Pushing & pulling barrel / drum of antifreeze and strained shoulder (R).

Fueling the jaw crusher, EE lifted the cover which had a broken strut. While attempting to secure the lid, it fell onto EE's left arm.

While removing the cutting edge of the loader bucket, the cutting edge dropped to the floor and caused a bolt to pop up and strike EE in mouth

Right thumb got caught between mounting plates while installing (replacement) speed reducer on #1 stacker

Employee was inserting lock pin as the rotor was shifted by another employee. The pin inadvertently shifted and struck employee's left index finger.

While installing pin for a hydraulic cylinder on a track hoe, the pin fell onto injured employee striking EE in the back.

Employee placed hand between clamp while repairing a conveyor belt. Clamp swung around and struck right middle finger causing avulsion to fingernail.

While working on a rock drill, employee used the upper arm, backside to push hoses up and away to move a bracket. Shoulder popped.

Raising hood on road dump truck and strained groin area.

Picking up a loader cylinder lift pin and strained left shoulder.

Strain in abdomen allegedly lifting a 100lb broken hammer tool bit.

Removing rock from impactor. Lifting a rock that cause the shoulder to pop and cause pain.

After cleaning cone crusher employee exited by pulling himself up to top. A shoulder sprain occurred with rotator cuff tendonitis. Corrective action will involve fabricating a portable ladder with attachments and non-slip rungs.

Changing screen in shaker, it fell twisting wrist.

Jammed rock; rock broke loose striking smaller finger of right hand. A cut and broken bone was the result. Corrective action: Cut plates to minimize hang-ups, use pry bar instead of hands.

EE reports that when entering the front end loader he struck his right shoulder against the side of the doorway.

Installing new training idler on the conveyor, the idler slipped down the frame of the conveyor a few inches and struck employee in the elbow.

Employee was restacking deck boards on low boy trailer and ruptured his bicep tendon.

Employee was attaching a band type hose clamp to a water pump. He cut the back of his left hand on the end of the clamp. The cut was between his middle and ring fingers. The location of the cut required stitches to keep the cut closed. Cut was stitched and employee returned to work.

Employee was helping to set a cone crusher and the crusher slipped cutting off the tip of his right thumb. Doctor is going to graft some skin on thumb and it should be fine.

Employee was shoveling spilled rock from around plant and pulled a muscle in his back.

Engine hood of 900 Komatsu loader fell on employees head.

Employee was pushing a radial stacking conveyor by hand and pulled his rib muscles.

MASHED FINGER BETWEEN TIRE AND SPINDLES WHILE CHANGING TIRE ON DUMP TRUCK.

WHILE ATTEMPTING TO ROLL A 300 LB ROCK, THE EE LOST GRIP AND ROCK ROLLED ONTO FOOT CAUSING FOOT TO TWIST AND BREAK TOE.

ROLLING UP USED 36" CONVEYOR BELT AND HURT BACK.
