Caught in, under or between a moving and a stationary object

Technician was removing bottom guard from machine. Guard was held with a lifting strap attached to the work truck. When the guard was removed it shifted to one side caused by the material that was inside the guard. This resulted in EE's finger being caught between the ground and impact gun. This caused a laceration and broken bone on EE's left pointer finger.

Fall to the walkway or working surface

As EE attempted to step over a support cable, his right foot became entangled in it, causing him to fall on his left elbow, hurting his left shoulder.

Struck by flying object

Employee was cleaning spillage around tail pulley at plant with water hose. As he walked to the water truck and stand to increase water pressure, he was struck in the left shoulder/back area with an unknown object. The impact resulted in pneumothorax (collapsed left lung).

Fall from machine

Employee was exiting 980G loader. As he did this, his right foot slipped and went under the step, causing employee to fall to the ground while his left foot stayed on the loader step and hands slid down rail. As this happened... it hurt his left knee.

Caught in, under or between a moving and a stationary object

Employee was removing weight at plant. As he was leaning the weight against an i-beam at the plant, his right index finger got caught between the two, causing a laceration that required stitches.

We were change out the bucket teeth on a 385 excavator. The hammer struck the bucket and a small piece of metal hit EE in the abdomen. The silver of metal punctured the skin causing bleeding.
