The employee and a contractor were removing an inspection cover from the top of the clinker cooler drag-chain. The employee's tip of pinky finger on left hand became pinched between the bottom of the cover and a metal flange as they placed the cover on the ground. The employee's fingernail was almost avulsed from the nail bed

Employee was pushing a small pile of material with their right foot in a sweeping motion, when their left foot slipped on loose material. Instead of falling forward, they leaned back and turned their body in a twisting motion, during which time they felt a pop in their knee.

EE was inside manlift basket using a steel lance attached to a vacuum hose to clear material from apron feeder below. While vacuuming the material the hose became stuck and EE tried to free it by yanking on the lance. The hose broke free and EE's hand flew back striking the corner of the basket's railing breaking the 5th metacarpal bone in EE's right hand.

EE was pulling a Wright dump hopper from under the Clinker Cooler. While navigating through the "I" beams the hopper struck the edge of a beam causing the hopper to stop suddenly. EE's momentum continued backwards while gripping the hopper during which they heard a pop in left shoulder. EE immediately felt sharp pain in their shoulder/neck and reported it to a Supervisor.

As the employee was stepping out of the raw mill, the employee placed a hand on the raw mill tire hook and disrupted the accumulated dust causing it to become airborne. The airborne dust entered the airstream of the cooling vent fans and blew onto the employees face and under safety glasses causing dust to enter left eye.

Employee was removing a bracket and a grease injector assembly from the center arm of a wheel loader. When they removed the bracket that was over their head they felt something pull in their left shoulder and arm gave out. They have pain in their left shoulder. They saw doctor on 8/5/25 and started restricted duty on 9/8/25.

EE helped two workers lift a 4x8 screw lid back on top of its housing at Silo 17 Screw Floor. EE and workers lifted the lid onto the housing and began to push it into place when they heard a pop and felt slight pain in left shoulder. EE asked to see a doctor after pain persisted for 10 days. EE had surgery on 09/02/25 to correct a torn rotator cuff and bicep tendon.

Employee was walking backwards down the backside of the clinker dome access ramp when EE felt a strain in EE's right knee. The employee continued to experience pain in EE's knee and was referred to an orthopedic specialist for further diagnosis and evaluation.

Employee was opening the water truck cab door to open the water tank fill valve when the tip if EE's right fifth finger got wedged in between the cab door frame and the water fill tank valve resulting in a laceration.

EE was attempting to unplug a triple gate at the coal mill with no success. EE then introduced EE's hand into the triple gate. The gate activated, nipping the tip of EE's right middle finger.

On 5/13/25 EE lifted an oil bucket onto a truck with right arm and felt pain in shoulder. EE told their supervisor but didn't seek medical evaluation. When the pain persisted EE requested a doctor's appointment and was seen on 06/25/25 where they were treated for strain of rotator cuff & prescribed PT. EE had MRI on 8/1 and then surgical repair of right rotator cuff on 11/20/25.

EE was fueling EE's loader and when EE finished EE went to lower the rubber cover over the fuel cap. As EE reached for the cover EE jammed EE's left thumb on a bracket EE didn't see. MRI reveals left thumb tendon tear. Tendon transfer surgery performed on April 11, 2025.

On 1/21/25 EE was lying on EE's back with 2" of clearance while replacing screens on C114 Screen. EE completed job and while lying on EE's back EE had to push self uphill using EE's feet and elbows. EE reported to supervisor a shoulder strain. On 3/20/25 EE saw doctor for first time. EE was given an MRI that revealed right shoulder labrum tear. Surgery was performed on 6/26/25.

EE was helping a coworker install a snub roller scraper on A20 belt. EE lifted the scraper with both hands and felt a pain in right bicep. The scraper is approx. 60" long and weighs approx. 50 lbs. This became reportable when the doctor placed EE on restrictions on 12/24/24.

After the employee bled the hydraulic system for the raw mill, the EE checked the gauges to verify that they were reading zero. EE started removing a blank off one of the hydraulic manifolds for #3 roll cylinder, but as EE was removing the third bolt from the blank, the hydraulic fluid sprayed out hitting EE in the chest with fluid.

