Employee cleaning off table in maint shop, got object in left eye. Employee tried to remove object. Employee called foreman after his shift and wanted to get treatment. Foreman met employee at hospital, where Dr removed piece of metal. Employee released for work. Employee went to hospital the following day for follow up. Dr found rust in eye. Employee sent to eye Dr., released.

Employee was working on a forklift repairing the main lift cylinder, due to a mechanical failure. The clamp and bracket holding the cylinder in place had come loose. The employee was loosening the clamp cylinder. The forklift was not properly secure from movement while loosening the clamp; the mast dropped about 6 inches striking the employee's right shoulder.

Fall from scaffolds, walkways, platforms

Employee was loading a bulk truck and had just finished closing and latching down the trailer lid. He noticed that the safety ramp was caught on the hatch. He raised the ramp a little to release it from the lid. The truck driver thought he was loaded and drove off leaving the employee hanging from the handrail of the ramp. He fell approx 12' 9"" to the truck scale.

Employee was cleaning the rail scale at the Dunphy Grinding Plant, employee walked across the manhole cover located in the middle of the scale. The employee was unaware of his surrounding and stepped into an uncovered manway.

EE WAS UNABLE TO DETERMINE THE ACTUAL TIME AND ACTIVITY THAT RESULTED IN THE BACK PAIN HE REPORTED. THERE WAS NO PAIN AT START OF SHIFT BUT STARTED JUST BEFORE HE REPORTED IT. THE PAIN WAS MOD ERATE AT REST BUT WORSE WHEN BENDING OR TWISTING.

SHORT SLOPE FROM MILL ARE TO RAIL CAR LOADING AREA WAS COATED IN SNOW WHICH HAD BEEN PACKED DOWN BY TRAVEL. SUBJECT FELL ON SLICK SURFACE.

EE PICKED UP BROKEN BAG FROM FLOOR. AS HE STOOD UP, HE FELT PAIN IN HIS LOWER BACK.
