When removing hydraulic filter, strained shoulder.
Employee was lifting a roll of skirting and felt pain in lower abdomen. EMPLOYEE WAS ON OVERTIME (15 HOURS)
While trying to move a starter sheet, employee felt pain in lower back.
Employee was in the process of removing an impeller from a pump in the shop when he suffered a laceration to his left index finger and a small cut to his right index finger. Employee was taken into the ER room and given a tetnus shot and anit-biotics. He was refered to a specialist and scheduled to have surgery to repair the tendon on his left index finger (3/9/07) On Going
Employee was removing conduit in the central tank house and some airborne sulfate dust got into his eyes. Employee was taken to local clinic and was given a prescription for his eyes and released to full duty.
Employee noticed that a valve on cell 20 was not close all the way. He went back to re close the inlet valve on cell G 20 when he splashed electrolyte that was on the floor and electrolyte got on his clothing and saturated them and electrolyte got on his right calf on his right leg.
Employee was putting grating back in place in the mill by the feed trunion air jack when the grating slipped. His left ring finger was lacerated.
Employee was in the process of retrieving a rope, that had been thrown over the bank, for cable removal. He came up from the bottom of the bank and when he pulled on the rope rocks came down from the bank and hit him. The employee lost his balance and fell causing injury to his shoulder. He was taken to the clinic.
Employee was working under the pan feeder of Crusher 2 clearing rock off a concrete ledge so a skid steer loader could scrape the material out of the pocket. The skid steer scraper attachment made contact with her left ankle causing a laceration.
Employee was pulling on rubber belting to straighten it when he felt a pop in his groin.
The employee was attempting to clear a ball jam in the chute for the #4 mill. He put his hand in to the chute to move a loose ball and the following balls rolled forward trapping his hand in the chute. While trying to dislodge his hand with his other arm, he scrapped his forearm.
EE was installing conductors into conduit. He was pulling in a downward motion from above his head to below his waist pushing conductors into the conduit. This motion exerts force int he elbows and shoulders. The strain/sprain is in both elbows where the forearm and tricep ligaments attach. (EE was placed on 4 days restricted duty: he had personal days scheduled prior to the
Employee states over a couple of months he has experienced pain in shoulder and arm as a result of lifting.
Employee was removing used ripper points from the back of his service truck. After tossing the next to last ripper point and reaching for the last ripper point, the employee felt something impact his left tricep. He did not see anything and continued to reach for the last ripper point when he noticed blood trickling down his arm.
Employee was moving transformers down a set of stairs assisting other employees. Transformer was manuvered sideways catching employees pinky in a pinch point causing a fracture. No days away. No restrictions.
The employee was closing the front loading door on 320275 transport truck. He used his left hand to grab the door and when the door came down it struck the pipe wrench that was in his right hand pinching his right middle finger between the wrench and the railing. The incident has broken his finger.
Employee states while loading casing onto pipe truck with fellow employee, casing rolled and lacerated his right middle finger.
While spraying at the A1A splitter, the employee was struck by muck and water in the face when co-worker sprayed toward him. Employee was taken to clinic and treated.
Employee states while bending over work product and laying pipe he experienced soreness in back.
Climing down ladder felt pain in stomach, had strain in stomach area may be hernia.
While dismounting the 511 HT for a bathroom break, the operator appears to have pulled/stretched his groin area.
Employee was connecting iron frame work on a structure when he felt a pain in the groin area. He reported it on 1/12/07.
Employee was placing crosstie under idler bracket. Employee's hand was over the end of the tie when the tie was moved on the opposite end causing his hand to get caught in a pinch point sustaining a fractured thumb. No days away. No restrictions.
Employee was working underneath a pickup and foreign matter dropped into right eye. Went to doctor on 1/9/08, however did not receive doctor's notes indicating prescription medication was given to him until 1/22/08.
Area was muddy, employee was assisting to remove grating and slipped then felt pain in his lower back. Transported to clinic for evaluation.
