While making repairs to the coal exhaust fan, employee was rolling a belt manually. Employee's right ring finger and pinky were caught between the belt and pulley resulting in fractures and sutures.

Employee stated that during a power outage ee was trying to close a gate that normally opens and closes with an electric gate opener. Gate was switched to manual setting, in process of closing the gate the employee strained left shoulder.

Employee was tightening a band around a 36" pipe on the recycle lake line at the west side of Phos Acid. Employee's hand slipped and contacted the edge of the band. The contact resulted in a cut on right hand that required four sutures to close. Employee was wearing gloves at the time of the incident.

Employee was changing drill bit on a press drill. Employee failed to engage lock fully and as EE removed bit the drift pin fell. Employees left thumb was struck. Thumb bone was fractured.

Employee was replacing dump rope on the 2550 dragline. When repositioning the dump socket to enable safe removal of the wedge, the socket shifted, pinching employee's right index and middle fingers between the dump socket and drag chain. Employee was wearing gloves at the time of the incident.

Float Crew was changing out a 7' long 20" exp. joint on matrix pump. Bolt holes were not lining up on flanges. Float Crew Leaderman(FCL) used a lifting strap as choker around exp. joint. FCL tightened down on strap & signaled operator to hoist up. As operator started hoisting, the strap broke. FCL left hand was still on strap causing it to suddenly shift.

Miner was using a 2.5 gallon urea (DEF fluid) container with a flexible funnel to fill a DEF fluid tank. While lowering the jug after filling, miner stated a few drops of DEF fluid entered his eye causing a minor chemical burn.

Employee was in the transfer station assisting with trying to get the #39 conveyor back running (frozen). He was walking back towards the head pulley area. As he passed under the 17-3 flop gate platform, he was concentrating on his walking/working surfaces and bumped his head on the 17-3 flop gate platform resulting in a laceration to his forehead.

While performing dragline repair work, employee's fingers were pinched between a pin component and flange while the pin component was being moved into position. Resulted in lacerations on two fingers requiring sutures.

A crane was being used to lift a load and the employees right fore finger was pinched between the load and the base. Employee was taken to the local medical facility where the wound was cleaned, and he received 3 stitches. He was also sent to the local hospital for x-rays and this showed no bone fracture. No prescription medications were given.

Operations was in the process of relocating a de-energized power cable. Employees were positioned as spotters as a backhoe was used to pull the cable. One employee was on the opposed side of the cable from the others. As cable was pulled it became hung up on a berm. As employees signaled for the backhoe to stop the cable broke through the berm and struck employee at the knee.

Employee was installing a bearing cap in a drag line. The bearing cap was being tapped into place when the 70# bolt dropped down and struck the employees hand resulting in an avulsion. This required 10 stitches to employees left hand.

Operator was attempting to lean seat forward in pit car. On his first attempt the seat would not adjust. The operator leaned forward to apply more pressure and moved his left hand to the front of the seat with his fingers curled up under the seat. When the seat released the operator's left middle finger was pinched between the seat frame and the adjustment bar.

Prestrip operators were assisting shift maintenance in the installation of a roller on the 239 conveyor belt. Employees were working on each side of the belt. As employees on one side of the roller positioned it into place the other end of the roller moved and pinched employee's left ring finger between the roller and the mounting bracket.

Employee was in the process of climbing up onto a dewatering barge in the No. 4 area. While doing so he felt a pop in his left hand, employee felt no immediate pain but approx. 10 minutes later his hand started to hurt. He was treated at company clinic. On 7/12/13 employee was referred to orthopedic institute and placed on light duty.

Employee was trying to knock a bearing in with a battering ram at top of boom. On the third attempt to strike bushing, the ram entered the pin hole and continued swinging forward toward the frame. Employee's left ring and small finger were pinched between the ram pad eye and the excavator.

Miner reported injury to right thumb while opening a soda during lunch. First Aid treated for a minor cut by EMT and returned to work. The Miner returned to work on 03/18/13 (next scheduled shift) with complaints of pain, wound obviously inflamed and red. Miner taken to see M.D. where he was dignosed with an infection and provided prescriptions. Return to work 03/21/13.

