Employee was in the process of removing an impeller from a pump in the shop when he suffered a laceration to his left index finger and a small cut to his right index finger. Employee was taken into the ER room and given a tetnus shot and anit-biotics. He was refered to a specialist and scheduled to have surgery to repair the tendon on his left index finger (3/9/07) On Going

Employee was removing conduit in the central tank house and some airborne sulfate dust got into his eyes. Employee was taken to local clinic and was given a prescription for his eyes and released to full duty.

Employee noticed that a valve on cell 20 was not close all the way. He went back to re close the inlet valve on cell G 20 when he splashed electrolyte that was on the floor and electrolyte got on his clothing and saturated them and electrolyte got on his right calf on his right leg.

Employee was in the process of retrieving a rope, that had been thrown over the bank, for cable removal. He came up from the bottom of the bank and when he pulled on the rope rocks came down from the bank and hit him. The employee lost his balance and fell causing injury to his shoulder. He was taken to the clinic.

Employee was working under the pan feeder of Crusher 2 clearing rock off a concrete ledge so a skid steer loader could scrape the material out of the pocket. The skid steer scraper attachment made contact with her left ankle causing a laceration.

The employee was attempting to clear a ball jam in the chute for the #4 mill. He put his hand in to the chute to move a loose ball and the following balls rolled forward trapping his hand in the chute. While trying to dislodge his hand with his other arm, he scrapped his forearm.

After tossing the next to last ripper point and reaching for the last ripper point, the employee felt something impact his left tricep. He did not see anything and continued to reach for the last ripper point when he noticed blood trickling down his arm.

The employee was closing the front loading door on 320275 transport truck. He used his left hand to grab the door and when the door came down it struck the pipe wrench that was in his right hand pinching his right middle finger between the wrench and the railing. The incident has broken his finger.

Employee was moving cribbing during truck disassembly at dump site. Cribbing needed to be moved numerous times in order to make crane pick from dump area to tranport truck. Cribbing pieces weigh in excess of 100 lbs per piece. Approximately 50 pieces of cribbing needed to be moved during this process.

Employee was getting off of a buggy and reached for his lunch bucket. His left hand was caught between the handrail and the lunch bucket putting pressure on his two middle fingers. Employee didn't think that his fingers were injured and went home. He went to the doctor the next day to find that they were fractured.

Employee was removing hydraulic line from hoist cylinder under pressure. Hydraulic oil and piece of metal shot out and went into his right thumb. Triage RN instructed employee to go to clinic. X-ray taken, foreign body (metal) was removed from thumb. Employee was released back to work with no restrictions.

On October 3, 2006 employee reported a near miss. Employee refused medical examination at the time. On 1/8/07 employee notified company that he had seen his physician on 1/5/07 and was diagnosed with a lower abdominal hernia. He is scheduled for surgery on 1/18/2007. Ongoing.
