Employee was trying to free up a frozen cam-lock on a coupling hose to a pump on a classifier. EE was pushing hard on the clamp when the locking pin broke. EE then smashed a finger into the pipe, breaking the middle finger on left hand.

Sometime in January (employee is unsure of date), employee was lifting a steel beam. EE picked it up and put it on EE's shoulder. EE felt pain in EE's left shoulder. The pain persisted for several weeks, but EE did not report the injury until April. EE had EE's shoulder checked, then had an MRI. It appears EE will need surgery.

A rock was caught in tail pulley. Employee turned conveyor off, locked it out, and loosened belt to remove rock. Employee tried to turn the belt to roll the rock out. When the rock came out, the belt shifted, and the pulley grabbed employee's glove, pinching left index finger, causing a fracture.

Employee was flipping blow bars on a Hazemag crusher. The blow bar spun and hit hand between the blow bar and the rotor. The blow broke the employee's right pinky finger. EE went to a hospital. They x-rayed finger, put in a few stitches, wrapped it up and released without restrictions to go back to work.

Employee was removing scrap repair items from a loader bucket and throwing them into a dumpster. He got a little piece of something in his right eye. He used eye wash to wash out his eye. He said it felt fine, so he went back to work. That night, his eye was bothering him, so he went to an urgent care facility. They told him he had a corneal abrasion.

Employee was working on a radiator shroud for a dragline. When handling the shroud, he scraped his left ring finger. He washed it and went back to work. He ended up going to the emergency room on July 4. They determined he had an infection in his finger. He did not report the incident to his employer until July 6.

The EE was changing a screen panel on processing plant. He threw the old panel over the plant hand rail. The panel fell back. It hit the EE, knocking his hard hat off & hitting him on the right side of his head near his temple. The EE finished his shift, but felt dizzy. The next day, he still felt dizzy, so he went to the doctor. The doctor determined the EE suffered a concussion.

EE was working in a van, roll-up door was open part way. Was climbing in. Part of body & one leg were in,& was pushing himself the rest of the way in. Foot slipped, & scraped his right shin on the edge. Continued working. That night, noticed a big bruise on his leg, and it was swollen. The next day, it was worse, so he went tot the ER. He had a strep infection in his skin.

EE was doing EQ. maint. He was pulling out an anvil in the VSI when he felt something pull in his groin area. He went to the Dr. on 10/21. They referred him to a surgeon that appt. is 11/7. We will know more at that time as whether he needs rest or surgery, if he will be off work and how long he may be off.

EE was cleaning out scale. As he was putting the top doors back on, he felt a "pop" in his left elbow, followed by pain and cramping in his left bicep.

Employee was repairing a primary crusher, when he strained some muscles in his groin. He saw the company doctor, who diagnosed the injury as a hernia. He was referred to a surgeon. NOTE: The employee did not report the injury to the employer until Sept 19

EE was changing screens when he pulled some muscles in his stomach area. He waited a couple of days to see if it would improve. It didn't, so he went to see the doctor on July 26th. He was diagnosed with an umbilical hernia. It is not restricting him, so he decided to wait until this fall to have it reassessed and to schedule surgery.

Employee was performing maintenance work on a bulldozer. He was removing the belly pan when it swung down, pinching his right hand between the pan and the track. He broke his right ring finger and cut the palm of his right hand.
