Employee was wearing appropriate heat resistant protective clothing working on shutting off the air to an air cannon, as he shut off the air flow, the air cannon "popped" causing hot material to become airborne. Material fell down the hot work gloves causing burns to both wrists.

On May 21, 2014 at approximately 6:30PM, employee was pulling a swing block out of the conex building just East of the bucket wheel. He reached in and pulled it out with his left arm. He felt a pain and burn in his left bicep area. Employee immediately notified another employee. He was taken to Texoma Medical Center ER for evaluation.

EE was assisting in change out of skid steer bucket. EE was having difficulty getting the bucket locking pins to release. EE was standing in front of skid steer bucket when locking pin finally released. When the bucket came free from the machine it landed on the employees left foot resulting in dislocation and multiple fractures.

Employee was replacing cord for a water pressure transduceron SS7 screen,he had set down so he could reach the LB to take the cover off. When he got up he felt a sharp pain in his lower back. He was taken to Mercy Occupational clinic and diag. with a lower back strain. He was prescribed two weeks of PT. He has been placed on light duty with follow up exams.

On Monday, August 5, 2013 at approximately 3:50pm, IE was assisting in reassembly of the booster pump. He was helping another employee pick up and carry a steel "foot" support for the pump. As the two were placing the foot onto the deck, EE appeared to have dropped his end first causing the weight of the foot to pull on IE arm.

On October 29, 2012 employee (Sr. Electrician) was changing out a size three starter bucket. He bent over to pick the starter bucket off from the floor of the MCC room and in the process of raising back up felt a pop/pain in his lower back. He was taken to the doctor and diagnosed with a lower lumbar strain, he returned to work the same day on light duty.

He then stretched his right leg out to step on the rear axle of the loader, when his left foot slipped off the battery box and caused his left shin to hit the corner of the battery box, causing a contusion.

The employee was cleaning the tower and opened a port for inspection. Hot material came out of the port and hit the employees protective hood. Particle dust got under the hood burning the employee on his right cheek, ear, and neck.

Employee was checking the water pump for refueling. When he came up on the pump he noticed the gas cap was off. As he was trying to put the cap back on the pump caught fire. Employee ran to the nearest fire extinguisher and as he was running back up a sloped area he felt something give in his right calf.

EE was trying to move steel plate back into place when he felt a pull in lower abdomen area. Went to DR and diagnosed with a strained muscle. After further testing it was found there was a hernia in the abdomen area and that surgery would be needed. After diagnose was made surgery was put off until 4/20/12 by Doctor.
