While reinstalling hinged inspection door on main system fan housing, EE pulled awkwardly resulting in back pain. EE sought medical attention and was prescribed medication and given restrictions. Axiom was notified after the fact and is currently managing the case.

Unloading 5th wheel plate from the back of pickup bed, pushed the plate & experienced pain in (r) forearm. 1st aid implemented, closed 8/15/24. Reopened (8/16) case & seen at medical facility. Referred to Orthopedic spc. 8/19 Ortho requested MRI for 8/29 & review on 9/03. 9/03 deemed EE needed surgery for rupture of the right distal bicep tendon, scheduled surgery next day 9/04/24.

IP (injured party) and another maintenance personnel were lifting a chute into place on an empty trough while replacing the #3 screw. IP brought chute to chest level when IP felt a pop in their lower right forearm. First aid recommendations followed. On 6/21/23 went to medical facility for follow-up due to little improvement. IP placed on restrictions and PT.

Shift was ending soon and EE was returning to the bulk shack to collect their things. Noticed a camlock dust cap lying in the walkway. EE was handling camlock when they cut their middle finger. EE went to the lunchroom for 1st aid. Recommendation made for EE to be seen at medical facility for further evaluation. After evaluation it was deemed that seven stitches were required.

EE replacing shaft in an auger, wore proper PPE (face shield, safety glasses etc.). After completed task EE felt irritation in left eye. EE flushed eye multiple times throughout the day but irritation persisted. Towards the end of the day examined eye better & noticed a foreign object in eye. Recommendation for medical evaluation. Medical facility removed sliver of metal from eye.

EE went on break, noticed dusting in main packer. Went to MCC panel check that dust fan was on (seen that the dust vane & #6 screw were off like they're supposed to be). EE went to dust vane visual inspection port, removed the inspection cap to see if product was flowing, stuck hand in port, while pulling their hand back out caught their fingers tips on the vane fins.

Employee lacerated the middle finger on right hand while installing a brash bushing into a journal. Employee was taken to the hospital and was treated and released without restrictions. The physician prescribed antibiotic medication.

The employees left index finger was pinched between a pallet of material and a live roller as the pallet was being moved from the hooder system. The tip of the left index finger and part of the fingernail was lacerated during incident.

An employee was not maintaining an awareness of their surroundings while walking in a laboratory when their shoulder came into contact with a handle which was protruding from a laboratory oven door. The case was treated with first aid initially but after several weeks it required medical treatment.

Employee was lifting 20lb lid to bucket elevator and felt discomfort in his lower back. Employee was seen by a physician who determined it to be a strain. Employee was given prescription medication and released back to full duty. The employee indicated he had been experiencing intermittent flares of lower back pain, including an episode two months ago after a baseball game.

Driller failed to remove deck bushing from upper sub per procedure and while trying to thread the 2nd section of steel the top of the carousel hit the safety wrench, breaking the chain which allowed the wrench to fall, glancing off the helper's hardhat the as he was visually checking to ensure that the steel was threading properly positioned outside of the cab on the drill deck.

Employee was welding wear iron onto the bottom of a detached loader bucket. He got help to lift steel onto tabs already in place to support steel, when one failed allowing it to fall to the ground catching his glove on the way down trapping his arm underneath. Resulted in a significant bruise to left forearm requiring anti-inflammatory prescription and follow-up physical therapy.

While changing a 637 Scraper Tire, the employee lost his grip on the tire flange that was positioned against the tire and on a 8" X 10" X 48" long block. The tire flange drop onto the employee's left foot. The caregiver diagnosed a foot fracture.

The auger on the ANFO truck was stuck. EE removed the inspection plate cover from the vertical auger and put his hand inside to remove the blockage. The auger moved pinching his fingers between the auger and the auger housing.

Foreman met employee at hospital, where Dr removed piece of metal. Employee released for work. Employee went to hospital the following day for follow up. Dr found rust in eye. Employee sent to eye Dr., released.

The operator was helping mechanic repair a leaking hydraulic line. EE was building a more stable area to stand. While placing the building materials, he caught his left index finger between the rocks. Care for the injury was needed by the local clinic. Stitches & an X-ray, showing a small fracture were the medical care received. No work restrictions. No lost time.

We have been trying to manage this case as a first aid as it was first reported to the employer. The employee requested to seek medical attention and at that time he was placed on restricted duty 8-27-2008.

Employee was working on a forklift repairing the main lift cylinder, due to a mechanical failure. The clamp and bracket holding the cylinder in place had come loose. The employee was loosening the clamp cylinder. The forklift was not properly secure from movement while loosening the clamp; the mast dropped about 6 inches striking the employee's right shoulder.

Employee was stepping from the warehouse floor onto the deck of a flatbed trailer. The deck of the trailer was aprox. 12" above the floor. Employee's shin came in contact with the side/edge of trailer just above his boot causing a laceration that required 6 stitches.

A piece of iron had gotten wedged in crude ore slicer plate. Employee was standing inside slicer hole attempting to dislodge the piece of iron using sledgehammer. On one of the downward swings his left forearm struck against a metal plate, used as an ore deflector plate, sticking out into the slicer hole approximately one to two inches. Employee received 8 to 9 stitches.

Employee was bending over to lay bags down and suddenly experienced a sharp pain in his back. Employee was diagnosed with lumbar strain and prescribed pain medication and an anti-inflammatory.

EE reported he bumped right elbow on the railroad track while working on the bottom of a railcar. EE advised that he now has a cyst about the size & shape of an egg on right elbow. EE had an incident similar to this in January 2005 in which he treated with his personal physician on his own. He stated that he had no problems with his elbow prior to 5/25/05.

Handling empty wooden pallets at bore-gel plant. Diagnosed with lateral epicondyitis. Employee tried home self tretment options before seeking professional medical treatment. Thus the delay in reporting. Diagosed with lateral epicondyitis and restricted to 48 hours of light duty.
