THE REST OF THE DAY. EMPLOYEE DECIDED TO SEE A DOCTOR ON 6-12-12. EMPLOYEE CONTINUED TO WORK FULL TIME THRU 6-27-12. HE WENT IN FOR KNEE SURGERY ON 6-28-12.
The employee struck his calf of his right leg on a piece of metal, causing a blood vessel to burst in his calf muscle. This required two incisions to relieve the pressure to his lower leg.
A lab tech. was taking a sample out of a pipe that was loading a railcar. The force of the flowing material took the bag from his hand. It startled him and he jerked his hand back hitting his index finger of his right hand on metal. This caused a laceration on his finger requiring five stitches to close.

Employee was lifting a rail car door with a forklift. He got off the forklift and pushed on the rail car door. The fork came loose and hit his chin, cutting his chin and inner lip requiring seven stiches.
Employee was filling bags with bentonite and got his ring finger pinched between the fill spout and the rubber bag clamp. When he pulled his finger back, it tore the skin. It required three stitches.

Employee was removing an old hooder above a box car and realized he was tied off to the hooder when it broke free knocking him off the top and sending him to the ground. He was able to disconect before he hit the ground. He sustained damage to his pelvis, right shoulder and left knee. No surgery or bracing of any kind was needed.
Gave OK to start up piece of equipment, then placed arm in to get chunks out and feeder grabbed his glove and cut off small part of index finger.

Forklift backed and hit employee while trying to ask driver question
Closing doors on manual grizzly that kept opening and a rock wiggled loose and fell and landed on his right foot.
Greasing the loader and when done was climbing back in to loader up the stairs. Grease was on the sole of my boots and I slipped on the top step of loader and was holding the handrail when I fell. Felt something pull in shoulder.
Closing car door, when handle got a little hard to turn, so he turned it a little harder and the handle went free and his hand jerked and caught a peice of metal that was sticking out on the door.
while closing the box car door worker slipped on some mud and lost hold of cheater bar. Cheater bar came back and hit employee in the elbow causing a laceration that required 3 stitches

Mega tex was running. Shut if off and boot of mega tex slipped into pipe, mega tex was winding down, reached down to grab sck in pipe and mega tex pinned left arm.
Holding a pin on bobcat bucket, bucket raised too quickly and arm caught.
Changing bushings on a millroll and adjusting a pinion shaft. Thurned the top of the roll to get a better angle, then the roll fell and smashed finger between the roll and spider gear.
55 gallon of oil came off the dolly when getting it off the truck smashing left ring finger.

EE WAS HELPING MOVE A 4' X 4' X 21"" METAL PLATE. HE LOST HIS GRIP & THE PLATE SMASHED HIS LEFT PINKY FINGER.
EE SLIPPED WHEN MOVING SEAL GLAND ON MILL BASE. WHEN HE FELL, HIS RT. ARM BECAME WEDGED BETWEEN THE MILL BASE & A TRASH HOPPER WHICH RESULTED INTHE DISLOCATION OF THE RT. SHOULDER.

EE WAS MOVING DOORS ON A RAILCAR. HE WAS HOLDING THE RELEASE LEVER WHILE ANOTHER EE WAS PULLING THE DOOR W/ A FORKLIFT. AS HE PRESSED ON THE GAS, THE DOOR SWUNG UP AND LANDED ON THE LEFT HEEL OF THE INJURED EMPLOYEE.
EMPLOYEE WAS PUTTING PRODUCT INTO #3 MILL. THEREARE 3 STEPS TO WALK UP TO FILL THE MILL. HE DUMPED A BAG OF PRODUCT AND WAS STEPPING DOWN WHEN HIS SHOE CAUGHT ON ONE OF THE STEPS AND HE FELL TO THE CATWALK. HIS SHOE APPARENTLY CAME LOOSE FROM BEING HUNG UP CAUSING HIM TO BREAK HIS ANKLE.
EQUIPMENT WAS NOT WORKING CORRECTLY. ROLLING DEVICE FOR CLAYMAX LINE WAS STUCK. EE STOOD ON THE ROLL OF PRODUCT TRYING TO HELP IT ROLL WHEN HIS FOOT WAS CAUGHT BETWEEN THE ROLLER AND THE ROLL OF PRODUCT THE LEFT LEG WAS ROLLED UP JUST UNDERTHE CALF.
