Technician was laying under a loader installing a driveshaft. The bearing caps were not secured and one slid off the end and struck technician in the tooth, causing a horizontal fracture.

Employee passed out due to aperient drug overdose.

Employee was helping replace return roller on crusher while crusher was down for repairs and EE failed to have EE's hard hat on. EE hit EE's head on the guard for the roller while going under the crusher.

Wile working in limited access area where hard hat could not be worn, employee stood from crouched position after inspecting parking brake area and hit head on bed lift cylinder causing laceration at left eyebrow.

Employee was operating a dozer on a berm. Employee went to back up and backed over the side of the berm, turned dozer over and slid down the berm.

Employee was cleaning grizzly bars in shaker feeder of crusher with pick axe, left foot slipped and he tried to catch his balance, did not fall, but twisted lower back. He has lumbar strain.

EMPLOYEE WAS CLIMBING DOWN FROM LOADER, MADE IT TO SECOND STEP AND FELL.

EMPLOYEE WAS TURNING THE TRUCK TO THE LEFT, STEERING WHEEL SPUN BACK TO THE RIGHT CATCHING RIGHT HAND AND ARM.

EMPLOYEE WAS OPERATING DRAGLINE AND STRAINED HIS BACK WHEN EXITING THE CAB.

He was checking water in radiator and slipped and fell on deck of machine hitting head and left hand.

EMPLOYEE WAS FEEDING CRUSHER WITH BACKHOE AND A SMALL PIECE OF ROCK HIT HIM IN RIGHT EYE.

EMPLOYEE REACHED IN FUEL TRUCK TO PICK UP RAG AND WAS BITTEN ON HIS RIGHT HAND.

EMPLOYEE WAS FINE WHEN HE LEFT WORK ON 6/7/2005. HE CALLED IN 6/8/2005 AND SAID HIS BACK WAS HURT AND DID NOT KNOW WHEN IT HAPPENED. WE ARE NOT SURE IF THIS IS WORK RELATED.

EMPLOYEE WAS CLIMBING DOWN FROM BACK HOE. RIGHT FOOT SLIPPED AND HE HIT HIS RIGHT KNEE ON THE TRACK.

EMPLOYEE WAS OPERATING DOZER, BLADE HIT PIECE OF FLINT ROCK, JARRING MACHINE AND HIS LEFT HAND SLIPPED OFF LEVER & HIT DASH.

I was installing a drag friction shoe on a 2400 Lima dragline and during the course of installing the shoe into the drag drum flange the shoe slid to the right and I reached down to catch the shoe to keep it from falling and it pinched my finger between the shoe and the mounting bracket

EMPLOYEE WAS OPENING DOOR ON ENGINE COMPARTMENT AND HE SLIPPED OFF AND LANDED ON LEFT FOOT.

EE WAS RUNNING DOZER AND GOT SOMETHING IN LEFT EYE. HE DID NOT REQUEST TO SEE DR UNTIL 1-15-04.

EMPLOYEE WAS WORKING ON THE FINES CONVEYER TAIL PULLEY WHEN ANOTHER EMPLOYEE TURNED ON CONVEYER.

EE CLAIMS HE TOOK JUG OF WATER OUT OF REFRIGERATOR AND CLAIMS TO HAVE DRANK 1/4 OF THE GALLON BEFORE REALIZING THERE WAS SOMETHING OTHER THAN WATER IN IT. HE CLAIMS HE GOT ILL AND VOMITTED BUT NO EVIDENCE WAS EVER FOUND.

EMPLOYEE WAS TRYING TO PULL ROCK OUT THAT WAS STUCK IN CRUSHER AND WHEN HE WAS CLIMBING OUT OF CRUSHER HE SPRAINED HIS SHOULDER.

EE WAS OPERATING DRAGLINE, BRINGING LOADED BUCKET OUT OF WATER AND HOIST CABLE BROKE AND CAME INTO THE CAB HITTING EE ON HIS RT. LOWER LEG.

EE WAS HELPING MECHANIC WITH ROLLERS ON D8N DOZER AND ROLLER FELL ON HIS THUMB.
