Technician was laying under a loader installing a driveshaft. The bearing caps were not secured and one slid off the end and struck technician in the tooth, causing a horizontal fracture.

Employee was handing up 10Ft. length of chain to fellow employee when he felt a strain in his left shoulder.

While another employee was lowering a screen down it slipped and hit employee on left foot causing a contusion.

Employee stated that while shaking out dust from a rag after wiping down truck dashboard, the tip of rag hit him in his right eye causing abrasion.

Employee was removing a hose clamp from a water pump when he felt a pop in his right ring finger. Diagnosis: Tendon strain.

Employee and Co-worker were handing up a 95lb. screen to another emplyee. He indicated afterward that he felt pain in left shoulder.

Employee was checking for a leak from a drill rig hydraulic hose. As he moved hoses around, pressurized fluid shot out & caused a puncture wound to his left middle finger tip.

Employee was sitting in haul truck, when he closed the truck door, his left thumb got caught between the door and the door frame. This caused a laceration, requiring 4 stitches.

Employee was removing a broken feeder plate and after the last bolt was removed, the plate released, striking employee on the back of his hard hat.

EMPLOYEE WAS HOLDING VALVE WHILE MECHANIC WAS REMOVING SHAFT AND A PIECE OF METAL FLEW OFF AND HIT HIM IN LEFT UPPER LIP

Employee was carrying a 4' x 8' shaker screen with a co-worker when he felt pain in his lower back.

While changing screens, employee reached under screen to retrieve a bolt while screen was being lowered. Screen edge hit employee on shoulder& back of neck causing bruise / contusion.

Employee was lifting a generator trailer tongue to connect it to a vehicle and he felt a pull in his left elbow.

While replacing a haul truck suspension cylinder the employee got his left index finger piched between the cylinder & the truck when the cylinder shifted causing a fracture to finger.

Employee was handling steel pipe when he felt a pull in his lower back.

Employee got his fingers caught between a steel cable and a pulley snatch block causing fracture of left ring & middle finger.

Employee was lifting a 4" x 4" x 1/4" angle iron when he pulled muscle in back which resulted in a thoracic spine, strain.

Employee was checking circle drive on a grader that was not working properly. As the operator moved the drive from left to right, the yoke broke free and the PTO drive shaft struck the EE above his left eye causing a laceration requiring 6 stitches.

Employee stated he strained his lower back while shoveling.

EMPLOYEE WAS DRAINING FUEL TANK AND THE FUEL SPLASHED UP INTO HIS RIGHT EYE.

Employee was lifting shaker screen into position when he strained his lower back.

While setting a steel plate into proper position, employee pushed from an awkward angle and felt a twinge in his lower back. He was diagnosed to have a lower back strain.

Employee was unloading a 20' piece of drill casing from the back of a truck when his left index finger was caught between the pipe and a rock on the ground causing a laceration requiring 2 stitches.

Employee received 6 stitches & a tetanus shot when he caught his finger between a screen deck hole and a plant rail. he was lowering the deck to another employee when it slipped.

I was installing a drag friction shoe on a 2400 Lima dragline and during the course of installing the shoe into the drag drum flange the shoe slid to the right and I reached down to catch the shoe to keep it from falling and it pinched my finger between the shoe and the mounting bracket

EMPLOYEE WAS PUSHING ON SMALL PUMP BARGE BECAUSE IT WAS HUNG UP ON WALL.

EMPLOYEE WAS LIFTING A DRILL BIT OUT FROM A SHARPENING HOLDER. HE LIFTED THE BIT FREE & ITS MOMENTUM CAUSED IT TO STRIKE EE IN THE CHIN, RESULTING IN A LACERATION (3 STITCHES).

Employee was shoveling around crusher and pulled muscle in stomach.

Employee was hand shoveling stone into a hole prior to blasting operation. Later that day he complained of numbness in his chest and fingers. diagnosis: pulled chest muscle.

EE was positioning a wing pulley on the welding table and pulled a muscle in his neck. the pulley weighed approx. 100 lbs an was being slid on the table.

EMPLOYEE PINCHED FINGER BETWEEN THE TWO HALVES OF A MOTOR BASE. HE HAD JUST CHANGED THE V BELTS ON THE NO 1 SHAKER AND WAS PIVOTING THE MOTOR BASE BACK IN PLACE. HE RECEIVED 7 STITCHES.

EMPLOYEE WAS PICKING METAL / GARBAGE OFF A CONVEYOR BELT. HE TOOK HIS SAFETY GLASSES OFF TO CLEAN THEM AND FELT SOMETHING GO INTO HIS EYE.

EE WAS HOSING OFF WINDSHIELD OF DRILLING MACHINE. A PARTICLE OF STEELE THAT MUST HAVE COME FROM EITHER THE DRILL BIT OR STEELE CASING BLEW INTO HIS EYE CAUSING IRRITATION.

WHILE KNOCKING OUT A BOWL LINER WITH A STEEL SHAFT, THE SHAFT SLIPPED AND PINCHED THE EMPLOYEE'S FINGER BETWEEN THE SHAFT AND THE BOWL CAUSING A LACERATION REQUIRING 5 STITCHES.

AFTER TWO DAYS OF SHOVELING EMPLOYEE STATED HE FELT PAIN IN LOWER BACK.

EMPLOYEE WAS TRYING TO PULL ROCK OUT THAT WAS STUCK IN CRUSHER AND WHEN HE WAS CLIMBING OUT OF CRUSHER HE SPRAINED HIS SHOULDER.
