EE was walking under conveyor, EE hit hard hat on the bottom of the conveyor. It knocked EE's hard hat off. After bending down to pick up hard hat EE hit head on way back up.

Showing all 8While cleaning out receiving box on conveyor, employee stepped backward and fell through ladder access opening. Fell approximately 12 feet resulting in left arm muscle strain.
Injured worker was pull starting a gasoline powered water pump. Pump engine "kicked back", pulling arm and straining shoulder.
Employee was installing drive belts on motor and pulley. While turning pulley his index finger was caught between belt and pulley. Index finger on left hand had 2 cuts and loss of fingernail.
EE STEPPED DOWN FROM WATER TRUCK, SOMETHING CAUGHT HIS RING ON LEFT HAND. RESULTED IN LACERATION BOTTOM SIDE OF FINGER.