Employee received a laceration and fracture of rt. index finger on metal banding while closing drill cab door.
Employee was attempting to move rocks that had rolled under the haul truck due to spillage while loading. He experienced pain in his right knee.
Employee was working in the 14 to S10 chute and was climbing down the ladder. Employee stepped on co-worker's foot and felt a pop on the left foot.
Employee was moving cribbing during truck disassembly at dump site. Cribbing needed to be moved numerous times in order to make crane pick from dump area to tranport truck. Cribbing pieces weigh in excess of 100 lbs per piece. Approximately 50 pieces of cribbing needed to be moved during this process.
While holding door jam, wind blew door closed, hitting employee's fingers on right hand.
Went to raise bore storage area to return shackles and slings. Picked up two shackles in one hand and walked to the job box. Lifted the lid of the job box with his left hand turned slighted to toss the shackles into the job box and felt a pain in his left shoulder.
While pulling cable, employee felt pain in lower right leg.
Two employees were removing a metal liner from the 1 crusher discharge conveyor. While bringing the liner down to the catwalk one of the employees lost his grip which caused the second employee to be struck on top of the right foot.
While loading copper cathode onto 4 stripping machine receiving conveyor, employee had one copper cathode fall from cathode strongback and hit thumb on left hand causing an avulsion to left thumb.
While installing new launders beween the 3 & 4 flotation rows, a hydraulic ram and spacer were being used to expand sections into place. The spacer kicked out from the jack and struck the employee on the right eyebrow, close to the nose, required 3 sutures to close.
While employee was lifting the inside elevator door at the Metcalf Secondary Crusher, he struck the top of his right hand on the elevator roof.
Employee sprayed with lime while installing valve. Albuteral was prescribed due to sore throat. (Under investigation due to flu symptoms being present and employee told health care providers he was feeling flu symptoms prior to shift start)
Employee was changing out a worn drill bit stablizer on the 49R drill. Employee picked up the bell cap and as he was turning, he experienced pain in his back. Cap weighs approximately 50 lbs.
Employee was lifting copper bookends to place in cabinet and felt pain in lower left back.
New trainee was climbing off of the 580 HT. When reaching the bottom step on the ladder it swung beneath him, causing the trainee to lose grip on the hand rail resulting in injury. ERRT was paged out and individual was taken to the clinic.
Employee strained his lower back while shutting a drain valve on the MFL shuttle.
Employee was moving fully loaded 55 gal drums from the lowere grinder floor. Employee was trying to roll the drum from a tight area and dropped the drum on his foot. The drum hit his foot on the area above his steel toe.
Employee was doing repetitive shoveling of dirt in area under pipes. He twisted to the left with a load and felt a strain in his lower back. Later he had pain in his left thigh.
In the process of installing a tire, his finger was caught between the wheel and hub.
Employee was taken to clinic due to shortness of breath while cutting cathode samples. Employee was given a prescription inhaler and released back to work.
Employee was getting off of a buggy and reached for his lunch bucket. His left hand was caught between the handrail and the lunch bucket putting pressure on his two middle fingers. Employee didn't think that his fingers were injured and went home. He went to the doctor the next day to find that they were fractured.
Employee was doing daily cleanup, shoveling belt spills onto reclaim belts. He stated that his back started hurting after he went to bed.
Employee was removing hydraulic line from hoist cylinder under pressure. Hydraulic oil and piece of metal shot out and went into his right thumb. Triage RN instructed employee to go to clinic. X-ray taken, foreign body (metal) was removed from thumb. Employee was released back to work with no restrictions.
On October 3, 2006 employee reported a near miss. Employee refused medical examination at the time. On 1/8/07 employee notified company that he had seen his physician on 1/5/07 and was diagnosed with a lower abdominal hernia. He is scheduled for surgery on 1/18/2007. Ongoing.
While rolling up a discharge hose, electrolyte splashed into employee's left eye.
