Hit left hand with hammer, while trying to move pin from from pulley. Broke left thumb.

While cleaning out receiving box on conveyor, employee stepped backward and fell through ladder access opening. Fell approximately 12 feet resulting in left arm muscle strain.

EE did not keep safe access between a service truck and a tractor trailer. The trailer swung short and struck EE resulting in a broken right leg.

EE was getting out of his truck, lost his footing and fell of truck onto ground. He broke his left femur. EE is a licensed CDL truck driver that delivers finished product to customers.

Employee was delivering a load of limestone @ the plant for resale. While dumping load, the truck and trailer turned over. Employee hit his left leg on steering wheel. Inner thigh swelled up.

Employee was installing drive belts on motor and pulley. While turning pulley his index finger was caught between belt and pulley. Index finger on left hand had 2 cuts and loss of fingernail.

THIS MAN WAS TRESPASSING ON OUR PROPERTY AFTER HOURS. WHEN I ARRIVED OUR PEOPLE & THE LIFE SQUAD WERE TRYING TO GET HIM OUT OF THE SAND. THE MAN RECEIVED NO INJURIES AND CHARGES WERE FILED BY SHERIFF.