Contract miner was shoveling dirt into a bucket taking 20 minute shifts. Contract miner completed his 20 minutes of shoveling and was standing, talking with other miners when he complained of a back spasm. Contract miner was taken to first aid for evaluation, given heat pack and returned to duty.

Mill maintenance mechanics were changing rollers on #3 conveyor belt. While both mechanics were replacing roller, one mechanic lost control of the roller. The weight of the roller shifted, pinching other mechanic's left index finger between the roller and conveyor support frame.

While dismantling some scaffolding on the contractor scaffold yard an employee suffered a broken tooth when a scaffold member was mishandled by a coworker. The scaffold member was mishandled while being passed to the injured employee. Injured employee suffered a broken tooth and busted lip. The injured employee received prescription medication and the tooth was filled.

Contractor Employee (C1) was moving refractory bricks from a manway inside a calciner when a second Contractor Employee (C2) accidentally dropped a tweeter pipe on C1's right hand. C1 was taken to Beaufort County Hospital and x-rays showed C1 had a fractured index finger.

Employee exited a two ton flat bed truck. Employee has the edge of the door between his ring and little finger and began to swing the door shut, when his elbow contacted the flatbed that extends out beyond the cab. This jammed the edge of the door in between the ring and little finger.

He was seen by co. dr. on 1/12/10 stating he was getting better. Returned to co. dr. 9/15/10 & referred for ortho consult. He saw ortho dr. on 10/4 & was prescribed PT.

Bucket Wheel Mechanic (BWE1),with thirty five years experience, was removing bolts from a brake pack/adapter plate secured in a vise on the back of a work truck. When the vise pressure was released to rotate the position of the assembly the unit rolled out of the vise pinning left index finger between the adapter plate & the truck bed. BWE1 was transported to First Aid.

Mob. Mech. was replacing alternator on D-10 tractor. Access is through a hole in floorboard of cab. He was reaching through the hole to install alternator when he felt pain in his L shoulder. He completed job & contacted Foreman re: incident. He was seen by co. dr. on 9/24 & referred for ortho consult. He saw ortho dr. on 9/30 & was prescribed PT & given a 20 lb. lifting rest.

Emp was moving a 3"X7' shaft (approx. 180 lbs). Later he noticed tenderness in his lower abdomen. He went to the clinic the next AM, evaluated by the nurse & given OTC meds. On 4/23 he was seen by the co. dr. The PM of 4/23, worker returned to the clinic with extreme pain & was referred to a 2nd dr., which he saw on 4/28. He was then placed on modified duty pending surgical eval.

Employee was working on #1 conveyor replacing return rollers. Employee had both hands on the roller with his right hand on the end and left hand supporting the weight of the roller. As he pulled the roller out from conveyor belt, roller slipped; his 4th finger of left hand was pinched causing a superficial abrasion.

Employee was repairing access ladder on #4 pit pump. The pump sled was covered w/uneven dirt/buildup, the dirt shifted and he felt a slight pain in his L knee. Reported to clinic on 5/1 w/discomfort in his L knee & received first aid treatment. On 8/20 he returned to clinic c/o continued discomfort. Referred to Dr. and on 9/12 was given restrictions and referred to ortho surg.

Employee was cutting a 600 - 800 pound piece of steel off of the center pin assembly of the #1 dragline tub. The steel was supported by a chain attached to a boom truck. When the steel was cut free, it shifted, striking the employee.

D9 dozer operator walked up to co-worker to ask where to pull tail pulley for repair. Co-worker was bent down hooking up a 3/4" U shackle to the tail pulley. Co-worker raised his arm to point with the shackle in his hand and hit the operator in the mouth, resulting in a chipped and cracked tooth.

Employee was lifting buckets of bolts from the back of a truck. The employee did not notice any discomfort that day, but during the night she began to experience pain in her left shoulder. The next day she told her supervisor of the pain in her shoulder. On 2/8/07 she was evaluated by the company physician and was ordered restricted duty.

The EE was cleaning the dust off the screw conveyor though with the back of his right hand before placing lids on it. The EE had cotton gloves on. He felt something on his right hand and tried to remove his glove, but would not come off. A piece of wire was sticking through his glove. A piece of wire was going through his right hand middle finger above the 1st joint.
