Fall onto or against objects

Clearing jammed crusher. IW was prying on rotor when pry bar slipped. IW slipped and fell backward to sitting position. Left leg caught on catwalk or handrail during fall. Sprain/bruise (soft tissue damage) to left leg at knee.

Over-exertion in pulling or pushing objects

Injured worker was pull starting a gasoline powered water pump. Pump engine ""kicked back"", pulling arm and straining shoulder.

Struck by falling object

Injured worker was helping foreman with crusher rebuild. Foreman tapped adjusting rod with sledge hammer. Apron fell, catching IW's left index finger.

Injured worker was cutting skirt rubber for conveyor with box knife. Knife slipped off rubber, blade penetrated right thigh.

Flash burns (electric)

Injured worker was preparing to open our Hazmag crusher to do clean up and adjustments. He was at the switch box to start a small electric pump motor which runs the hydraulics used to open a crusher. He pushed the ON button (or threw the disconnect lever), and an electric arc blew open the box door, causing flash burns to arm of injured worker.

Fall down stairs

WORKING MUDCAT STAND WITH (WITNESS) EE. REACHED OVER TO THROW LARGE ROCK OFF CONVEYOR BELT, LOST BALANCE, FELL AND ROLLED DOWN STAND ACCESS STAIRS. APPRENTLY STRUCK ROCK ON FRAME STRUCTURE WIT H RIGHT HIP.

INJURED WORKER WAS HELPING MOVE SCREEN SECTIONS PREP FOR SCREEN CHANGE ON GRAVEL PLANT SHAKER.

Struck by falling object

INJURED WORKER WAS HELPING FOREMAN ATTEMPT TO OPEN JAMMED OVERHEAD DOOR AT GENSET BUILDING. TOP OF DOOR DISENGAGED FROM TRACK, FLIPPED DOWN, THE LEADING TOP EDGE OF DOOR BROKE OFF AND FELL TO GROUND. INJURED WORKER WAS APPARENTLY STRUCK ON HAND AS DOOE TOP FELL.
