Technician was laying under a loader installing a driveshaft. The bearing caps were not secured and one slid off the end and struck technician in the tooth, causing a horizontal fracture.

Employee was removing upper engine panel on 980 Loader. After removing 3 of the 4 bolts, the panel swung down, striking the employee's left forearm causing laceration requiring 9 stitches
Employee was adjusting a conveyor belt, his wrench hit the conveyor guard, dislodging it from its hook. The guard fell, hitting the employee finger causing a contusion.

I was installing a drag friction shoe on a 2400 Lima dragline and during the course of installing the shoe into the drag drum flange the shoe slid to the right and I reached down to catch the shoe to keep it from falling and it pinched my finger between the shoe and the mounting bracket
EMPLOYEE WAS LIFTING A DRILL BIT OUT FROM A SHARPENING HOLDER. HE LIFTED THE BIT FREE & ITS MOMENTUM CAUSED IT TO STRIKE EE IN THE CHIN, RESULTING IN A LACERATION (3 STITCHES).