As employee was installing bolt on the jack of the raw mill. As the employee began installing bolts the padeye gave way. Causing the rams head to fall back down onto the employees right index, middle, and ring fingers. Crushing the tips of the fingers. The employee was taken to the hospital, and the tips of each finger were severed between the last knuckle and the nail bed.

Employee was removing concrete blocks that were installed to contain the grinding balls from a finish ball mill for bull gear work. The employee was assisting in the removal of the blocks by guiding a block out from under the mill. Employee's finger was pinched between the block and the wall.

EE was installing new dust collector bags in one of the old dust collectors. In order to properly install, EE had to push upwards above their head and twist at the same time. While doing this EE experienced a sharp pain in the middle of EE's back. The employee visited Urgent Care. The doctor gave EE restrictions.

EE was diagnosing problem with a blower. They locked/tagged/tried out the blower and was wearing gloves. They tried to maneuver the pulley manually. As they did the tip of their left thumb was pinched between the belt and pulley. They received a small fracture at the very tip of their thumb and was given a splint for the thumb.

Employee was cleaning on top of the blending bins silos. The manhole lid for a silo was removed for material to be swept and pile to be cleaned. The employee stepped on the edge of the grate causing it to lift up. As the grate lifted up, EE's leg fell in the hole causing it to strike EE's right shin on the edge of the manhole opening, causing a laceration.

Employee worked in Primary Crusher removing rocks to unplug crusher. Placed one rock on a mound of material while partner would take rock and place it in a bucket. They were working in crusher for 3 hours when one rock they placed on mound slid back towards them falling 2 feet off mound and struck right thigh above knee eventually causing right knee abscess. Surgery: 8/12/24.

Employee was attempting to manipulate hydraulic lines for the Loesch roller lift cylinder in the number one position when EE felt a pain in left testicle. The physical constraints of the location and relationship of the hydraulic lines to the cylinder required EE to be in an awkward position.

EE and EE's partner were trying to push a new troughing idler into position on the 215 Belt Conveyor when EE tweaked lower back. EE informed the supervisor and no first aid was requested. Because of ongoing back pain EE was sent to the doctor on 4/19/24. Diagnosis: low back pain. Work status: Restricted Duty. Limit bending, pushing, pulling, lifting to 15lbs or less.

EE was operating a Komatsu D275 Bulldozer. As EE dismounted the dozer, EE felt a pull in their lower back. EE experienced pain down leg past EE's knee. EE immediately reported this to the supervisor. The next morning upon coming to work, EE requested to see a doctor because of lower back pain. Diagnosis: Strain of muscle, fascia and tendon of lower back, initial encounter.

EE was installing a takeup pulley on clinker feeder belt. It was placed on upper part of mobile work platform. When they & partner were attempting to slide it on platform to get it in place it started to slide off platform. EE reacted by trying stop the pulley and got a finger between pulley and grating of the platform causing a small fracture at the very tip right ring finger.

EE attempted to unclog the vac truck hose with EE's left hand. When EE did, the rock then pinned EE's hand between the hose wall and the rock, causing pain and swelling.

An employee was in the process of moving some fairly large steel cooler parts (beams) from the forklift forks onto some grating. In the process initial indications are that the beam slid, and EE's finger got caught between two solid objects causing the injury. Cut finger and fracture to finger tip.

Employee was working on a flat platform for easy access to door closure. They felt the door was loose and did not relieve pressure from the unit. The pivot arm swung once they released the connecting pin. The arm lacerated the employee's ear.

Employee was removing a wheel chock from behind the tire of a haul truck. EE reached down to pick it up with right hand and proceeded to lift it above their waist to the front of the haul truck in the designed rack. As they lifted the wheel chock up, they felt a pull in the lower abdomen, groin area. Employee refused treatment until 2/22/24, the day of the diagnosis.

In the process of unloading a portable building, a hinged wall fell and struck the employee's left thigh, causing a contusion. Initially seen by the local hospital and released with no restrictions. Due to weekend soreness, the employee's doctor placed them on one week of restricted duty.
